I found a lot of value with the health coaching to help continue the prevention and development of my diabetes. What is Chicago Lives Healthy Wellness Program? Chicago Lives Healthy is a comprehensive wellness program for eligible employees and spouses/domestic partners/civil union spouses covered by the employee's health plan. The program offers a powerful but flexible approach to wellness aimed at helping eligible employees and their spouses/domestic partners/civil union spouses achieve improvements in their health and well-being and live healthier lives. Active participation on behalf of the population will help improve health and maintain high value health care benefits at a reasonable cost to employees. The program combines the latest science around Biometric Screenings and Well-Being Assessment (WBA) with effective, user friendly tools, resources and coaching services to empower you to adopt healthier lifestyle habits. By enrolling and engaging regularly with the Chicago Lives Healthy wellness program you can achieve your personal and family wellness goals so you can pursue a longer, healthier and happier life. Our Program Partner For over 30 years, Healthways has inspired more than 38 million people worldwide to achieve their personal best. By using proven techniques, quality medical information and caring health professionals, the Healthways team meets you where you are and takes you where you want to go, helping you achieve whole-body wellness. Chicago Lives Healthy a Free Benefit Every part of the Chicago Lives Healthy wellness program is free to all eligible employees and their covered spouses/domestic partners/civil union spouses who choose to participate. This includes services, programs and activities such as: Free annual Biometric Screening: A short, simple health check-up at on-site work locations, community locations, and Walgreens Pharmacies. You can also go to your health care provider to have your biometric screening completed*. Free annual Well-Being Assessment (WBA): A quick health survey that helps inform you about your health Free Health Check-In Call: A 15-20 minute phone call with a trained health professional to receive your results and ask any questions Free Health Coaching Calls: World-class resources to help you achieve your health goals for individuals with moderate to high risk health factors. Free ongoing health activities, programs and classes (including those offered on-site and online throughout the year) *Biometric Screenings done at your physician's office will be subject to your current medical plan benefits. In the pages that follow is a brief explanation of the Chicago Lives Healthy wellness program components along with the steps you need to complete to get started! Protecting Your Privacy Protecting your privacy is a top priority for Healthways and your employer. Your information will be kept confidential. Healthways may use and disclose your personal health information only as permitted by applicable law, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Healthways may disclose health information to Wellness Corporate Solutions, the screening agent, and to your employer acting as a health plan sponsor for program administration purposes only and as permitted by HIPAA. Program administration purposes may include offering and determining eligibility for Program incentives, if offered. 3

The choice to participate in the Chicago Lives Healthy Wellness Program is up to you. If you and/or your covered spouse/domestic partner choose not to participate, there is an impact to your monthly employee health care contributions. This wellness program is designed to be participation-based rather than outcome-based. All eligible employees and their covered spouses/ domestic partners need to do to participate is engage in a program of their choice and explore its services, tools and resources. In other words, if you set a goal to quit smoking but you don t actually achieve that goal, you will not be penalized. All you need to do is make the effort to try and adopt healthier behaviors. The only thing that s not free is not trying. Eligible employees or covered spouses/domestic partners who choose not to participate in the program will incur an additional $50 per non-participant increase in their monthly employee health care contribution. An increase in your monthly health care contributions will result from any of the following: Indicating during open enrollment that you choose not to participate. - If a single employee chooses not to participate: $50 per non-participant increase in your monthly employee health care contribution - If an employee and covered spouse/domestic partner each choose not to participate: $100 ($50 per non-participant) increase in your monthly employee health care contribution - If an employee chooses not to participate, but the covered spouse/domestic partner chooses to participate (or vice versa): $50 per non-participant increase in your monthly employee health care contribution Failing to complete any of the three enrollment steps within the due dates Failing to participate in ongoing quarterly wellness program activities Individuals who elect not to participate will not be eligible to enroll in the wellness program again until the next wellness open enrollment period (planned for fall of 2015). 4

STEP 1: Schedule and Attend a Biometric Screening A biometric screening is a quick and easy check-up with a certified medical professional that helps you evaluate your health and pinpoint potential health issues. Three biometric screening options are available. Choose one of the following: Please only attend one biometric screening appointment to complete Step 1. 1. Schedule and attend an Onsite Biometric Screening. (Scheduling begins on 12/15/2014 and appointments begin on 1/15/2015). Beginning December 15, 2014 you can go to www.chicagoliveshealthy.com to schedule your free biometric screening appointment at select onsite work locations and community locations such as various Chicago Public Libraries, Park District facilities and City Hall. Both you and your covered spouse/domestic partner/civil union spouse should each schedule your appointment individually on the screening self-scheduler tool located at www.chicagoliveshealthy.com. If you need assistance scheduling your appointment, you can call Healthways at 1-866-556-7671. Onsite biometric screenings begin on January 15, 2015 and all results must be received by March 8, 2015. 2. Print out a personalized physician form beginning 12/15/2014 and make an appointment with your doctor. Results of your biometric screening completed by your physician must be submitted on your personalized physician form available at www.chicagoliveshealthy.com. If your physician submits biometric results on any other type of form or attaches any test results with the form instead of completing the form, the results cannot be accepted. Healthways will accept biometric screening results if the test results are no more than six months old (no results collected prior to 6/15/2014 will be accepted). Go to www.chicagoliveshealthy.com to printout your personalized physician form. If you need assistance printing out your personalized physician form, you can call Healthways at 1-866-556-7671. Biometric screening results from a physician's office visit must be faxed in on your personalized physician form. Faxes must be received by March 8, 2015. 3. Print out a personalized Walgreens Health Test Voucher beginning 1/15/2015 and make an appointment at your local Walgreens Pharmacy. Beginning on January 15, 2015, go to www.chicagoliveshealthy.com to printout a personalized Walgreens Health Test Voucher and make an appointment at your local Walgreens Pharmacy. Walgreens Pharmacies will take walk-ins, however, you will not have to wait if you have an appointment. If you need assistance printing out your personalized Walgreens Health Test Voucher, you can call Healthways at 1-866-556-7671. Free biometric screenings at your local Walgreens Pharmacy are available between January 15, 2015 and March 8, 2015. Go to www.chicagoliveshealthy.com for a list of participating Walgreens Pharmacies. STEP 2: Complete your online Well-Being Assessment (WBA) Well-Being Assessments must be completed between January 15, 2015 - March 31, 2015. The Well-Being Assessment (WBA) is a confidential questionnaire that assesses life and environmental factors that are critical to your health and well-being and your ability to improve both. The Well-Being Assessment (WBA) has been designed by health experts and consists of 60 questions to help get a better sense of factors such as:: Your basic health history Your mental health and stress, and Any barriers to change that might keep you from moving forward All questions are simple to understand and easy to answer. It should take 20 to 30 minutes to complete the questionnaire. Beginning on January 15, 2015, you will find a link to the WBA at www.chicagoliveshealthy.com. STEP 3: Health Check-In Calls The Health Check-In Call must be completed between January 15, 2015 - March 31, 2015. The Health Advisor Check-In Call is an outbound call to you from one of Healthways Health Advisors who are certified health professionals. During this 15 minute call, the Health Advisor will review your biometric screening and your Well-Being Assessment results with you. The Health Advisor will talk with you about your health and well-being and if you wish, develop realistic health or lifestyle improvement goals with you. Quarterly Ongoing Participation Earn 12 new points each quarter by completing wellness participation activities. Quarter 2: Earn 12 points and you have met the quarterly requirement. Quarter 3: Earn 12 points and you have met the quarterly requirement. Quarter 4: Earn 12 points and you have met the quarterly requirement. 5

Quarterly Wellness Activities Beginning In April, 2015 Quarterly Points Earning Requirement Beginning April 2015, all participants in the Chicago Lives Healthy program will be required to earn 12 new points each quarter by completing wellness program activities. If you wish, you can earn all 12 quarterly at one time to make it more convenient for you! 2015 Quarterly Point Requirement APRIL / MAY / JUNE 2015 JULY / AUG / SEPT 2015 OCT / NOV / DEC 2015 Quarter 2 = 12 points Quarter 3 = 12 points Quarter 4 = 12 points Ongoing Wellness Activity Options Available to All Chicago Lives Healthy wellness program participants: Online Well-Being Connect Well Being Connect (WBC) is a dynamic online portal designed to inspire improved health and well-being. Every personalized feature is meant to help you set goals and connect you to support to achieve them. It is available to all Chicago Lives Healthy wellness program participants and includes a comprehensive suite of tools and resources. You can choose the activities that best fit your lifestyle and learning preferences. Well Being Connect is a convenient and effective way to earn 12 points per quarter. Online Activities Available in Well-Being Connect EACH ONLINE ACTIVITY COMPLETED = 1 POINT Complete 1 action item = 1 point Write 1 journal entry = 1 point Set up an exercise, weight or nutrition tracker = 1 point Read and rate a health article = 1 point Read and rate a recipe= 1 point You can earn more than one point by competing the same online activity multiple times. Telephonic Other Wellness Activities Options for those who qualify: 1 TELEPHONIC COACHING SESSSION = 6 POINTS Based on your responses to the questions you completed on the Well Being Assessment and the values presented from your biometric screening you may be selected by Healthways credentialed health professionals to participate in telephonic health coaching. Telephonic health coaching is a voluntary program, offering personal support from a health coach who can help individuals with moderate to high health risk factors overcome the challenges that stand in the way of their good health. Onsite 1 HEALTHY ACTIVITY SESSSION = 6 POINTS Chicago Lives Healthy recognizes that local and community activities, such as organized runs and walks, are great ways to improve your health and well-being. The Chicago Lives Healthy wellness program will count these types of activities toward your ongoing quarterly participation requirement. Most of us would really like the chance to change something about our lifestyle or behaviors that put our health at risk. And sometimes, just having extra personal support system in place can help overcome the challenges that stand in the way of good health. One of the best parts of the Chicago Lives Healthy wellness program is access to free, top-quality health education sessions for participants. Whether you re setting goals to lose weight, managing an ongoing lifestyle issues like quitting smoking or just want to learn more about the Chicago Lives Healthy wellness program, a health education session allows you to focus specifically on your questions, needs and goals with a credentialed health professional. ADDITIONAL OPTION AVAILABLE FOR CITY OF CHICAGO PPO PARTICIPANTS ONLY: Participate in a Care Management Program, participation is worth 12 points. If you are pregnant or have a chronic health condition such as diabetes, cardiac disease, asthma, COPD and participate in one of the care management programs offered such as Taking Control of Your Health or Telligen Maternity or Condition Management Programs, you can earn 12 wellness participation points each quarter while participating in these health management programs.
