EQUIVEST is a combination variable and fixed annuity deferred contract issued by AXA Equitable Life Insurance Company (New York, NY) and is designed to help people to contribute toward their retirement. Annuities are longterm investment products designed for retirement purposes. The Annualized Monthly Rates of Return performance shown reflects reinvestment of dividends and capital gains and deduction of all annuity contract fees and charges except the withdrawal charges and the annual administrative charge. The Standardized (SEC) quarter end performance is net of all annuity contract fees and charges including the annual administrative charge and the maximum applicable withdrawal charge of 6% in years one through six and 0% all subsequent years.
