Congratulations and welcome to our practice! This is a very exciting time for you and your family and we would like to help in any way possible. Whether this is your first pregnancy or you already have other children, we hope to make your journey through pregnancy as easy and comfortable as possible. We have put together a comprehensive package for you including hospital preregistration forms, insurance information, disability information and other pertinent items that we hope are of interest to you. Of course if there are concerns or questions that you have that are not answered in the packet, please contact the office. Any member of our staff will be happy to assist you in any way possible. Our ultimate goal is to help you have an enjoyable and healthy pregnancy. Again from all of us, congratulations! We look forward to caring for you during your pregnancy. 059-011 (3-12) PAGE 1 OF 5

Insurance Plans We participate with a variety of different HMO s, PPO s and indemnity plans. After your first visit for the pregnancy we will verify your benefits and precertify your pregnancy with your insurance company. At this time, we will find out if there is any portion of the prenatal care that will be your responsibility. If there is a balance due from you, we will send you a bill showing an estimate of the charges as stated by your insurance company. This balance must be paid by your sixth month of the pregnancy. Please note: The amount you will be paying applies to the delivery fee only. Any other charges such as ultrasound, initial visit, non-stress test, etc., are separate fees and may not be covered by your insurance company. You are responsible for any fees not covered by your insurance company. 059-011 (3-12) PAGE 2 OF 5

Cord Blood Collections Services We are happy to provide cord blood collection services for our patients. This procedure is done at the time of delivery. It is the responsibility of the patient to make all arrangements for this procedure with the company of their choice. The following are four companies that provide this service: Cord Blood Registry 888-267-3256 ViaCord 800-998-4226 CorCell 215-864-0400 Life Bank 877-543-3226 If you would like to have this procedure done, please discuss this with us. **Please note that there is a fee charged by this office for the collection of cord blood cells that must be paid by your 24th week of pregnancy.** Thank you. 059-011 (3-12) PAGE 3 OF 5

Ultrasounds It is the office policy to perform an ultrasound of your baby, in our office, when you are in the tenth week of your pregnancy. The purpose of this is to make sure that your pregnancy is viable, to verify your due date and to rule out any early problems that may develop. This is not related to the Nuchal Translucency Scan, which is recommended to be completed between your 11th and 13th week of pregnancy, which will be discussed with you by the physician These will most likely not be the only ultrasounds that will be ordered during the course of your prenatal care. It is impossible for us to know whether or not your insurance company will pay for this ultrasound or any follow-up scans that may be needed. Most insurance companies do cover at least one ultrasound during a pregnancy and many cover more than one. If you are concerned about whether or not your ultrasounds are covered, please contact your insurance company. We cannot be held responsible for any ultrasound (or any other test that the doctor feels is necessary) that is denied by your insurance company. If precertification or a referral are necessary, for any outside testing that is ordered by the doctor, we will assist you in obtaining it, but it is ultimately your responsibility to know what is and is not covered by your insurance company. Our ultimate goal is to maintain the health of both you and your baby. If you have any questions about any insurance issues or any other concerns, please ask to speak to a member of the office staff. 059-011 (3-12) PAGE 5 OF 5
