New Horizons of the Treasure Coast In 1958, a small group of citizens who were concerned about the lack of mental health services in our community came together and created the Indian River Mental Health Clinic in Ft. Pierce, now known as New Horizons of the Treasure Coast, Inc. Through the years, the organization has evolved to meet the ever changing needs of the community and in the process has experienced many changes including names and locations. New Horizons of the Treasure Coast and Okeechobee, Inc. has become the primary provider of behavioral health services for adults and children in four counties, serving a population of more than 603,000. Through the years, New Horizons has also become one of the largest employers throughout the Treasure Coast and Okeechobee with more than 350 employees. The programs at New Horizons are available to all residents and visitors of Indian River, Martin, Okeechobee, and St. Lucie Counties. Services and treatment for mental health and substance abuse are provided at the 104 bed main campus, 4500 W. Midway Road in Ft. Pierce, and in outpatient centers in each county. The main criteria for receiving services is need. Individuals may voluntarily request treatment for mental health or drug and alcohol abuse. The agency also operates in accordance with Florida's mental health and substance abuse laws to provide evaluation and treatment to individuals on an involuntary basis. New Horizons provides services through a contract with the Florida Department of Children and Families, as well as a variety of local and state grants, private donations, third party payments and client fees. Client fees are assessed on a sliding scale, based on income and family size. Accepted methods of payment include most insurances, Medicaid and Medicare. New Horizons of the Treasure Coast, Inc. is a private nonprofit 501c (3) corporation. The organization is governed by a local Board of Directors representing a cross section of the community. The President/CEO, employed by the Board, has responsibility for overall administration and leadership of the organization. The agency is licensed and regulated by the Florida Department of Children & Families (DCF) and the Agency for Health Care Administration (AHCA). The organization is also accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) for 19 programs. Accommodations are provided for disabilities. Interpreter Services are available for individuals who are deaf and/or hard of hearing. Page 3

Hallmark of Quality CARF is an international standard-setting accrediting body that promotes and advocates for the delivery of quality rehabilitation services. A CARF three year accreditation was awarded to New Horizons for 19 programs. PROGRAMS AND SERVICES New Horizons of the Treasure Coast provides numerous programs and services to support the men, women and children in our community who live with mental illness and substance abuse. All programs are based on hope, recovery and resiliency. New Horizons has made a commitment to provide quality care for our clients and to help them on their road to recovery. EMERGENCY SCREENING CENTER The Emergency Screening Center is available for children and adults who are experiencing a mental health or substance abuse crisis. The Center is available for residents of Indian River, Martin, Okeechobee and St. Lucie Counties. Services include evaluations for mental illness, serious emotional disturbances, and chemical dependency in order to determine the appropriate level of care. If a more extensive evaluation is required, individuals may be admitted to the Adult Crisis Stabilization Unit or Detoxification Unit either voluntarily or by legal order. Referrals to other less restrictive services are made when appropriate. MOBILE CRISIS RESPONSE TEAM The Mobile Crisis Response Team provides immediate on-site assessment, crisis intervention, therapy, linkage and referral for people who are experiencing crisis 24 hours a day, 7 days a week. Services are accessed by a referral from law enforcement officials, emergency room staff, or school officials. Services also include follow-up as needed to promote crisis resolution, evaluation and arrangement for inpatient hospitalization if necessary. Mobile Crisis services are provided by trained mental health professionals in an effort to provide safe alternatives to involuntary admission to a treatment facility. ADULT STABILIZATION UNIT (ASU) The Crisis Stabilization Unit is a 30 bed unit that provides emergency and acute care services for people experiencing a mental health crisis. The Unit s primary function is the evaluation, diagnosis and stabilization of mental illness and/or co-occurring substance abuse disorders. This is accomplished by providing a protective environment, medication, counseling and discharge planning with the client s family and social support system. Services are available to all adult residents of Indian River, Martin, Okeechobee and St. Lucie Counties. Clients are accepted in compliance with the State of Florida and the Department of Children and Families regulations. Page 5

DETOXIFICATION UNIT (DETOX) The Detoxification Unit is a 12 bed unit. Services are available to all adult residents of Indian River, Martin, Okeechobee and St. Lucie Counties. The Detox Unit is a medical, non-hospital service for adults who are experiencing acute symptoms from substance abuse. The Detox Unit provides necessary medication to prevent and manage withdrawal symptoms. It also provides medical and psychosocial evaluation, diagnosis, and treatment to establish motivation for continued sobriety. INDEPENDENCE RECOVERY CENTER (IR) Independence Recovery is a 6 bed unit. This 28 day treatment program is for individuals in need of substance abuse treatment. Detoxification must be completed prior to admission. Emphasis is on developing skills utilizing the 12 step model to remain clean and sober. Treatment for co-occurring disorders (mental health and substance abuse) will be provided as necessary. Referrals are accepted from community providers, Mental Health Court or jail reentry as well as self and family. SHORT-TERM RESIDENTIAL TREATMENT (SRT) Short-Term Residential Treatment is a 20 bed adult rehabilitation program that provides intensive psychosocial services to men and women with severe mental illness and/or cooccurring disorders. The average length of stay is 30-90 days. The goal of this comprehensive program is to re-integrate the client into the community, focusing on recovery. Clients are referred to this program from crisis units. TRANSITIONAL GROUP HOME (TGH) The Transitional Group Home engages individuals with severe and persistent mental illness with major skill deficits in activities of daily living and independent living and in need of intensive staff supervision, support and assistance. The Transitional Group Home employs a philosophy of recovery that emphasizes the potential of all individuals to recover from the challenging impact of psychiatric illness. Our system of care is recovery-oriented, consumer directed and family/support centered. The Transitional Group Home provides room and board in a supervised setting with the goal(s) to empower, motivate and teach individuals to take responsibility for their recovery teach and encourage adaptive coping and life skills, gain independence, integrate and foster success in the community for all individuals. The Transitional Home Project has access to a large variety of services we offer, including psychiatric management, case management, emergency crisis intervention, crisis stabilization, short term residential treatment, substance abuse services and primary medical care. INJECTION CLINIC The Injection Clinic serves clients living with schizophrenia who find that Clozapine is the most effective medication for managing their symptoms. The Injection Clinic follows a drug regime that is tightly monitored according to guidelines established by the FDA. Clients are referred from hospitals, private physicians, mental health centers and patients. Page 6

OUTPATIENT THERAPY SERVICES Outpatient Therapy Services provide mental health, and substance abuse services for men and women in our four county areas. Clients may be referred by family, a community agency, the courts, or may be self-referred. Comprehensive social, psychological and psychiatric evaluations are provided. Based upon the evaluation and diagnosis, the appropriate treatment will be provided for adults, children and families. ADULT CASE MANAGEMENT Targeted Case Management provides coordination of services to adults with a mental illness and who may also have a co-occurring substance abuse disorder. A wide range of therapeutic interventions are provided to individuals who need multiple services and supports, and who lack a natural support system to assist them in their recovery. Services may include; assistance with securing and maintaining housing, referrals to internal and external services, education to family and significant others to facilitate a better understanding of mental illness and/or co-occurring disorders. Ongoing monitoring is also provided to evaluate the need for interventions to prevent relapse and maintain the individual in a community setting whenever possible. INTENSIVE CASE MANAGEMENT Intensive case management services are available for individuals requiring more frequent interventions and a higher level of support to maintain stability. These individuals have experienced multiple inpatient hospitalizations or are displaying behaviors putting them at risk. Services are provided by a team of individuals and a staff member is available 24/7 to meet the needs of clients. Smaller caseload sizes allow for increased oversight, support and assistance. Services are provided primarily in the home or community setting with the goal of reducing inpatient and/or other out of community placement. FLORIDA ASSERTIVE COMMUNITY TREATMENT (FACT) FACT is an evidenced-based program that provides intensive and comprehensive community treatment, rehabilitation, and support services to adults with severe and persistent mental illness. FACT operates 24/7, and functions with a multi-disciplinary team approach. FACT provides services in homes, at work, and in community settings for St. Lucie and Indian River County. The primary goal of this program is to lessen or eliminate debilitating symptoms of mental illness, meet basic needs, promote recovery, and enhance our member s quality of life. Individuals are referred to FACT by case managers and other mental health providers. PSYCHOSOCIAL REHABILITATION (PSR) PSR are recovery-oriented services to assist individuals with learning, relearning or improving skills necessary for independent living and employment. Services focus on the management of barriers created by disabilities & a variety of life domain issues, including: medication management; pre-employment; social network enhancement; health maintenance skills; and use of community resources. Improved life management allows the individual to focus on recovery. Page 7

PSYCHIATRIC MEDICAL SERVICES Psychiatric Medical services are provided to adults, children, and adolescents. These services consist of crisis intervention, diagnostic evaluation, medication management, and other treatment recommendations; such as inpatient or outpatient treatment. Inpatient services provide psychiatric stabilization on the ASU, residential care/treatment, rehabilitation on the Short Term Residential unit, and chemical dependence detoxification. Discharge from an inpatient unit ideally results in seamless transition into the community with referral for follow-up care. Outpatient services are directed towards maintenance of the individual in the community, a focus on recovery and resilience, and prevention of relapse. These services are rendered by physicians, nurse practitioners, and licensed mental health providers. PRIMARY CARE CLINIC The Primary Care Clinic provides services to our adult clients to promote educate and prevent basic health issues, while offering diagnostic evaluation for referral. These services are rendered by nurse practitioners and physicians assistants with physician oversight & supervision. Health conditions targeted include: weight management, chronic disease management, diabetes, and smoking cessation as well as other primary care needs. This coordinated system combines medical and behavioral services to address the whole person, not just one aspect of his or her condition. MENTAL HEALTH COURT New Horizons is participating in Mental Health Court, an innovative program in St. Lucie County that addresses the needs of individuals with mental illness who are charged with a crime. This successful program is a collaboration of the judicial system, the Public Defender s office, the State s Attorney s office and area mental health and substance abuse treatment providers. The judicial system has created a specialized court docket for defendants living with mental illness. Mental Health Court substitutes a problem-solving model instead of traditional court processing. Eligible participants are identified through referrals from the State s Attorney and the Public Defender with participation being voluntary. Professionals from New Horizons work with the defendants to help them navigate the judicially supervised treatment plan and obtain the necessary services and treatment in the community. Rather than remaining in the jail system, the participants receive care more appropriate to their needs. The reward for successfully completing their customized treatment plan is having their charges dismissed. FORENSIC IN-JAIL MENTAL HEALTH SERVICES New Horizons contracts with Indian River County to provide direct intervention to meet the needs of emotionally disturbed adults held in correctional facilities. Referrals are made by the penal system. The direct services consist of psychiatric evaluations, psychosocial assessment, treatment planning and counseling with the individual prescription monitoring and management. Page 8

FORENSIC SUBSTANCE ABUSE INTERVENTION PROGRAMS New Horizons contracts with Indian River County and Martin County to provide Substance Abuse Intervention Programs for inmates. The programs consist of structured services targeted toward individuals at risk. The programs focus is on reducing those factors associated with the onset or early stages of substance abuse and related problems. Substance abuse intervention programs, aftercare, and counseling are available in the contracted jail sites. Referrals are made by individual inmate request, forensic mental health, and by correctional staff. The intervention programs: Steps to Recovery in Martin County Jail, and Steps to Freedom in Indian River County Jail consist of 6-10 week closed groups and are cyclical. Group work is based on the Hazelden Mind-Body approach, 12-step influence, and addictive brain process. Public defenders and state attorney offices may be contacted when appropriate to coordinate court ordered evaluations or intervention treatment. Page 9

CHILDREN AND ADOLESCENT MENTAL HEALTH AND SUBSTANCE ABUSE SERVICES New Horizons offers a variety of treatment programs and services to support the children and adolescents in our community who are experiencing mental health challenges. We firmly believe that by providing early treatment and intervention, children will have an opportunity to grow up and lead productive lives. As with all of our programs, services are provided by licensed and certified physicians, nurses and mental health professionals. Services are available for children and their families in Indian River, Martin, Okeechobee and St. Lucie Counties. Children and adolescents may be referred by family, school, courts, self, or community agencies. CHILDREN S CRISIS STABILIZATION UNIT (CCSU) The Frances Langford Center for Children at New Horizons The Children s Crisis Stabilization Unit (CCSU) is a 20 bed integrated Mental Health Crisis Stabilization Unit and Addictions Receiving Facility (ARF). Services are provided 24 hours a day, 7 days a week for children ages 6 through 17 who have mental health and/or substance abuse issues and need crisis intervention. The length of stay is usually 3-7 days. CHILDREN S OUTPATIENT SERVICES Services include diagnosis, evaluation and treatment. Based upon the evaluation and diagnosis, appropriate treatment will be provided. Treatment may consist of individual, group or family counseling, case management, psychiatric evaluation and medication management. Many of the services are provided in home or school, based on needs. CHILDREN S CASE MANAGEMENT The Children s Case Management Team provides support to the child and their family. The Team helps to link families to community resources and advocates for the family when needed. Case Managers serve as coordinators to the children and their families who are in need of multiagency services. They provide services in the home, school and the community. THERAPEUTIC BEHAVIORAL ON- SITE SERVICES (TBOS) The TBOS program is designed as an intensive intervention to assist children who are experiencing severe emotional problems. The program is offered in the child s home and in the child s school setting and includes input from parents, teachers and school officials. This program provides therapeutic services such as teaching problem solving skills, anger management skills and other treatment methods as needed. These services are designed to maximize the child s strengths and reduce behavioral problems in school and at home. STRENGTHENING FAMILIES Strengthening Families is a family skills training program that serves families in St. Lucie County with children ages 5 to 14. The program provides a 14 week curriculum that addresses family management issues and early and persistent anti-social behavior. Families attend classes and learn parenting skills, children s skills and family life skills. Page 10

The Strengthening Families program outcomes have shown significant improvement in several areas: parents are less likely to mistreat their children, children show reduced problem behaviors, delinquency and alcohol and drug abuse. Children have also exhibited improved social behavior and school performance. This collaborative program is funded by Children s Services Council of St. Lucie County. This program is free, provides free dinner, free baby sitting and access to a behavioral analyst. STUDENT ASSISTANCE PROGRAM (SAP) The Student Assistance Program (SAP) is a substance abuse and violence prevention program that provides a range of preventive strategies to selected elementary, K-8 and middle school students in Indian River, Okeechobee and St. Lucie Counties. The program is provided at no cost to the schools, students or their families. Services include providing evidence-based prevention programs to students in a classroom setting. These programs educate youth on the dangers of alcohol, tobacco and other drugs as well as help youth to develop life skills to prevent future use. Some of the skills taught include goal setting, effective decision-making, effective communication, anger management and conflict resolution. Individual students with identified risk factors can also be referred to the Student Assistance Program by school staff. These students are then able to learn the life skills and strengthen their protective factors in individual prevention counseling sessions with one of the program s Prevention Specialists. During the summer the Student Assistance Program partners with summer camps in Indian River, Okeechobee and St. Lucie Counties. Evidence-based substance abuse and violence prevention curricula are then provided to all youth in the camps. The program is funded through grants with the Children s Services Council of St. Lucie and Okeechobee counties and the Department of Children and Families.
