Downloading the App In the App Store or Google Play, search for symbility health. Download the app named Symbility Health Mobile Claims. Opening the App (Initial Setup) To open the app, tap the Health Claims icon. Once loaded you will see the login/registration screen. Once the download is complete, you will see the app on your phone screen as Health Claims. After you have registered and logged in once, the login screen will change to show the Sirius Benefit Plans logo and colors. 1
Signing Up To sign up, tap the Register button. This will open the registration form on your phone s browser. Required Information for Signing Up: Group number: 5 digit number found on the STAR Card for either DENTAL, DRUGS or HEALTH. Certificate number: This is found on the Star Card for either DENTAL, DRUGS or HEALTH will be 6 numbers. Once you have entered all your information, you may read the Terms and Conditions. Agree to the terms and conditions by checking off the box. Tap Sign Up and you will be taken to the home screen. If you see the following after you have completed everything and clicked Sign up: You will need to check all your entered data as there may have been an error in the information entered. Note: if your coverage effective date is in the future, you will need to wait until that date in order to sign up. Postal code: Your home postal code Date of birth: Your date of birth. Username: Choose and enter the user name you will use to log in Email: A valid email address. This address will be used for communication. EFT Information: Having your bank account information entered will be necessary in order to use Sirius Online Solutions. Having Problems? Confirm email: Enter your email address a second time. Password: Must be at least 8 characters long; must not contain your user name, first name or last name and must include at least 3 of the following 4 features: 1. Uppercase letters (A-Z) 2. Lowercase letters (a-z) 3. Numbers (0-9) 4. Special characters (!, $, % ^, &, etc.). Confirm password: Enter your password a second time. CAPTCHA: Enter what you see in the picture. If you are unable to read the words / numbers in the image click on Can t see the words? Try another one to get a new image. 2
Home Profile The Home screen is where you can submit claims, check your annual benefit balances and view / change information. Submit Claims Submit an electronic claim from your phone (explained in the section Submitting a Claim). The profile section shows your information. You can edit the information that has an arrow beside it (address, dependents, and direct deposit) by tapping in that area. Your Annual Balances Check your annual benefit balances (explained in the section Coverage). Your Information Your personal information and preferences (explained in the section Profile). A down arrow ( ) provides a drop down box with further selections where you can then select the item you would like. Tapping on a section that is blank (e.g. Branch transit number) will open the keyboard allowing you to enter data. 3
Claims Submitting a Claim The Claims screen allows you to view your recent claims, use the advanced search function or submit a new claim (explained in the section Submitting a Claim). You can submit a new claim from the Home Screen: or the Claims screen: Submitting a claim is done in 6 steps: 1. Selecting the type of claim 2. Selecting the patient or claimant 3. Selecting the service provider 4. Uploading your receipt(s) 5. Inputting the claim details 6. Final Review. 4
1. Select the type of claim Select the appropriate claim type. Note that claims for eye exams and glasses must be submitted under Vision. 4. Upload your receipt(s) When submitting a claim, a picture of your receipt of service is required. You may take a picture of your receipt or download a picture from your album. 2. Select the appropriate patient or claimant If you have more than one receipt you can add it to the claim. Note that there is a limit on the amount of data that can be submitted. 5. Input Claim Details Details are required for each new claim. Tap the Add + button. 3. Adding and Selecting a Service Provider You will need to populate the app with your service provider. Enter all applicable information. Tapping Select an item brings up an item list. Tap the correct service/procedure code. You must enter all information, except for the Unique No and Email text boxes. 5
An exclamation mark will appear if a field is not completed correctly. Tapping on the calendar will pull up a date selector. Initial Visits This blue box should be selected if this is the first time visiting this provider. For example, if you see a chiropractor provider once, and then see a different provider on your next visit, you would check off the initial visit box. Claim Total/Dentist s Fee/Lab Charge Enter in the appropriate amounts for each claim expense. Tap Done once all details are entered. If you have any other claim details to be entered you can click the Add+ button again and repeat the process 6. Final Review The final screen provides a summary of the claim. Once you are ready to submit the claim, tap the Submit button. The following will popup: Important You cannot edit a claim once it has been submitted. You must retain all original receipts. Sirius Benefit Plans Inc. reserves the right to request submission of receipts. All fraudulent submissions will be prosecuted. Tap Yes to submit your claim, or tap No and you will return to the final review screen. 6
Recent Claims This screen displays the procedure, the service date, the amount submitted, the amount that is payable and the status of the claim Paid (P), Rejected (R) or In Progress (I). A submitted claim will show up as In Progress. Once the claim is adjudicated, the claim will change to either Rejected or Paid. Tap a claim to display detailed information. Advanced Search The Advanced Search link enables you to look for a specific type of claim, process date (start and end dates) or claimant 7
Coverage Settings Tapping the Coverage tab shows coverage status, coordination of benefits, and your EHC benefit balances. Please be advised lifetime maximums and balance maximums that are extended over multiple years cannot be tracked using this app. The Settings tab allows you to change your preferences, view your insurance cards, see information about the application or Sirius Benefit Plans and logout of your profile. 8
