Executive Summary The past four years have seen a steady rise in hospital turnover rates. The current rate is 17.2%, up from 13.5% in 2011. Based on this study, the profile of a hospital with the lowest turnover rate is a for-profit hospital with over 500 beds located in the South East. Conversely, a Government Acute Care facility with 200-349 beds and located in the South Central region should experience the highest degree of turnover. Similarly, the turnover rate for bedside RNs also increased during this same time period to 16.4%, up from 11.2% in 2011. Registered Nurses working in Pediatrics and Women s Health recorded the lowest rate, while nurses working in Med/Surg, Emergency and Behavior Health experienced the highest. Certified Nursing Assistants (CNAs) exceeded all other positions with a 23% turnover rate. Advanced Practice Nurses and Allied Professionals were the bright spot in the survey and recorded rates lower than the hospital average. The cost of turnover can have a profound impact on the already diminishing hospital margin and needs to be managed. According to the survey, the average cost of turnover for a bedside RN ranges from $36,900 to $57,300 resulting in the average hospital losing $4.9M $7.6M. Each percent change in RN turnover will cost/save the average hospital an additional $379,500. RN vacancies continue to trend negatively with 24.2% of hospitals reporting a vacancy rate of 10% or greater. Currently, the RN Vacancy Rate is 7.2%. When viewed with the RN Recruitment Difficulty Index, averaging to 85 days, it is clear that the RN labor market continues to tighten its belt which results in excess labor cost utilization. The greatest potential to offset margin compression is in the top budget line item (labor expense). Recapturing lost productivity, controlling contract labor and excess overtime can help offset this compression. While an overwhelming majority (89.1%) of organizations view retention as a key strategic imperative it is not evident in operational practice/planning. Although viewed in such a strong light, just over a third or 36.4% have a formal retention strategy. As seen over the past year, as the economy grows so too will turnover. Overview of Survey Participants In January, 2015, over 3,100 invitations were sent to healthcare facilities across the nation to participate in the National Healthcare Retention Survey. To maintain consistency and integrity, all facilities were asked to report data from January, 2014 through December, 2014, and for systems to report each hospital separately. I am pleased to announce that 141 facilities responded, resulting in a 24.7% increase from the previous year. In total, this survey covers 468,706 healthcare workers and 113,622 Registered Nurses. The following tables and graphs identify the region, healthcare organizational classification and bed size of the respondents. REGION Responses were received from thirty-seven (37) states. To identify trends and establish regional benchmarks, the data was split into five geographic regions, as indicated by the following matrix. All regions are well represented. The participation rate for the South East and South Central regions continue to be strong and account for over half (52.4%) of the respondents. ORGANIZATIONAL CLASSIFICATION Healthcare Organizational Classification was requested to maintain the focus of the survey. Acute care facilities were further delineated by ownership. As consistent with previous years, the overwhelming majority of responses (75.2%) were from Non-Government/Non-Profit Acute Care Hospitals. For-Profit and Government owned facilities made up 16.3% and 7.1% of the responses, respectively. Results for these hospital classifications will be provided. However, given the participation level for Long Term Acute Care and Behavioral Health, we are unable to report findings for these institutions. Although all groups saw an increase in participation, it is important to note that there was a significant jump by hospitals with more than 500 beds. This explains the leap in covered workers. Survey Findings - TURNOVER This section records the results of the survey. All findings are reported in the aggregate, with no individual hospital identifying information provided. Since organizations track and report turnover differently, it is important to establish a consistent methodology. To this end, raw data was collected on all employee terminations, whether voluntary or involuntary. Temporary, agency and travel staff were specifically excluded. Also, this survey does not measure internal terminations or transfers. According to the findings, hospitals continue to be split on which employment classifications are included when calculating turnover. By a slight majority, 50.9% of hospitals only include full time and part time employment classifications when reporting turnover. The remaining hospitals include all employment classifications, such as full time, part time, per diem, prn, casual, occasional, etc. when reporting turnover. Given this even split, respondents provided data on all employees and for full/part time staff only. For comparative purposes, we will adjust for this distinction and report TOTAL Turnover, which includes all employee classifications, and we will report FULL/PART TIME Turnover, which only includes these two classifications. HOSPITAL TURNOVER The following graph illustrates the turnover rate for hospital employees since 2010. Last year saw a continuation in the upward trend in turnover. This climb is expected to continue as the economy strengthens. Presently, hospital turnover ranges from 8.8% to 30.0%. The national average TOTAL hospital turnover rate is 17.2%, a.7% increase from 2013. Hospitals that only measure full/part time terminations reported a 1.6% increase to 15.7%. The number in parenthesis reflects the percent change from 2013. For Profit hospitals with more than 500 beds and located in the South East tend to have a greater retention level. Conversely, the profile of a hospital with the highest turnover is a Government Acute Care facility with 200-349 beds and located in the South Central region. 4

With the exception of the South East, all regions experienced an increase in turnover from the prior year. The percent change from 2013 ranges from -4.5% to 5.4%. The South East reported the lowest turnover rate at 14.2%. In addition to the South East, turnover rates for hospitals in the North Central region were also below the national average. Although the West experienced the highest percent change, the South Central region recorded the highest turnover at 22.4%. When sorted by ownership, For-Profit hospitals saw a dramatic decrease in overall turnover and posted the lowest rate. Turnover at Non-Profit entities grew at a faster pace than the national average and posted rates just below the national norms. Government owned facilities exhibited the greatest increase and posted turnover rates well above the national norm. Upon review of turnover by bed size, only hospitals with greater than 500 beds were below the national average. Hospitals with under 200 beds experienced the smallest change, while facilities with between 200 and 349 beds had the highest turnover. Participants were asked to select from a list of 20 common reasons. Relocation, personal reasons (caring for a child/parent, marriage, disability, etc), and career advancement continue to be the dominant drivers. The top 5 reasons employees voluntarily resign, in descending order, are: relocation, personal reasons, career advancement, salary and retirement. Retirement continues to remain in the top five. With 3 million baby boomers reaching retirement age each year for the next 20 years, expect to see this as a major driver of turnover. Commute/Location, education, scheduling, workload/staffing ratios and immediate manager rounded out the top ten. 5

RN VACANCY RATE The vacancy rate for Registered Nurses continues to rise and currently stands at 7.2%. A year ago, this stood at 6.7%. Thirty-four percent (34.3%) of hospitals reported a RN vacancy rate of less than 5%. This is a 5.7 point drop from 2014 and a 25 point decrease from 2012. In 2012, 59.5% of the hospitals indicated a vacancy rate of less than 5%. This rightward shift, along with the RN Recruitment Difficulty Index, (see page 7) is a clear indication that the RN labor shortage has returned and is becoming more intense. The vacancy rate of 5.0% to 7.49% saw the greatest increase to 25.7% of the respondents. Of significant concern is that 24.2% of all hospitals have a RN vacancy rate higher than 10%. This is up from 4.8% in 2012. As the economy improves, as RNs no longer delay retirement, as RNs reconsider travel nursing, as part time RNs take less shifts and as the demand for RNs increase expect the vacancy rate to further deteriorate. When the labor market tightens, hospitals have historically sought to bridge the gap by utilizing overtime, agency staff and travel nurses. All of which are costly strategies and can lead to issues with quality, safety, physician satisfaction, employee satisfaction and the patient experience. The greatest potential to offset margin compression is in the top budget line item (labor expense). When patient volume is flat; when Medicare/Medicaid is squeezed; when commercial insurance rates decrease or are bundled; when PPACA assesses a readmission penalty; recapturing lost productivity, controlling contract labor and excess overtime can help offset this compression. RN RECRUITMENT DIFFICULTY INDEX The RN Recruitment Difficulty Index gauges the average number of days it takes a hospital to recruit an experienced RN. Participants were asked to identify the range which best describes the time to fill an experienced RN in the following specialties. This year saw an increase in the Difficulty Index for all RN specialties, with a larger number of hospitals indicating that they can t find experienced RNs. In fact, the average time to recruit an experienced RN ranged from 53 to 110 days, pending specialty. Last year, this ranged from 36 and 97 days. The following graph illustrates the average number of days it took to recruit an experienced RN by specialty. The yellow line is the 2015 RN Recruitment Difficulty Index and represents the average time to fill an experience nurse vacancy regardless of specialty. Currently, this stands at 85 days. Although Med/Surg RNs were hired at a faster rate, it still took over 2 months (68 days) on average to fill. This represents a 33.3% increase from the prior survey. Specialty RN positions posted higher than the index and went vacant for three months before being filled. RNs in Surgical Services continue to be the most difficult to recruit. On average, it took 88 to 110 days to fill this position, with the average being 95 days. This resulted in an 18% increase from the prior year. Recruiting Emergency Room RNs was equally as difficult and experienced a 32.3% jump in time to fill. Based upon this information, it is easy to understand the magnitude of the shortage and the challenge facing many recruitment functions. BEDSIDE REGISTERED NURSE TURNOVER This section will follow the same format as Hospital Turnover. The following graph illustrates the turnover range for bedside RNs since 2010. Similar to hospital turnover, last year saw a continuation in the upward trend in turnover. This upward trend is expected to continue, signifying a tightening of the RN labor market. Currently, turnover for bedside RNs ranges from 9.8% to 39.8%. The national average TOTAL RN turnover rate is 16.4%, a 2.2% increase from 2013. Hospitals that only measure full/part time terminations reported a 1.5% increase to 14.0%. The overwhelming majority (83%) do not track this cost. Based upon feedback, the average cost of turnover for a bedside RN ranges from $36,900 to $57,300 resulting in the average hospital losing $6.2M. RN turnover will cost a hospital from $4.9M $7.6M. Each percent change in RN turnover will cost the average hospital an additional $379,500. Whereas the cost of turnover can range to two times annual salary for professional positions, this conservative figure represents a tremendous drain on profit. The following table reports the average TOTAL Turnover and FULL/PART TIME Turnover by region, hospital ownership and bed size for staff RNs. The number in parenthesis reflects the percent change from 2013. From a national perspective, Total RN turnover increased 2.2% to 16.4%, while Full/Part Time RN turnover increased 1.5% to 14.0%. Non-Governmental hospitals located in the South East and North Central regions experienced the lowest turnover. The profile of a hospital with the greatest RN turnover is a Government Acute Care facility located in the South Central region. Compared to the previous survey, only the South East region realized a decrease in turnover. By far, the West experienced the greatest increase of over 7%. The South East and North Central regions both posted RN turnover rates below the national average. The South Central continues to experience the highest RN turnover rate. When sorted by ownership, RN turnover at Government-Acute Care hospitals experienced the highest rate and exceeded the national average with a turnover rate over 19%. Non-Profit entities were at the national average. 8

Upon review of RN turnover by bed size, the results were inconsistent based upon methodology. Hospitals with less than 200 beds reported the highest and lowest RN turnover depending on whether a facility uses a Total or Full/Part Time approach, respectively. Participants were asked to select from a list of 20 common reasons. For the second consecutive year, relocation is the top reason why nurses leave. Moving up three spots to number four is retirement. Given the aging workforce, retirement is expected to be a top driver of nursing turnover. Rounding out the top 10 reasons why RNs voluntarily resigned, in descending order, are: relocation, personal reasons, career advancement, retirement, education, salary, commute/location, scheduling, workload/staffing ratios and unknown. 9

REGISTERED NURSE TURNOVER BY SPECIALTY The following graph compares the average RN turnover rate by nursing unit for 2014. The solid yellow line represents the current turnover rate for RNs (16.4%). By far, Behavior Health nurses were the most mobile and exceeded the national average. Also exceeding the national average were nurses in the Emergency Room, Med/Surg, Telemetry, Step Down and Surgical Services. Pediatrics and Women s Health RNs were more stable with turnover rates well below the national average. Pediatric nurses have shown the most stability over the years. Compared to 2013, Step Down, Women s Health and Med/Surg all experienced a decrease in turnover of -4.9%, -3.7% and 3.3%, respectively. All other specialties posted a gain from 0.2% in Pediatrics to 11.5% in Behavior Health. In the past two years, Behavior Health turned over 49.2% of their RN staff. Nurses working in Emergency Services, Med/Surg and Step Down have consistently turned at a rate higher than the national average. Currently, those specialties are turning at 21.7%, 20.7% and 18.5%, respectively. Managing retention should be a strategic imperative, particularly as turnover continues to creep up. Without intervention, these areas will turn over their RN staff every 2 to 5.4 years. ADVANCE PRACTICE AND ALLIED HEALTH TURNOVER The following graph compares the average turnover rate for advance practice and allied health personnel in an acute care setting. The solid yellow line represents the current turnover rate for acute care hospitals (17.2%). New to this year, is the inclusion of Medical Technologists, Radiologic Technologists and Certified Nursing Assistants. All advance practice nurses and allied health professional positions reported turnover rates below the hospital average. Certified Nursing Assistants posted a turnover rate above the hospital average at 23%. In comparison to 2013, the majority of advance practice and allied health professionals reported a decrease in turnover. This decrease ranged from -1.7% for Pharmacists and Physical Therapists to -11.9% for CRNA s. Respiratory Therapists and Physician Assistant s each saw a modest increase of 0.5%. RETENTION STRATEGIES The purpose of this section is to understand the relative importance of retention from a strategic perspective and identify operational programs hospitals employ to protect their human capital investment. An overwhelming majority of hospitals (89.1%) view retention as a key strategic imperative. However, it is not evident in operational practice/planning with just over a third (36.4%) reporting that they have a formal retention strategy. This is 7% lower than last year. Conclusion The healthcare industry is experiencing a paradigm shift. The ever expanding healthcare rolls, the aging population, the mandate on quality & safety, the squeeze in reimbursements, the competition for patient volume, the shift in the delivery of care, the shortage of physicians, nurses & allied professionals, and the PPACA are all stressing the industry. In this flux state, the labor force has responded. Healthcare workers have become more mobile with hospital and RN turnover on the rise, and, as the labor market expands expect retention rates to further deteriorate. Compounded by the increasing Recruitment Difficulty Index, CNOs and CHROs will experience higher RN vacancy rates. Each of which are a drain on hospital resources and the already diminishing margin. A bright spot in the survey was the increase in retention rates for advance practice RNs and allied health professionals. The value hospitals place on their people will have a direct correlation to their commitment, confidence and engagement. Enhancing culture and building programs to reinforce these values is critical to driving retention. Hospitals believe that retention is a key strategic imperative, yet are slow to translate this into a formal strategic plan. Focus on strategies that enhance culture and eliminate those that do not. To strengthen the bottom line, hospitals need to build retention capacity, manage vacancy rates, bolster recruitment initiatives and control labor expenses. Breaking through the myopic ways of hiring travel and agency staff to band-aid the issue or utilizing excessive overtime which taxes the staff, the quality and the patient experience is a start.
