Welcome to myuhs, your free, web-based patient portal. myuhs enables you to communicate with your UHS healthcare team from your home, work, or anywhere else you can connect to the web. myuhs helps you to more easily take charge of your health because myuhs and many other online UHS services are available at a time that is right for you. Through myuhs, you can communicate with your provider s office to request an appointment, renew a prescription, ask non-urgent questions, view lab results and much more! Let s take a look at just how easy myuhs is to use!
ENROLLMENT INSTRUCTIONS: You received an email with a link to the myuhs Patient Portal. Follow this link to complete your registration.

Choose I was given an enrollment token then click Next Token Enter the security token you received from your healthcare provider, our date of birth and your email address, then click Next
CREATE YOUR USERNAME, PASSWORD, LOGIN SECURITY AUTHORIZATION AND SECURITY QUESTION Enter the security token you received from your healthcare provider, our date of birth and your email address, then click Next
CREATE YOUR PASSWORD RECOVERY CREDENTIALS Enter the security token you received from your healthcare provider, our date of birth and your email address, then click Next
LOGIN/ENROLL Login to myuhs using your username and password. If you have forgotten your user name or password, click on the Need help with your user name and password? link and follow the on-screen instructions.
SECURITY QUESTION You will be asked your security question for your safety at every login.
HOME PAGE Your inbox, lab results and medications are all located on the home page.
MAIL From the MAIL drop down tab, you can navigate to your inbox, sent items or compose a new message. Your inbox will contain messages sent to you securely by your provider s office. Icons are used to indicate the type of message sent by your provider s office. When a new message is available in the Patient Portal, you will also receive an alert sent via email to the personal email account you used to register. Clicking once on the message will open it.
MAIL OPEN MESSAGE You will be able to reply, delete or print the message.
MAIL - REPLY MESSAGE Your reply message will contain your provider s original message and your response. Click SEND to send your message back to your provider s office.
MAIL - COMPOSING A MESSAGE Choose Compose Message from the MAIL drop down tab. The Practice will always be myuhs Patient Portal. Complete the form and click SUBMIT

MY CHART - VIEW MY CHART From the MY CHART tab, you may view your chart.
MY CHART - DOWNLOAD AND SEND MY CHART A PHR is a document that contains all the information from your chart. You can download and save the PHR as a PDF file. You need the Adobe Acrobat Reader to view the PDF file. Click the Download and send my chart link to view.
MY CHART - DOWNLOAD AND SEND PERSONAL HEALTH RECORD (PHR) The PHR details display. To save your PHR, click Save CCDA
MY CHART - REQUEST PERSONAL HEALTH RECORD Choose Request Health Record from the MY CHART drop down tab and click SUBMIT. Your PHR will be sent to your inbox; click on the message and your PHR will be downloaded for you to view, save or send.
RENEW MEDICATIONS Choose Renew Medications from the RENEW MEDICATIONS drop down tab. The Practice will always be myuhs Patient Portal. Complete the form and click SUBMIT
PATIENT EDUCATION Patient education information such as educational materials, symptom checkers, general medication information and a health information tool can be found underneath this tab.
SETTINGS - ACCOUNT SETTINGS You may change your username, password, security question and forgotten password question from this page.
SETTINGS MY INFORMATION You can update your name, address, email and phone number at any time, however, updates to your personal contact information are not sent electronically to your provider s office. You must contact the practice by sending a secure message or calling your provider s office directly.
SETTINGS MANAGE USER GRANTS You must contact your provider s office to define a care manager relationship.
