Executive Summary: The Kansas Department for Aging and Disability Services (KDADS) invites interested parties (IP), consumers, stakeholders and advocates to respond to this Request for Information (RFI) regarding the re-procurement of the Agency and Disability Resource Center (ADRC) contract related to single point of entry for information, referral, and assistance, assessments for long-term supports and services, reporting and related assignments. Interested Parties shall respond to this RFI and submit it to the address shown in Section C below no later than 10:00AM on March 15, Electronic ( ) submission is acceptable and preferred. The responding IP will not be bound by the written response; however, because KDADS may rely upon information provided by responders when developing the Request for Proposal of the Aging & Disability Resource Center (ADRC) contract and its program requirements and implementation strategies, it is requested that the responding IP be as specific or detailed as possible. Responding IPs will not be bound to any further obligations as a result of response to this RFI. Nor will IPs be made any guarantees related to a future Request for Proposal as a result of responding to the RFI. With regard to each response section set forth in Section F and Section G, below, the responding IP is asked to answer each question as specifically and detailed as possible. B. Questions Concerning this RFI Questions requesting clarification of this RFI must be electronically submitted via to the ADRC Program Manager, no later than noon on March 1, Responses to any questions will be gathered and posted online at during the RFI process. Response Preparation Costs The cost of developing and submitting the information is entirely the responsibility of the responding IP. This includes costs to determine the nature of this RFI, preparation of the response, submitting the response and any other costs associated with this RFI. Responses should be prepared simply and economically, providing a straightforward, concise description of the information being requested in this RFI. Emphasis should be on completeness and clarity of content. 7. 5 D. General RFI Information 1. Confidentiality All information included in this RFI is confidential and only for the recipient knowledge. No information included in this document or in discussions connected to it may be disclosed to any other party. 2. Liabilities of Agency This RFI is only a request for information about potential services and supports and no contractual obligation on behalf of the Kansas Department for Aging & Disability Services whatsoever shall arise from the RFI process. This RFI does not commit the Kansas Department for Aging & Disability Services to pay any cost incurred in the preparation or submission of any response to the RFI. 3. Confidentiality & RFI Ownership RFI Ownership: All responses to the RFI will become the property of the Kansas Department for Aging and Disability Services and will not be returned. Open Records Act: Under the Kansas Open Records Act (reference K.S.A ) all materials received or created by KDADS are considered public records. These records include but are not limited to bid or proposal submittals, agreement documents, contract work product, or other information submitted by a vendor to KDADS. The State of Kansas Open Records Act requires that public records must be promptly disclosed by KDADS upon request unless those records are excluded in accordance with K.S.A (27)n (Specifications for competitive bidding, until the specifications are officially approved by the agency). 4. Marking Records Exempt From Disclosure (Protected, Confidential, or Proprietary) If you believe any of the documents you are submitting to KDADS as part of your informational material are exempt from disclosure due to patent or proprietary issues, you can request they not be released. To do so, identify which areas are confidential and the reason why. Only the specific records or portions of records properly identified will be protected and withheld for notice. Agency Overview The Kansas Department for Aging and Disability Services (KDADS) is a new agency created by the merger of programs from the Kansas Department on Aging (KDOA), the Department of Social and Rehabilitation Services (SRS) and the Department of Health and Environment (KDHE). The merger occurred under an executive order by Governor Sam Brownback in order to streamline the delivery of state services. The new department began on July 1, All of the aging and disability programs previously operated by KDOA and SRS are now the responsibility of KDADS. Some licensure programs previously part of KDHE are also incorporated into KDADS. Our commitment to providing services and choice remains the same. Our Mission: The Kansas Department for Aging and Disability Services mission is to foster an environment which; promotes security, dignity and independence, while providing the right care at the right time in a place called home. Our Vision: The Kansas Department for Aging and Disability Services envisions a community that empowers Kansas older adults and persons with disabilities to make choices about their lives. KDADS administers several Medicaid and Medicare funded programs under the authority of several federal agencies, federal laws, and federal regulations. The aging and disability programs operated by KDADS include Senior Care Act, Older Americans Act, 1915(c) waivers, Program for All-Inclusive Care for the Elderly, and Money Follows the Person grant. F. System Background 1. Federal Design What is an ADRC? The term "Aging and Disability Resource Center" means an entity, network, or consortium established by a State as part of the State system of long-term care, to provide a coordinated and integrated system for older individuals and individuals with disabilities, and the caregivers of older individuals and individuals with disabilities, that provides (A) comprehensive information on the full range of available public and private longterm care programs, options, service providers, and resources within a community, including information on the availability of integrated long-term care services, and Federal or State programs that provide long-term care services and supports through home and community-based service programs; KS Dept for Aging & Disability Services ADRC RFI Open: Due: Page 6 of 20

7 (B) person-centered counseling to assist individuals in assessing their existing or anticipated long-term care needs and goals, and developing and implementing a person-centered plan for long-term care that is consistent with the desires of such an individual and designed to meet the individual's specific needs, goals, and circumstances; (C) access for individuals to the full range of publicly-supported long-term care services and supports for which the individuals may be eligible, including home and community-based service options, by serving as a convenient point of entry for such programs and supports; and (D) in cooperation with area agencies on aging, centers for independent living, and other community-based entities [such as community developmental disability organizations (CDDOs) and community mental health centers (CMHCs)], information and referrals regarding available home and community-based services for individuals who are at risk for residing in, or who reside in, institutional settings, so that the individuals have the choice to remain in or to return to the community; Aging and Disability Resource Centers (ADRCs) are the result of a collaborative effort of Administration for Community Living (ACL), the Centers for Medicare & Medicaid Services (CMS), and the Veterans Health Administration (VHA) and are intended to streamline access to long-term services and support (LTSS) options for older adults and individuals with disabilities. The ACL, CMS, and VHA support the goal of States developing a sustainable No Wrong Door (NWD)/Single Entry Point (SEP) model for providing long-term supports and services, options counseling that streamline assessment, eligibility determination, enrollment, service initiation, data collection, and reporting across all programs and services. As States undergo health system transformation and long term care rebalancing activities, development of a sustainable infrastructure is critical to ensuring ongoing coordinated access to services. Equally important, the ADRC system helps states effectively braid public resources (e.g. Medicaid FMAP, VHA funding, Older Americans Act dollars, state revenue programs, etc.) and private resources in a way that will truly sustain these models over time. NWD systems provide information and assistance not only to individuals needing either public or private resources, but also to professionals seeking assistance on behalf of their clients and to individuals planning for their future long-term care needs. NWD systems also serve as the entry point to publicly administered long-term supports, including those funded under Medicaid, the Older Americans Act, Veterans Health Administration, and state revenue programs. Ideally, a NWD system will make it easy for people of all ages, disabilities and income levels to learn about and access the services and supports they need. 8 accomplished in order to realize the joint vision across Administration for Community Living, Centers for Medicare & Medicaid Services and the Veterans Health Administration vision of each state having a single NWD System that is a fully integrated for all populations. For these purposes, "all populations" means everyone, regardless of age, income, or disability and "all payers" means any state administered program that provides LTSS to the people that will be served by the ADRC System. States are expected to design systems that will best organize and operate the various NWD functions. Nationally, ADRC programs have taken important steps towards meeting ACL, VHA and CMS s vision by: Creating a person-centered, community-based environment that promotes independence and dignity for individuals Providing easy access to information and one-on-one counseling to assist consumers in exploring a full range of long-term support options Providing resources and services that support the needs of family caregivers Enhancing integration and/or coordination of existing aging and disability service systems. 2. Federal Design What Should an ADRC Do? ADRCs are intended to be No Wrong Door (NWD) or Single Entry Point (SEP) systems, and are designed to serve as highly visible and trusted places available in every community across the country where people of all ages, incomes and disabilities go to get information and one-on-one counseling on the full range of LTSS options. Finding the right services can be a daunting task for individuals and their family members. The current LTSS system involves numerous funding streams, and is administered by multiple federal, state and local agencies using complex, fragmented, and often duplicative intake, assessment and eligibility processes. There are more and more options for services and supports, in home, residential, and institutional settings. Individuals trying to access the multitude of new LTSS frequently find themselves confronted with a maze of agencies, organizations and bureaucratic requirements at a time when they may be vulnerable or in crisis. These issues frequently lead to use of the most expensive forms of care, including institutional care such as nursing homes or extended hospitalization, and can cause a person to quickly exhaust their own resources. Aging and Disability Resource Centers (ADRCs) are the first place to go to get accurate, unbiased information on all aspects of life related to aging or living with a disability. ADRCs are friendly, welcoming places where anyone - individuals, concerned families or friends, or professionals working with issues related to aging or disabilities - can go for information tailored to their situation. 9 services, helps people understand the various long term care options available to them, helps people apply for programs and benefits, and serves as the access point for publiclyfunded long term care. These services can be provided at the ADRC, via telephone, or through a home visit, whichever is more convenient to the individual seeking help. It is expected that states will better integrate, and in some cases, restructure and strengthen, their existing ADRC programs as they develop or enhance a No Wrong Door (NWD) or Single Point of Entry (SEP) approach to information, assistance, referral, access, assessment and options counseling for aging and disabled individuals. An Aging and Disability Resource Center can provide the following services and more: Information and Assistance Information about local services and resources o Includes referral to appropriate community supports, services and resources o Includes directing individuals to appropriate public benefits and applications Assistance in finding services to match a person s needs o Personal care assistance, in-home care and services and/or respite o Home modifications, assistive services, and devices for safety and maintenance o Health (healthy lifestyles, management of chronic conditions, dementia, etc) o Transportation (medical and non-medical) o Nutrition and home delivered meals o Housing, including senior and low income housing o Assisted Living, nursing homes and other long term care facilities and options o Financial assistance (e.g., Social Security, Medicare, Medicaid, benefit programs) o Legal issues (guardianship, power of attorney, client rights advocacy) o Abuse, neglect and financial exploitation o Mental health, alcohol and drug abuse, crisis intervention o Employment, vocational services, volunteer work o Adaptive equipment and assistive technology Long Term Care Options Counseling Information about the choices you have when making decisions about where to live, what kind of help you need, where to receive that care and help, and how to pay for it. One-on-one consultation to help you think through the pros and cons of the various options in light of your situation, values, resources and preferences. Assessment and functional eligibility determination if an individual will be eligible for public funding for long-term supports and services Assistance preparing a Medicaid application, if eligible, or a renewal application Explain program choices, community resources, and other options for long term care Referral ADRCs can help answer questions and solve problems related to benefits such as Medicare, Medicaid, Social Security, PACE, and private health insurance and refer to appropriate resources, services, supports, and programs Connect a person to wellness programs to help keep the person healthy and independent KS Dept for Aging & Disability Services ADRC RFI Open: Due: Page 9 of 20

Current Kansas Model Eleven (11) Area Agencies on Aging (AAAs) locations have operated regionally under a single ADRC contract since The three year contract ends September 30, 2015, and the ADRC contract will be up for RFP and procured in KDADS operates seven (7) Home and Community Based Services (HCBS) Programs for aging and disabled individuals for the Autism Program, Frail Elderly (FE) Program, Intellectual and Developmental Disabilities (IDD) Program, Physical Disability (PD) Program, Severe Emotional Disturbance (SED) Program, Technology Assisted (TA) Program, Traumatic Brain Injury (TBI) Program. Currently, the ADRC only completes functional eligibility assessments for the PD, TBI, and FE Programs as well as the nursing facility and traumatic brain injury rehab facility. Additionally, the CDDO serves as the functional assessor for the IDD Program, and the CMHC serves as the functional assessor for the SED program, and two other contracted entities serve as the functional assessors for the TA and Autism programs. ADRCs also assess for other federal and state programs including Senior Care Act (SCA), Older Americans Act (OAA), WORK/Working Healthy, and the Program for All-Inclusive Care for the Elderly (PACE). Additional duties include case management and services related to SCA and OAA as applicable to the AAAs. ADRCs must have a physical location and provide comprehensive call and walk-in centers capable of covering the entire regional area and accessible to all individuals to complete all functions of the ADRC. The location may include a main location, any branch or satellite locations that are clearly identifiable, available by public transportation, provides adequate parking, and have the capacity to serve individuals with disabilities. Additionally ADRCs are expected to meet the following criteria for service delivery: Routinely tracks service delivery and consumer outcomes Monitor program quality and a process to ensure continuous program improvement through the use of the data gathered such as consumer satisfaction evaluations. Establish strong partnerships with resources and programs instrumental to ADRC activities, such as Adult Protective Services (APS), State Health Insurance Assistance Program (SHIP), Senior Medicare Patrol (SMP), Benefit Outreach/Enrollment Centers. Provide sufficient staff skilled in care coordination, information, referral and assistance, benefits counseling, crisis intervention and system navigation. Be available during regular business hours including during lunch and other times that are convenient for aging and disabled individuals. Ensure streamlined eligibility determinations for public programs and serve as a single point of entry to all publicly funded long-term supports, including those funded by Medicaid, Medicare, the Older Americans Act (OAA), and other state/federal programs Provide options counseling and assistance for decision support, including one-on-one assistance, to consumers and their family members and/or caregivers. Describe and detail its outreach and marketing plan focused on establishing operating partners as highly visible and trusted places where people can turn for the full range of long-term support options as well as raising awareness in the community about long term supports and services options. Project Overview Kansas is continuing to ensure a No Wrong Door (NWD) system through integrated ADRC that encourages system collaboration for all Kansans seeking information on community supports, services and resources for aging and disabled individuals and is seeking information on potential bidders for a Request for Proposal (RFP), system design recommendations, and feedback on system operations and areas for development. The three year ADRC contract ends September 30, 2015, and the ADRC contract will be up for RFP and procured in Kansas intends to award a future RFP by fall of Kansas is developing independent ADRC regions to work collaboratively as part of a single NWD/SEP system in Kansas, and is seeking input from consumers, providers, contracted entities and stakeholders through this Request for Information. Interested Parties are encouraged to provide input on system design, operational capabilities, and collaborative efforts. 1. Introduction and Purpose of RFI In considering the intended design of a NWD system for access, information, referral and assistance through the ADRC model, Kansas is seeking information from consumers, providers and stakeholders for consideration in the upcoming RFP for a comprehensive Aging and Disability Resource Connection to public programs, community resources, and aging and disability information. The ADRC system will streamline eligibility determinations for public LTSS programs, increase access to community-based alternatives that can help avoid institutional care and preserve personal resources, as appropriate, and provide information, assistance and referral. The enhanced ADRC system will be more person-centered, more efficient, and more supportive of community living. 2. Concept Aging and Disability Resource Centers are welcoming and accessible places where individuals can obtain information, advice, and assistance in locating services. They provide a central source of reliable and objective information about a broad range of programs and services, and help people understand and evaluate the various options available. By enabling people to find resources in their communities and make informed decisions about long term care, ADRCs help people conserve their personal resources, maintain selfsufficiency and delay or prevent the need for potentially expensive long term care. The ADRC will work collaboratively with community agencies and organizations to significantly improve access to long-term services and supports through streamlined collaborative and integrated processes for service delivery, including common intake and assessment tools, referral protocols, and integrated information systems; provide options counseling and decision support, including one-on-one assistance, to consumers and their family members and/or caregivers; to help consumers assess and understand their needs, and to assist them in making informed decisions about appropriate long-term service and KS Dept for Aging & Disability Services ADRC RFI Open: Due: Page 11 of 20

12 support choices in the context of their personal needs, preferences, values and individual circumstances; identify and provide detail of formal linkages between and among the major pathways that people travel while transitioning from one setting of care to another, such as preadmission screening programs for nursing home services. 3. Population The ADRC shall serve as a resource for both individuals eligible for publicly funded services and supports and individuals with private resources. Individuals of all ages, disabilities and income levels should have access to information, assistance and referral to appropriate resources, services and programs. The populations will include older individuals and individuals with disabilities, family caregivers and other individuals planning for future longterm care services and supports needs. Currently, there are 10 Areas Agencies of Aging (AAA) subcontracted with 1 AAA to serve as the statewide ADRC. These entities serve the entire state of Kansas and provide information, assistance, options counseling, assessment and referral for a variety of programs including Older Americans Act, Senior Care Act, the WORK/Working Healthy Program, the Physical Disability Program, Traumatic Brian Injury Program, and Frail Elderly Program as well as nursing facilities. 4. Catchment Areas/Service Areas KDADS intends to enhance the ADRC system by providing regional ADRCs that work collaboratively to form a Statewide ADRC, allowing a contractor to provide proposals for more than one regional area and identify partnerships and other funding resources to ensure a robust system for information, assistance, referral, assessment and options counseling. KDADS is looking to regionalize the ADRCs, which will continue to work collaboratively as part of the No Wrong Door System. Any agency, entity, organization or consortium willing and able to perform the functions of an ADRC may bid to become an ADRC and respond to this request for information (RFI). 5. Interested Parties and Partnerships There are generally three types or levels of partner: the lead operating agency, which coordinates the fiscal management and reporting for the ADRC; operating partners, which contribute substantially to the daily functions of the ADRC by providing services through subcontracting relationships or other arrangements; and support partners which participate by offering support when needed or with particular aspects of the ADRC such as marketing, but at the minimum serve on the ADRC s steering or advisory committee and participate in the ADRC s system of expedited referral protocols. Required ADRC partners may include the local CDDO, CMHC, AAA, Children s Resource Center, CIL and other disability organizations, services and programs. 13 the lead operating agency; it does not have to be a required partners. As many potential partnerships as possible should be described in a response from an Interested Partner. Examples of community entities to consider as partners with an ADRC include: 2-1-1, the local mental health authority, centers for independent living, non-profit agencies serving older adults and individuals with disabilities, agencies serving or advocating for children with special needs, Department of Assistive and Rehabilitative Services, Adult Protective Services, area health care partners, organizations serving veterans, and other relevant community organizations, agencies, and providers. In addition, the planning process will incorporate meaningful input from key stakeholders including consumers; their advocates; Community Developmental Disability Organizations, Area Agencies on Aging; Centers for Independent Living; Community Mental Health Centers, local and county agencies including health departments; local organizations that serve or represent the interests of individuals with physical disabilities, individuals with intellectual and developmental disabilities, frail elders, children with disabilities and individuals with mental/behavioral health needs; Veteran Service Organizations; as well as service providers and other relevant public and private entities. In short, the ADRC is intended to ensure that states will meaningfully include persons who use LTSS in all aspects of the planning process. 6. Standard Operations ADRCs will be expected to operate with a level of flexibility to allow them to assume new projects, duties, and responsibilities as new programs and opportunities arise within the areas of streamlining access to LTSS, integration of services, and reducing confusion for consumers. ADRCs will work closely with KDADS to develop statewide standards and best practices, ensure quality and program integrity, and report on access, information, referral and assistance for all individuals served. All ADRCs are expected to perform the following services: 1. Information, Referral, and Assistance 2. Options Counseling 3. Medicaid Eligibility Assistance for initial and renewal applications 4. Streamlined Intake, Screening, and Eligibility Determination for Public Programs, including in-person assessments and notification of eligibility determination 5. Person-Centered Transition Support, including Nursing Facility Diversion Activities and outreach to critical pathways and health care providers, including hospitals, managed care organizations, physicians, etc. 6. Involve Consumers Populations, Partners, and Stakeholders, including ongoing crosstraining for ADRC partner agencies 7. Quality Assurance, Continuous Improvement, and reporting 8. Community Education and Marketing Activities 9. Community Transition Opportunities 10. Meet Any Future Statewide Operational Standards. 14 H. Desired RFI Outcomes and Strategy KDADS has identified the following considerations as critical elements for success (things to achieve and things to avoid). Please review these considerations to assist in developing responses to the questions of this Request for Information. KDADS has determined that the ADRC system must demonstrate a commitment to principles consistent with the following: 1. Top 3 things we must achieve for success. a. Principles of Person-Centered Planning and Independent Living: personcentered planning should enable aging and disabled individuals to live in their homes and in the community with appropriate supports and services that are person-focused and promote independent living, including person-centered transition supports. b. Collaborative Partnerships: From past work, it is clear that no one agency or network has the capacity, expertise, or authority to effectively carry out all the NWD System functions for all of the different populations that will be served. It is expected that multiple agencies and organizations at the state and local level will be formally involved in the operations of a state s ADRC system in order for the No Wrong Door model to be effective. c. Access to the Right Services at the Right Time: ADRCs should provide individuals and their families with the information, assistance and referrals they need to make informed decisions about long-term care, to find and access public and private resources in their communities, and to conserve their personal resources, maintain self-sufficiency and delay or prevent the need for potentially expensive long term care. 2. Top 3 Things We Must Avoid for Success b. Limiting options that support individuals: The system cannot meet the desires of every individual receiving services; however, it can provide clear guidance on the services and supports available for identified needs and identify solutions that support individuals c. Scope of Request: This communication provides information about the upcoming RFP and KDADS goal to support a No Wrong Door system through the Aging and Disability Resource Centers (ADRC) model in Kansas that simplifies access to information about community resources, services and programs for aging and disabled Kansans regardless of whether the person is eligible for publicly-funded benefits or programs. The purpose of the RFI is to solicit information from persons served, friends, families, providers, and other stakeholders who have used or provided I&R/A, ADRC (model/system) regarding suggested system changes and their potential impact on persons served by the ADRC and community programs in Kansas. Additionally, KDADS would like each responding IP to address each of these questions regarding the ADRC system in Kansas. 1. For Interested Parties, please address the following questions in your response: o If you are a community service provider, what services do you provide, and in which areas? Do you have plans to or would you consider expanding into other service areas to provide the same or additional services if the need arose? Do you or your organization have experience with long-term supports and services, KanCare, assessments, and aging and disability populations?
