The Bainbridge State College Associate of Science Degree in Nursing Program welcomes your interest in the Licensed Practical Nurse (LPN) Associate Degree in Nursing (ADN) advance placement track and invites applications from all qualified individuals. This is a three-semester program, aimed at persons holding a certified, unrestricted LPN license interested in advancing their career, which begins annually in the summer semester with completion at the end of spring semester the following year. The program qualifies graduates to apply to sit the NCLEX RN exam to become a Registered Nurse. The curriculum plan of the LPN RN track recognizes the licensed practical nurses education and experience as a foundation and provides academic and clinical experiences for Associate Degree nursing education. The program is both challenging and rewarding, and it is important for the applicant to recognize that the advanced placement track requires time and dedication to succeed. The TEAS placement exam is a mandatory part of the application process that must be fulfilled for the application to be considered complete. Please contact the Testing and Career Development Center at (229) 248-2579 to schedule an appointment to sit the exam; only two attempts are allowed and the highest score earned is considered. You will be required to bring a picture I.D. and $45.00 payable only by debit or credit card. Skills demonstration, drug calculation exam, and ATI Fundamentals exam are also mandatory parts of the application process and must be fulfilled for the application to be considered complete. Please submit scores from the TEAS exam and ATI Fundamentals exam with the application. Please note there is no interview process scheduled for this admission cohort. Only complete application packets submitted prior to the deadline will be considered. All required information must be included for the application to be considered. The application packet will be found to be incomplete if all required information is not submitted by stated deadline. All incomplete applications or applications received after the deadline will not be considered. Admission to the program is competitive and the number of students is limited by the number of faculty and clinical facilities available. Additionally, any student who provides false information on the Nursing Program application will be excluded from admission or if accepted into the program will be dismissed. Student Release and Responsibility Notification I understand and agree that I am bound by Bainbridge State College s regulations as published in the college catalog and in the nursing student handbook. I understand that if I am admitted to the Nursing program, I must submit, by the date required, the completed Health History/Physical Exam, Immunization Record, TB Skin Test and/or Chest X-Ray, Drug Screen, Health Insurance, and Liability Insurance. I understand that these requirements are my financial responsibility. I understand that if I do not obtain the recommended vaccinations, I must provide physician documentation that states the reason for deferral. I understand that if I do not submit this information in its entirety that I will be withdrawn from the Nursing program. I understand that if I am admitted to the Nursing program, I am responsible for all personal health care expenses including expenses resulting from accident or sickness, illness or injury while I am engaged in learning experiences required by Bainbridge State College Associate Degree Nursing Program. I also give permission for emergency treatment to be rendered to me if necessary. I understand that Bainbridge State College, the nursing program, and /or the clinical sites are not responsible for any expenses incurred. I understand that if I am admitted to the Nursing program, it is my responsibility, throughout the program, to keep my immunizations and TB skin testing current. I agree to inform my instructors or the program director of any health problem that could possibly affect my performance and the welfare of my patients in the clinical area. I understand that if I am admitted to the Nursing program, I may be required to attend a Basic Cardiac Life Support (BLS) course unless previous certification has been obtained. I further understand that it is my responsibility to ensure that I remain certified, and am able to provide a valid American Heart Association BCLS card at all times. I understand that upon selection for admission, I am required to complete a criminal background check via PreCheck services and progression in the program is contingent upon student acceptance at clinical facilities. I give permission to release the results of my Criminal Background and Drug Screen to all clinical facilities. I understand that denial of acceptance by clinical facility will result in being unable to fulfill clinical requirements and dismissal from the program may occur. In addition, information will be provided regarding student requirements of drug screen, physical and immunization record, current CPR card, liability, and health insurance as indicated by the clinical facility. I understand the ADN faculty and/or clinical instructors may request random drug testing at any time. I understand that if admitted to the Nursing program, I will procure and maintain health insurance and liability insurance coverage throughout my tenure in the program. If my insurance policy should change, I will notify the Bainbridge State College Associate Degree in Nursing Program, in writing, of the change. I understand the LPN-ADN Advance Placement Track is not a one-day a week program; class and clinical times will vary per semester depending on the courses enrolled. I understand this is a 3 semester program, including the summer and it must be completed within a 3 year period. I further understand that any failure in the LPN-ADN Advanced Placement Track disqualifies me from returning to the LPN-ADN AP Track but I may be eligible to apply for the traditional ADN program if accepted by the ADN faculty. I also acknowledge that any failure in the Bainbridge State College ADN program since becoming an LPN, disqualifies me from applying to the LPN-ADN AP track. Admission Standards Minimum admission standards for the program include: 1. Admission to Bainbridge State College, including submission of all high school, GED, and college transcripts as well as payment of all associated fees. 2. Completed application for the ADN program by the deadline. 3. Student must provide copies of transcripts from ALL colleges/universities attended with this application. 4. Have completed successfully all developmental classes for degree level programs. 5. A student will not be eligible to enter the program if they have failed a core class twice within the last 3 years. 6. A student who has failed two or more nursing courses in any RN program within the last 5 years will NOT be eligible for admission to the LPN-ADN AP Track. 7. Any student who fails a nursing class in the AP track is NOT eligible to return to the LPN-ADN Track but may be eligible to apply for the traditional ADN program. 8. Human Anatomy & Physiology and Microbiology courses must have been taken within 5 years of LPN-ADN AP Track entry date. 9. Have earned a minimum cumulative grade point average of 2.5 on all courses taken. If a pre-requisite course has been taken twice the two grades will be averaged. Students must have successfully completed all core classes with a grade of C or higher prior to admittance to the BSC ADN Program. 10. Take the TEAS nursing entrance examination by May 9, 2015. TEAS scores are valid for two years. Only two attempts are allowed. Scores should be submitted with this application. 11. Take Fundamentals ATI exam at student s cost of $27 per attempt payable only with debit/credit card. Must score a level 2. (Provide a copy of scores with application.) Student Selection Criteria The following criteria will be utilized to rank the applicants by a composite sum. Applicant scores will be ranked from highest to lowest using the attached selection criteria. Notification of acceptance will be made by mail by May 12, 2014. Performance standards are used to assist students in determining whether accommodations or modifications are necessary and provide an objective measure upon which informed decisions can be based about whether students can meet requirements. A full description of the performance standards will be provided after entrance into the ADN Program. Critical thinking: ability sufficient for clinical judgment. Interpersonal: abilities sufficient to interact with individuals, families, and groups from a variety of social, emotional, cultural, and intellectual backgrounds. Communication skills: ability sufficient for interaction with others in verbal and written form. Physical abilities: sufficient to move from room to room and maneuver in small spaces. Gross and fine motor abilities: sufficient to provide safe and effective nursing care. Auditory abilities: sufficient to monitor and assess health needs. Tactile abilities: sufficient for observation and assessment necessary in nursing care. Visual abilities: sufficient for physical assessment. The following process will be used: Before admission to the nursing major, all students will have information regarding Core Performance Standards. After admission to the major, students will be given a copy of the Core Performance Standards. If an otherwise qualified student believes that he or she cannot meet one or more of the standards without accommodation or modifications, the nursing program, with guidance from the BSC Disability Coordinator, will determine, on an individual basis, whether or not the necessary modifications can be made reasonably. 2/23/2015 7
