Site OHSE Supervisor Checklist The following list of requirements for the Site Supervisor to administer and implement into the manual specific to this project: Information to be recorded by the Supervisor or all employees prior to commencement date: 1. Supervisor to ensure all employees are in the possession of required certificates. 2. Site induction to take place for each employee with all relevant site specific issues raised. Particular attention to safety issues and any hazardous material on site. 3. All employees during site induction are to read all of the Work Method Statements and are to sign at the bottom to verify that they have read and understood the Work Method Statement. 4. All employees must read Advanced Group Services, OHSE, Live Work, Injury and Environment Company Policies and Environmental Plan. Information to be recorded periodically and when necessary during the project: 1. Tool box talks are to take place periodically and when necessary due to site specific conditions and events. These are to be used as a forum for all personnel to raise issues of concern in relation to the OHSE and any other site conditions. Information Page - 10 of 17

Live Work Policy & Duty of Care Advanced Group Services acknowledges our moral and legal responsibilities to protect our employees and others from the risks that may arise from our work activities. As part of this Duty of Care, working on or near exposed conductors that are or could be live, will not be done by our employees or other people doing work for us, unless there is an emergency situation and the risk of harm would be greater if the circuits and apparatus were de-energised and all the following conditions are met: 1. The necessity to do the particular task live has been discussed by our person responsible, the client and the people who may perform the work. 2. It has been genuinely agreed that there are no other options, such as isolating the area where the work is to be done or working de-energised out of normal working hours. 3. A task specific risk assessment is carried out and documented. 4. As a minimum, the risk assessment will be in accordance with Appendix 4 of the NSW Work Cover Publication; Guide to Electrical Workers Safety Practices (for up to 1000 volts). 5. All necessary protective equipment is on the job site, in good condition, tested as appropriate and used correctly. 6. First aid and communication facilities are immediately accessible. 7. An observer, current and competent in electrical rescue and cardiopulmonary resuscitation (CPR) is continuously present. 8. Authorisation for the particular task must be obtained from the Client s Site Manager and Advanced Group Services Site Manager. The name and position of persons giving this authorisation to be documented. 9. The standard working procedures and safe working practices for live work as set out in our documents shall be observed at all times. 10. This policy does not apply to testing or fault finding where procedures as set. Information Page - 11 of 17

Drug & Alcohol Policy In keeping with our requirements to provide and maintain a safe and healthy place of work, this policy sets out the responsibilities of all employees and other people working on site to be fit for duty. It is the personal responsibility of all people to ensure that their work performance is always such that their safety, the safety of others and their work eiciency is not impaired. While there is not simple or reliable way to assess a person s impairment, the consumption or use of drugs, alcohol or any other substance that may aect a person s ability to work safely and eiciently is not permitted. People taking prescription or over-the-counter medications that may impair their performance are to advise their supervisor. Such advice will be treated confidentially. Employees or other people who are observed to be in breach of this policy will be subject to the Company s disciplinary procedures and depending on the circumstances their behaviour may be treated as serious misconduct. Counselling, including the provision of independent and confidential counselling, will be oered as appropriate. Information Page - 12 of 17

Occupational Health & Safety Policy This policy is a statement of Advanced Group Services`s commitment to provide safe and healthy workplaces for our employees and others who may be aected by our work, with the objective of preventing work related injuries and illnesses. Managers and supervisors have the responsibility for implementing this Policy as an integral part of their accountability. They may delegate the OHS duties and activities but not their responsibilities. Advanced Group Services is also committed to protecting our employees by complying with OHS legislation, demonstrating due diligence and the application of industry-based OHS procedures over job expediency. Further key objects with this Policy are: 1. Implementing proactive health and safety management strategies 2. Integrating OHS management into our projects at the planning stage. 3. Controlling hazards by the Occupational Risk Management process of Hazard Identification, Risk Assessment and Risk Control. 4. Monitoring and reviewing risk controls to ensure they remain eective. 5. Involving our employees in the decision making processes through ongoing training, consultation and structured communication. 6. Providing ongoing education to ensure our employees to work in the safest way possible at all times whilst on site. 7. Necessitate all employees to comply with the legislated and Company OHS responsibilities. 8. Implementing continuous improvement strategies. 9. Providing eective injury management and rehabilitation stratagem. The objectives of this Policy will be achieved by total commitment and demonstrated leadership from all managers and supervisors and the involvement of all employees. Managing the Safety of Advanced Group Workers Please be aware, Advanced Group provide regular OH&S site inspections This publication is designed to assist host employers to prepare for and manage Advanced Group Services workers. It provides information and tools to help: Clarify your workplace health and safety (OH&S) obligations Identify risks to health and safety Match the labour hire worker to tasks Complete induction and training Supervise and monitor the health and safety of people at the workplace. Further Advanced Group Services guidance material can be found at www.advancedgroupservices.com.au Managing the safety of on-hired workers. These publications provide useful information and tools which will assist you to provide a safe place of work for Advanced Group Services workers. What is a host employer? A host employer is a firm that uses Advanced Labour Hire workers supplied by Advanced Group Services agency. Host employers workplace health and safety (OH&S) obligations Why do host employers need to manage the safety of Advanced Group Services Workers and contractors? Under NSW laws, employers are responsible for the health and safety of all workers at their workplace. As a host employer engaging Advanced Group Services workers, your obligations as an employer also extend to these Advanced Group Services contractors. Eective consultation between the host employer, Advanced Group Services agency and their workers is essential to providing a safe work environment. As a host employer, if you are asked to consult with workers on OH&S matters, you have a legal obligation to do so on an ongoing basis. Information Page - 14 of 17

Managing the Safety of Advanced Group Workers What do you need to do as a host employer? As a host employer with Advanced Group Services workers, you need to take the following actions: Treat Advanced Group Services workers, apprentices and trainees as though they are your own workers in providing and maintaining a safe working environment and conditions Define the job, tasks or roles that Advanced Group Services workers will perform Identify hazards associated with the work, and assess and control risks Identify the skills and knowledge (including any licensing and certification requirements) that Advanced Group Services workers need in order to do their work safely Advise the Advanced Group Services agency of the job requirements, work environment, organisational arrangements, health and safety risks associated with the work, and the skills and knowledge required for the work. Refer to the Job description checklist Verify that the workers selected have the necessary skills and knowledge to perform their work safely Clarify who will provide any equipment, including personal protective equipment (PPE), to enable the workers to do the work safely. Ensure that equipment brought into your workplace meets health and safety standards Conduct workplace inductions for all Advanced Group Services workers, including new workers or new tasks. Provide any other training that enables them to perform their work safely. Support and encourage consultation with workers in relation to health and safety issues Establish an open communication process and encourage Advanced Group Services workers to participate in workplace consultative arrangements. Introduce Advanced Group Services workers to health and safety sta and the person with the OH&S management responsibility in the area where they will work Verify that Advanced Group Services workers understand the OH&S requirements in your workplace Provide adequate supervision to monitor whether the work is being conducted safely Encourage Advanced Group Services workers to participate in hazard management Information Page - 15 of 17

Managing the Safety of Advanced Group Workers Inform Advanced Group Services if there is a change to your workplace that impacts on the Advanced Group Services workers. Do not transfer Advanced Group Services workers to other tasks until the agency responds. You should provide any further training, instruction or information to Advanced Group Services workers as necessary Encourage Advanced Group Services workers to maintain contact with Advanced Group Services agency Cooperate with the Advanced Group Services agency to help meet its legal OH&S obligations Allow the Advanced Group Services agency to have access to the workplace and relevant documents to enable them to perform a workplace safety assessment. To make the process easier, Advanced Group Services will actively assist you to manage the safety of all workers at your workplace. What should you expect from Advanced Group Services? Advanced Group Services WILL: Gather information about the job, including the qualifications, skills and licences needed to match the right person to the position Gather information from the host employer to determine the potential for injury to their workers Consult with the host employer on ways that identified risks can be controlled Have a written agreement with the employer that clearly outlines OH&S expectations and responsibilities, including the channels to quickly communicate information Provide OH&S induction and training at the agency to ensure work is done eiciently and safely Visit the worksite regularly and have eective consultation processes in place with their workers Have a documented system for managing safety, including agency sta trained in OH&S. Advanced Group Services will not provide workers to a business when the particular business safety assessment or record demonstrates that the host employer does not provide a safe workplace. Injury Management & Return to Work Policy This Policy forms part of Advanced Group Services Occupational and Safety Policy and to be read in conjunction with that Policy. Should a work related injury or illness occur to any of our employees, Advanced Group Services is committed to providing timely and cost eective injury management to facilitate an early and durable return to the work program that will be in the best interests of all employees. Our Return to Work Coordinators, in consultation with all stakeholders, will oversee this process. Our key objectives are to: 1. Prevent work related injuries and illness. 2. Provide primary care for employees sustaining a work-related injury or illness. 3. Initiate the injury management process as soon as possible. 4. Ensure injury management is the expectation following a work-related injury or illness. 5. Ensure returning to work as soon as possible is normal practice and expectation. 6. Ensure participation in a return to work program is not prejudicial to an injured person. 7. Provide information to employees so that the injury management process is understood and acceptable. 8. Provide suitable duties, where practicable, for an injured or partially incapacitated employee as an integral part of the return to work process. 9. Resolve disputes regarding injury management by consultation with all stakeholders.
