Tools You Can Use to Navigate Aging and Disability Resources

What is the Aging and Disability Resource Center (ADRC) Initiative? A collaborative effort of the Administration for Community Living, the Centers for Medicare and Medicaid Services and the Veterans Health Administration designed to streamline access to long-term care services and supports for people who are older, people with disabilities and caregivers

What is the Aging and Disability Resource Center (ADRC) Initiative? ADRCs are known to be No Wrong Door or Single Entry Points Designed to serve as a highly visible, trusted entity that assists consumers regardless of age, income or disability

What is the Aging and Disability Resource Center (ADRC) Initiative? Important steps to implement and ensure the ADRC vision: Create a person centered-community-based environment promoting independence and dignity Provide easy access to information and one-onone counseling to assist consumers in exploring a full range of LTSS options Provide resources and services that support the needs of family caregivers Enhance integration and/or coordination of existing aging and disability service systems

Care Connection ADRC s Mission Work in partnership with organizations to assist older adults, persons with disabilities and caregivers to access a full range of long-term services and supports and make informed choices about health and well-being

Options Counseling Interactive conversation that helps individuals make decisions that suit their needs The consumer leads the process You tell us, we don t tell you want you want/need

Local Contact Agency A support for nursing home residents who utilize Medicare private pay for nursing home care Individual expresses a desire to move back to their residence Individual looking for options that will be beneficial in the home

Housing Navigation ADRC Staff helps consumers identify appropriate, affordable and accessible housing Housing Navigation can help: People with disabilities who need special housing accommodations Caregivers who are trying to identify the best options for an older adult in need of another form of housing

Respite Sometimes caregivers need a break from day-to-day care activities The ADRC provides vouchers to caregivers so they can take a break To use vouchers, caregivers must attend an educational class Vouchers can be used to: Pay a provider to come into the home Pay for another form of respite such as a camp for children

Benefits Counseling Certified Benefits Counselors help consumers understand and/or apply for services related to: Medicare Medicaid Social Security Veterans Benefits Advance Directives

Hospital Care Transitions A program designed to help people who are discharged from hospitals receive assistance from a transitions coach to prevent going back into the hospital Care Transitions Coaches located in select hospitals throughout Houston

Evidence-Based Programs A Matter of Balance Stress-Busting for Family Caregivers Chronic Disease Self-Management Diabetes Self-Management Stay tuned for more programs!
