Insurance Plans We participate with a variety of different HMO s, PPO s and indemnity plans. After your first visit for the pregnancy we will verify your benefits and precertify your pregnancy with your insurance company. At this time, we will find out if there is any portion of the prenatal care that will be your responsibility. If there is a balance due from you, we will send you a bill showing an estimate of the charges as stated by your insurance company. This balance must be paid by your sixth month of the pregnancy. Please note: The amount you will be paying applies to the delivery fee only. Any other charges such as ultrasound, initial visit, non-stress test, etc., are separate fees and may not be covered by your insurance company. You are responsible for any fees not covered by your insurance company. 059-011 (3-12) PAGE 2 OF 5

Ultrasounds It is the office policy to perform an ultrasound of your baby, in our office, when you are in the tenth week of your pregnancy. The purpose of this is to make sure that your pregnancy is viable, to verify your due date and to rule out any early problems that may develop. This is not related to the Nuchal Translucency Scan, which is recommended to be completed between your 11th and 13th week of pregnancy, which will be discussed with you by the physician These will most likely not be the only ultrasounds that will be ordered during the course of your prenatal care. It is impossible for us to know whether or not your insurance company will pay for this ultrasound or any follow-up scans that may be needed. Most insurance companies do cover at least one ultrasound during a pregnancy and many cover more than one. If you are concerned about whether or not your ultrasounds are covered, please contact your insurance company. We cannot be held responsible for any ultrasound (or any other test that the doctor feels is necessary) that is denied by your insurance company. If precertification or a referral are necessary, for any outside testing that is ordered by the doctor, we will assist you in obtaining it, but it is ultimately your responsibility to know what is and is not covered by your insurance company. Our ultimate goal is to maintain the health of both you and your baby. If you have any questions about any insurance issues or any other concerns, please ask to speak to a member of the office staff. 059-011 (3-12) PAGE 5 OF 5
