Patient Portal User Guide for Patients This document gives patients a complete description and functions on the use of the Patient Portal website. The Patient Portal helps patients communicate with their providers, access their health records, and fill out their forms online. Patient Portal website The patient gets an Email Verification link, User ID, and system-generated Password through email to start using the Patient Portal website. To sign in to Patient Portal website, do the following: 1. Click the Email Verification link sent to your email so the system can verify your email address with the Patient Portal module of IMS. Figure 1.1 Click the Email Verification link so the system can verify your email address with the Patient Portal module of IMS.

2. When the system opens, enter your user name and password and click Login. Figure 1.2 When the system opens, enter patient s username and password and click Login. Note: For patients who sign in for the first time, the system will redirect them to change their password and enter a new password before they can proceed to the Patient Portal website. 3. How to send Forgot Username and Password request Retrieve a forgotten user name. If you cannot recall your user name, you can retrieve it using the Forgot Username and Password functionality in the Patient Portal website. To send a Forgot Username request, do the following: 1. Click Forgot username below Login. Figure 1.4 Click Forgot username below Login button. 2. When the message Your request for Username is in process, You will receive email soon. appears, click Login. Wait for the the clinic to approve the Forgot Username request and send you an email with the user name. Retrieve a forgotten password. If you cannot recall your password, you can retrieve it using the Forgot Username and Password functionality in the Patient Portal website. To send a Forgot Password request, do the following: 1. Wait for the clinic to approve the Forgot Password request and send you an email of the password. Patient Portal Homepage After you sign in to Patient Portal website, the Patient Portal Dashboard appears. You can view the summary of notes, lab orders and results, prescriptions, documents, and details of future appointments. Dashboard. In the Dashboard, you will see the four most recent entries from IMS (e.g. four most recent messages under My Message the clinic has uploaded) in the center pane. 8 P a t i e n t P o r t a l U s e r G u i d e f o r P a t i e n t s

This is where you can find the start page of the Dashboard after signing in to the Patient Portal screen. Forms to Fill. In this section, you can find the forms to you need to fill out. About Us. Figure 1.12 The Contact Us page Help. Providers or clinics can give you more information about their practice if ever there are questions or clarifications. Logout. You can securely sign out your account. Manage your health records. Using the Blue Button functionality in the left pane of the Patient Portal, you can view, download, or transmit your health records. To view, download, or transmit your health record, do the following: 1. Click Blue Button to open the Health Record screen or click Health Record in the left pane where a list of modules is found. 11 P a t i e n t P o r t a l U s e r G u i d e f o r P a t i e n t s

When the My Health Record screen opens, click the View your health record tab to view your health record in a new tab. Figure 1.14 The Health Record screen 3. 4. Click the Download your health record tab to download the health record in a format you can use to transfer to other providers. 5. Click the Last Updated tab to view the date and time of your last health record update. 6. Enter provider s address in the Direct Address of the Provider entry box, and then click Transmit My Health Record. Note: Type the first three characters to enable the searching of existing email addresses of the concerned providers. 7. Click the last transmitted address and status shown at the bottom of the screen. This opens the Add New Contact screen where you can add your address as a new contact. 8. Enter the necessary information in the Add New Contact screen. My Message. The My Message section is the communication bridge between the patient and the clinical staff. The patient can view notes or messages with brief descriptions and can send or receive messages to or from the clinic. The My Message tab in the center pane of the Dashboard shows the recent entries from IMS. Figure 1.16 The My Message tab New Message. Read new message from the provider or clinic. View. View a specific note with detailed description. Compose Message provider.. Compose and send a message to the clinic or the Show All. Click the Show All link to show all message links. When the My Message screen opens, use the following tabs to manage your messages: Sent, Received, and Send New Message. Figure 1.18 The My Message screen Sent. Received. View the messages that have been received from the clinic or from the provider. By default, My Message opens with Received messages. Send New Message new message.. Click Send New Message to compose a o Enter texts in the message box of the Compose a Message screen and then click Send. We will call you back, if required. prompts, click Ok. Unread Message Read Message. Click Unread Message to open not yet read messages.. Click Read Message to open message that is already read. Filter Criteria. Search for specific messages using the following criteria: o Keyword. o From and To. Filter the messages basing on the specified date range. My Documents. The patient views Document(s) with brief description under the My Document section in the Dashboard. The My Document tab in the center pane of the Dashboard shows the recent entries from IMS. Figure 1.20 The My Document tab View. Click the View symbol to view a specific document. Download. Click the Download symbol to download a specific document. Note: The documents written in bold are identified as unread documents. Show All. Click the Show All link symbol to view all document links. View Unread. Click View Unread symbol to unread document. Download Read and Unread. Click Download Read and Unread symbol to download the unread documents. Download documents.. Click Download button to download specific Select the check boxes to transmit patient s documents to the provider. Enter the provider s address in the Direct Address of the Provider entry box and then click Transmit My Documents button. The patient can view lab orders and results with brief description under the My Lab screen of Dashboard. Click Show All link under My Lab pane to view all lab orders. The My Lab(s) screen opens with Lab Order Status(s). To manage your lab orders and results 1. My Care Plan. A care plan contains the preventions and other necessary steps generally issued by the doctor for a patient to follow. It also contains the detailed description of an ailment and the basic treatment depending on the patient. To open the My Care Plan screen, do the following: 1. Click My Care Plan in the left pane of the Patient Portal website. Figure 1.27 Click the My Care Plan link in the left pane of the Patient Portal website. 2. My Vital Sign. You can view your recorded vital signs in the Patient Portal website. To open the My Vital Sign screen, do the following: 1. You can view the billing statements uploaded from IMS through the My Billing Statement screen of the Patient Portal website. To open My Billing Statement screen, do the following: 1. Click Last Statement to view patient s last statement. Note: You can also search specific billing statements using the From and To date entry boxes. After entering the dates, click Search. 6. You can also pay your bill online via Patient Portal website. The remaining copay amount can be seen on the upper-right corner of the Patient Portal home screen or below Patient Balance in the My Billing Statement screen. Click View Payment History from the My Billing Statement screen to view patient s payment history. My Appointment. Patient can view future appointments for specific dates, time, procedures, and Appointment Status under the My Future Appointment pane in the Dashboard. The appointment was not confirmed. o Cancelled By Provider provider.. The appointment was cancelled by the o Cancelled By Me. The appointment was cancelled by the patient. o Cancel request Pending. The patient requests to cancel the appointment and still waiting for confirmation. o Missed o Room. The patient missed the appointment.. 4. You cannot cancel any appointment from the Patient Portal website. If you click Cancel Appointment, a message asking you to call the clinic appears. In the Appointment Request form, click the drop-down arrow in the Office entry box to select an office. 7. Select a doctor in the Provider drop-down list. Note: By default, Appointment Request Form automatically displays the patient s phone number and amail address. 8. Enter the reason for the appointment in the Reason entry box. 9. You can view the prescription details in the My Prescription screen in the Dashboard window. 1. Select the My Prescription tab in the center pane of the Dashboard to show the recent entries from IMS. 2. Click Indication/Contra Indication in the top right of the Drug Education pane to view indication/contra indication of drug. Back to Home. Show All. Click Show All to open with prescribed drugs where the patient can also send Prescription Refill Request form from the Patient Portal website. 4. Select the Send Request checkbox in the S column. 8. Click Send Request. 9. When the message Request for Refill has been sent successfully. appears, click Ok. You can view your demographic information in the My Profile link. You can also edit your information from the Patient Portal website. To open the My Profile screen, do the following: 1. You can view your insurance details such as the insurance plan, insurance ID, priority, start date, and end Date. To open the My Insurance screen, do the following: 1. Patient Forms The appointment form in the Patient Portal website is a requirement for the you to fill out. You can directly access the forms online. You will receive an email of the web form link.
