TRIP DISCLAIMER CONSUMER DISCLOSURE NOTICE : Please read the Terms & Conditions carefully, as your signing the contract and/or deposit payment on a trip signifies acceptance of the Terms & Conditions and the general information provided with the proposal. These trips are arranged by UR Tours and Events (hereinafter UR ). It has made the travel arrangements as agent for the transportation carriers and other suppliers (SUPS) of services connected with the tour, all of which are independent contractors. The tour cost is based on actual travel dates and total number of paid passengers noted by the Group Leader at time of contract. Should the number of paid passengers fluctuate, adjustments to the tour cost will be made accordingly to cover additional variable costs. Prices are subject to change for matters over which UR Tours & events, Inc. has no control, such as fuel surcharges, government taxes, airline schedule changes or cancellations which are imposed by one of our suppliers. A contract is made when your reservation and receipt of initial payment are accepted by UR in UR home office in Orlando, Florida. UR TOURS & EVENTS, INC is registered with the State of Florida as a Seller of Travel Reg No. ST39495 A) By signing the Application, the Participants represents to UR that he/she is in good health and has no medical condition or disability, or need for prescription medication which would require special attention on the tour or at the event (except where separately noted in writing to the UR office). If the Participant becomes ill or injured while on the tour or at the event, the Participant and his/her parent(s)/guardian(s) agree that UR shall have the right, but not the duty, to take reasonable action to secure emergency medical attention and/or transportation for the Participant. The Participant agrees to immediately inform UR of any injury or illness incurred during the tour or event and to pay for any expense incurred for such medical attention, transportation, or other emergency related services. Any and all medical expenses incurred by the Participant are the sole responsibility of the Participant. UR, its officers, employees and agents is not responsible are not responsible for personal injury suffered, incurred or caused during the trip. B) Once UR issues and delivers travel or other documents, i.e., theme park tickets, event/activity tickets, these documents become the sole responsibility of the Participant. UR shall not be responsible for replacements of or refunds for lost or stolen documents. C) The price quoted is per person, unless noted otherwise, and the price is subject to adjustment if the number of participants in each room varies significantly. UR does not assign nor is responsible for determining roommates for the tour. D) Deposits and payments may be made by check, cashiers check, postal money order, Visa, Mastercard, or Discover. FINAL payments MUST be in the form of a cashier's check, postal money order or credit card only and must be received at the UR office no later than the final payment date set forth on the reverse side of this flyer. UR is not responsible for, nor does it accept, any cash payments. Reservations and late payments received after the final payment deadline require immediate payment in full in the form of a cashier's check or postal money order only and MUST include a 3% late registration/payment fee. UR will charge the Participant $35.00 for all returned checks. Checks and money orders must be made payable to UR Tours and Events Credit card numbers will not be accepted over the phone. This Agreement, the Informed Consent (the terms of which are incorporated herein by reference) and the tour or event brochure, together constitute the entire agreement between the parties, and supersedes any previous agreements or understandings. The agreement can only be modified by a writing signed by the party to be charged, and shall be governed by and interpreted pursuant to the laws of the State of Florida. The venue shall be Orange County, Florida. 5
