PERMISSIONS PERMISSION FOR ROUTINE OUTINGS ROUTINE EXCURSIONS ARE DISPLAYED DAILY ON THE EXCURSION BOARD IN THE SIGN IN AREA OF YOUR SERVICE. Typical Outings include school, kinder, playgroups (community or joint educators), library, local shops, parks and playgrounds. Please indicate your agreement to monitor the excursion board and raise any concerns regarding risk managing particular hazards, for your child that may present themselves at these locations. Please note that the Educator has provided you with the risk plans and the routine outings form has been signed by you. I give consent for photos to be taken of my child in the following circumstances: (PLEASE CIRCLE) Educators PHOTO PERMISSIONS 1. By the New Futures Educator; to be utilised in individual child portfolios recording interests and development of my child and to be shared with and retained by myself, the parent, when my child exits the service. 2. By the educator; to be utilised in group photos of program activities all children engage in and to be used in weekly diaries, observations, communications showcasing the program. 3. By the Educator to be posted on a closed parent blog managed by the Educator and only accessible to parents of children in care. 4. The educator to promote /market their services locally. The New Futures National Quality Standards (NQS) Consultants regularly visit Educators and take photographic evidence of practices and the program offered to children. 5. I, parent, am happy for these group photos to be taken and these recording be maintained on the Educator s visit record which is shared with the Department of Education authorised officers during audits of the service. All photographs are archived and destroyed 3 years after a child exits. 6. Photos taken by NQS consultants on visits at special events; to positively promote and document the activities of children and the program. 7. VICSEG New Futures media staff; to positively promote and document the activities of children and the program for marketing purposes on the VICSEG NEW FUTURES Website, newspapers or electronic media; to develop training or promotional materials for families and Educators. I understand that no other use will be made of these recordings of my child or me without my prior consent. I give this permission without being entitled to personal compensation and understand that any electronic recording will be owned by VICSEG New Futures until I withdraw the authorisation. Please note New Futures is required under the legislation to retain copies of assessments which may include photos for 3 years after a child exits and then destroy this information. New Futures educators may maintain electronic records of programs, observations and assessments which are also to be destroyed 3 years after a child has exited their care. I understand and agree that I have a right to withdraw this authorisation. If I wish to do so, it will be my responsibility to inform the Family Day Care staff and the Communications team at VICSEG New Futures on 9383 2533 or 9353 5888 or by email to familydaycare@vicsegnewfutures.org.au PARENT SIGNATURE DATE DECLARATIONS OF UNDERSTANDING IMMUNISATION The Australian government requires that all parents ensure their child immunisations are kept current or all Child Benefit Payments cease. All parents are required to provide a service with an immunisation status certificate from the Australian Childhood Immunisation Register (ACIR). To obtain this certificate parents can: * Call 1800 653 809 * Email acir@medicareaustralia.gov.au * Download from acir@medicareaustralia.gov.au/online * Visit a Medicare service centre * Visit your local doctor or council immunisation service Parents may enrol unimmunised children and children on immunisation catch up plans. Please note all families are responsible for full fees and managing their benefit payments from the Australian Government. Has your Child been immunised? DECLARATIONS AND CONSENT FOR EMERGENCY MEDICAL TREATMENT I, (print full name) declare that the information in this enrolment form is true and correct and undertake to immediately inform the children s service in the event of any change to this information; agree to collect or make arrangements for the collection of the child if he/she becomes unwell at the service; consent to the family day care service to seek medical treatment for the child from a medical practitioner, hospital or ambulance service and understand any costs incurred are my responsibility. 4 PARENT FAILURE TO ARRIVE Policy: Ensure health, safety and wellbeing of children in care at all times. All parents/guardians must advise Educators in the event of any anticipated delays in collecting their child/ren stating their anticipated time of arrival. Failure to do so within 30 minutes of the usual collection time will result in the emergency persons being contacted. If the emergency persons are unreachable, the Educator is required to contact the New Futures service who will take the appropriate action. This may include taking custody of the child and seeking support from the department of human services, the regional office and the police. I have read and understood VICSEG New Futures policy regarding failure to collect my child/ren. I understand that the relevant actions noted above will be taken. PARENT SIGNATURE PARENT RESPONSIBILITY TO INFORM SERVICE TO ENSURE CHILD ENROLMENT INFORMATION IS CORRECT I declare that the information in this enrolment form is true and correct. I understand that any changes to my child s enrolment form must be communicated to my Educator and Service staff as soon as possible. Please note all parents are able to view their child s enrolment form information and invoices online at any time. PAYMENTS AND FEES 1. Child Care Benefit (CCB), Child Care Rebate (CCR) and Gap Fees Australian Government contributions to the cost of childcare include: Child Care Benefit (CCB) paid to families o School age children are eligible for 85% of the CCB rate. Child Care Rebate (CCR) paid to families Job, Employment and Training (JET) paid to families studying or training in preparation for entering the workforce. Families can choose to reduce their child care fees by directing CCB, CCR and JET payments to New Futures Family Day Care. In this instance New Futures Family Day Care will issue an invoice for the gap fee only. Gap fee refers to the payment that parents need to make for the remainder of the child care cost not covered by Australian Government contributions. CCR, CCB and JET payments are calculated by the Australian Government Department of Human of Services/Centrelink. For more information call 13 61 50. 2. Timesheets All parents must time and sign children in and out of care at arrival and departure time and when an absence has been noted using New Futures FDC timesheets. Do not sign in advance. All timesheets are legal documents subject to Australian Government and taxation audits. At the end of each fortnight pay period parents are required to sign and date timesheets to confirm accuracy. Gap fees for the previous fortnight are noted at the bottom of the timesheet and signed by the Educator. A carbon copy of the timesheet is provided to families as a receipt for payment of the gap fee to the Educator. Parents should check the copy to make sure their gap fees are accurately recorded. New Futures Family Day Care Levy 4. Invoices and quarterly and annual statements 5. Notice periodceasing care In 2016 New Futures FDC parent admin fee is $1.00 per child/per hour. Families are given four weeks notice of any changes to the levy and the date that it will take effect. New Futures Family Day Care invoices families on a fortnightly basis. Quarterly and annual statements are also provided to families by email and are available through online access at any time. Invoices and statements include the following information: Fees. Amount charged for the hours for care received. New Futures Family Day Care Service Levy Standards hours of care used (between 8am and 6pm). Non-standard and casual hours of care used (before 8am and after 6pm). CCB, CCR and JET payments received (If families have chosen to direct these payments to New Futures Family Day Care). Payments received for gap fees and outstanding fees to be paid. Two weeks (14 days) notice must be given by either party for the termination of the care arrangement. All fees must be paid in full prior to leaving care. The service reserves the right to terminate care immediately, if after negotiation the child care fees are not paid.

Where a child is absent for the final days of the 2 week notice period, no CCB or CCR is payable. Parents are required to pay 100% of the fee. 6. Changes to Agreed Hours A 2 week (14 days) notice period applies to all changes to hours of care unless otherwise agreed upon with an Educator. 7. Late Fees Late fees of $ 5 per 15 minutes are applied to timesheet records where children have been signed in/ out more than 15 minutes after or before operational hours unless the educator advises otherwise on the comments section of the timesheet. Where late fees apply, no CCB or CCR is paid. 8. Casual Booking Cancellation If a casual booking is cancelled within 24 hours of the requested time of care, then the parent must still pay for that care. CCB and CCR do not apply. 100% of the fee is required. 9. Bond Educators as agents of New Futures FDC may request a bond to confirm a child care enrolment and booking. Bonds are refunded as reduced child care fees on invoices issued after families have provided 14 days (2 weeks) notice of ceasing care. 10. Absences and Holidays 11. Public Holidays The Australian government (CCR, CCB & JET) allows up to 42 absences for holidays, public holidays, illness and other family reasons. Where families need additional absence days a medical certificate is required and approval from Centrelink. The Australian government does not require educators to provide care on public holidays. If a child s normal booked day falls on a public holiday, the normal fee applies and CCB/CCR subsidies are paid. Educators may also apply public holiday fee rates, if care is provided on a public holiday. 12. Sick Educators When an educator is unwell they will notify all parents and service staff as soon as possible. Alternative arrangements for care will be made where possible. Child Care fees do not apply if care is unavailable. 13. Resignation or holiday of Educator Parents will be given 28 days (4 weeks) notice when an Educator is taking leave, or has resigned. Alternative arrangements for care will be made as soon as possible. Child Care Fees do not apply if care is unavailable. 14. Kindergarten A full day s booked fee is payable to the educator where he/she is responsible for drop off and pick up of preschool children attending sessional kindergarten and/or other educational services. 15. Overdue fee payments. If payments for services are a week late, educators acting as agents of New Futures Family Day Care, will not be available until all overdue payments are received, or the parent has entered into an agreed payment plan and/or reduced hours of care. Payments overdue more than a month are treated as a debt collection liability. NOTIFICATION OF EDUCATOR ASSISTANT CARING FOR CHILDREN Only complete this section when instructed by an Educator who is supported by an Assistant I have been notified about an Educator Assistant working in a Family Day Care home. New Futures Family Day requires Educator Assistants to undertake a Police Record Check and hold a Working with Children Check and First Aid Certificates. All Educator Assistants undertake training and are monitored by the New Futures Family Day Care service. NOTIFICATION OF RELIEF EDUCATOR CARING FOR CHILDREN Only complete this section when instructed by an Educator who is supported by a RELIEF EDUCATOR My Educator has advised me that a Relief Educator ensures continuity of care for my children when the Educator is on holiday, or ill; and / provides weekly programming support. The Educator has explained to me that the relief Educator must hold a clear Police Record Check and Working with Children Check, First Aid, Asthma and Anaphylaxis Certifications, as well as be enrolled or qualified as an Educator with obligations for continuous professional development.
