PT/OT CHILDRENS" REPORTS 2012-04-11 1. Tian Yu is a 3 y.o. boy with mild CP and motor delay, silent and keeps looking around the therapy area. Impression to have poor cognitive skill and can't speak. His main problem is the delay of motor skills including hand function. This is his first time to have therapy and the therapist is working over the treatment plan to improve his condition and catch-up his delayed motor development. No feeding problem. At his age he has good potential for progress. 2. Jian Qiang is a 3 y.o. boy with Spina Bifida. When he recover from his surgery he started with his therapy with motor problem on his (B) lower extremities secondary to weak muscles. And can only speak few words e.g. "ayi", "nainai". After 6 months of therapy he was able to walk without assistance. Therapist still continue his plan for enhancing his cognitive and communication skills. He has no feeding problem. 3. Xiang Fei, a 4 y.o. boy with CP, R-hemiphlegic. He's new and now he undergo physical therapy. We had our initial assessment last March 20, 2012. He had difficulty walking and there's a neglect of his(r) arm. He's lively and playful when you don't touch his (R) arm or else he will cry and stop for therapy. We did our treatment plan and now he slightly tolerate therapist holding his R arm and he can walk freely with minimal assistance. He still can't speak. Therapist will continue his treatment as plan. 4. Longlong, 2 y.o.boy, with CP and motor delay. He got a sweet smiles but he often get sick that causes him to be deferred on therapy. He got mild spasticity on (B) upper and lower extremities, absence of motor skills except on side-lying, poor hand function and fair to poor motor control on head, trunk and body. He has intact sensory motor skills and express himself by crying. No problem in drinking milk still can't eat semi-solid foods. Therapist advice caregiver to observe health-care of the child. 5. Xiuxiu, a 5 y.o. girl, with CP. She loves to creep and slightly crawl on the new crawling exerciser. Her spasticity had reduce from moderate to mild on (B) her upper and lower extremities. She ca'nt freely crawl that limits her skill on sitting and pushing her up to stand. She has good cognition and can speak with a low voice. She can already eat by herself. Therapist updates some of the plans to improve her motor skills. 6. Chunling, 3 y.o. girl with CP-athetoid. She's smiling and happy to be on therapy. She shows involuntary movement and improper positioning on her (R) arm. She got normal muscle tone on her (L) arm. But (B) lower extremities have weak muscle tone. She can only rool-over but can't creep and crawl. Can sit with support and had not gained motor control. Still can't speak and have fair cognition. She has no problem in feeding. Therapist will continue the treatment plan.
7. Yu Qing, a 4 y.o. girl with CP. She always come for therapy silent and smiling. Her muscle tone had reduce from severe to moderate spasticity on (B) upper and lower extremities. She can only roll-over but can't creep and crawl. She can only sit-up on chair but no trunk and pelvic control. She can understand and only speak "Ayi". No problem on feeding. Her therapy will continue as planned. 8. Guo Qing, 5 y.o. boy with CP and spinal deformity. He's silent and smiles when play. He got moderate spasticity on (B) his upper and lower extremities. He has poor motor skills and can only turn-over and creep but slowly. He can fairly understand but can't speak. He has no feeding problem. Therapist will continue the treatment plan. 9. Zhen Jie, 7 year old girl with CP. She always smile when brought to therapy. She got severe spasticity on (B) her upper and lower extremities. No motor skills acquired and motor control. Can't speak and understand. Dependent to her Ayi in all areas of activities of daily living (ADL's). Treatment plan is continued for health reasons. 10. Jie Shi, 9 yr. old boy, with CP-athetoid. Loves to go around the therapy area with a smile. His tight muscles on his (B) upper and lower extremities secondary to condition finds him difficulty to stand with assistance and can walk alone on parallel bars. He has difficulty in speaking. He can eat by himself except on self-care. His treatment plan continues until he can mobilize by himself. 11. Ting Ting, a 10 y.o.girl with CP and with mild mental retardation. After almost 3 years of therapy she can now push to stand without support for 1 sec. She can walk by herself on parallel bars. She can understand simple instruction and command and assist other children to play in the therapy area. She can eat by herself except on self-care. Her therapy will continue until she can manage to mobilize independently. 12. Xie Nan, 1 y.o. boy with post clubfeet surgery. He was silent when brought to therapy but later started to cry when his corrective shoes was removed. He can only do side-lying and no voluntary reach and grasp of toys. Fair head and poor body control and keep sounding his mouth and babbling. Treatment plan was made to stabilized (B) ankle-feet joints and sensory stimulation to enhance the RGR patterns. This afternoon he already started grabbing toys only about a second. Play and vestibular activity be given to develop his motor control and skills. 13. Zhe Hui, 2 y.o. girl, CP with motor delay. She's silent and loves to play. She can only Do side-lying and and grab toys but can't hold it for more than 5 mins. Still can't speak but can drink milk and eat semi-solid foods. Therapy is going on to develop her motor skills and resolve her other problem. At her age she has good potential for progress.
14. LiangLiang, 3 y.o.old boy with Severe CP. He's silent with occasional crying. He got severe spasticity on (B) upper and lower extremities. He is generally not able to acquire any motor skills and functions. He can drink milk with spillage. Treatment plan was done and implemented to prevent further complications. 15. Wang Hao, 2 y.o. old boy with a Post Clubfeet Surgery. He is still on casting and therapy will be manage after this procedure. He is smart, happy and active baby. SENSORY INTEGRATION THERAPY CHILDRENS" REPORTS 2012-04-11 1. Xuan Xuan, 4 y.o. boy with Autism. He loves to play his (B) fingers by himself. He can stand with led and can walk with wooden frame with assistance. Don't initiate to reach and grab toys. He can't speak and don't understand. After few weeks of therapy he looks and smile when called by his name. He smiles and listen to music. Sensory integration treatment will continue with hand and motor skills training. 2. Xiang Xiang, a 5 y.o. girl with Autism. She smiles and loves to listen to music and songs. She can now stand and walk with assistance. She can't speak but laugh and giggle when the therapist tickled her. She only drink milk. After the management of sensory integration therapy, she started to look at her therapist and held her hands with closeness and attachment. She knew that her therapist held her hands and she smiles. The sense of taste and smell is closely manage that she will appreciate eating foods. When the sense of taste was conducted she let her tongue out and sense the taste.. The therapist will continue the plan as indicated. 3. Pang Pang, a 5 y.o.boy with Autism. He is sometimes silent and with occasional crying. He got a serious medical problem that deferred him for therapy. He can only do side-lying and sit on chair with support. He has no eye contact and doesn't care around. After the management of sensory integration on touch, he can now grab the object touch to him and he start holding and playing on it. The treatment plan will continue to be manage. 4. Bei Bei, a 4 y.o. girl with Autism. She's silent and always biting her hands. She had (B) ankles deformity due to immobilization. She's delayed on all motor skills and body control. Sensory integration therapy was conducted to her and lately she began uttering the letters "mmmm". Still treatment plan needs to be continued.
5. Lai Bing, 5 y.o. boy with Autism. He shows repetition of movements e.g. hitting, play his hand by himself. He can do voluntary movements on mat but can't assume sitting and needs assistance in walking using the wooden frame. Still he can't understand simple command but cries when he want another activity. He loves to be riding on the boat and swing. Lately, he gaze when somebody called his name. Sensory integration therapy will continue. 6. Ming Ming, a 3 y.o. old boy with Autism. He's silent and biting his (B) hands. He can only do side-lying and sit on a chair with support. Sensory motor skills are impaired and cognition and motor functions are absolutely affected. Passive functional exercises and Sensory integration therapy is given to maintain his health. 7. Wang Zhi, 3 y.o.boy with Down Syndrome. He's just silent and reach and grab toys. There's mild muscle weakness on (B) lower extremities. Can't perform any motor skills except side-lying, can fairly control his head only. Sensory motor skills is impaired and can't speak. Sensory integration therapy and functional exercises is given. 8. Rong Xiang, 3 y.o. boy with Down Syndrome. His head is heavy, impression of hydrocephalus. He's silent and keeps on babbling and swinging actively his (B) legs. Can't perform any motor skills but can reach and grab toys. Sensory integration therapy and Functional exercises is given. 9. Xie He, 5, y.o. girl with Autism. She's smiling and silent. No repetition of movements. She only sit quietly when put in a chair. She doesn't have any mobility skills and hand function. Can eat semi-solid foods fed by Ayi. Sensory integration therapy and functional exercises are given and swinging activity during her therapy session. 10. Wang Wang, 4 y.o. boy with Autism. He's silent and no eye contact. Cries occasionally when felt discomfort. He got respiratory problem and so often he is deferred in therapy. No repetition of movements shown. Treatment plan for sensory integration and functional exercises and vestibular activities is given. 11. Shuang Shuang, a 9 y.o. old girl with Autism. Happy and loves to move around holding bars And walk with led. Still she can't speak but will respond by looking at you when called her name. She can eat solid foods but fed by Ayi. This time she can now walk by herself. A program for feeding training was given to be done by the Ayi; Therapy session still going on for sensory awareness and cognitive training.

Can eat by themselves but need supervision. 2. Min Hong, 5 y.o. boy with Down Syndrome. He is a loner, he plays by himself. He is independent in caring himself but still needs supervision. He can't speak and fairly understand. 3. Qiu Ju, a 4 y.o. girl with Down Syndrome. She loves to go around. Can't speak and Can't understand. She can only eat by herself but can't personal;y care. 4. Le Le, 4 y.o. boy with Down Syndrome. Loves to play around. Can't speak but eat by herself but can't do any personal care. Still can't understand simple command. 5. Rong Rong, 40 y.o. girl with Dwarftism and mental retardation. Slow to move around Speak with a low voice and can fairly understand. She can take care of herself with Supervision.
