Health Savings A First Midwest Health Savings Account (HSA) is a checking with interest account that offers a different way to pay for health care. You can pay for current health expenses and save for future qualified medical and retiree health expenses on a tax-free basis. 3 Statement Savings This is a simple, interest-bearing account that comes with a personal savings register upon request, allowing clients to continue monitoring their savings in a fashion similar to that of a traditional passbook. If you would like to request a personal savings register or have a First Midwest Bank Debit or ATM Card associated with your savings account, please call 1.800.322.3623 on or after the Effective Date. Additional Services for Personal Accounts Direct Deposit If you have an existing direct deposit into any deposit account, it should continue seamlessly without interruption. Automated Phone Banking We are excited to offer you 24/7 access to First Midwest Bank-Line beginning on the Effective Date. Once the conversion is complete, First Midwest Bank-Line can be accessed by dialing 1.800.322.3623. The first time you access First Midwest Bank-Line, you will need to enter your account number. When prompted for your 4-digit Personal Identification Number (PIN), enter the last 4-digits of the primary account holder s Social Security Number. We recommend changing your 4-digit PIN at your earliest convenience for security purposes. You may also contact First Midwest Bank Customer Care at 1.800.322.3623 for assistance. If you currently have accounts set up to allow phone transfers, that functionality will remain the same. For more information regarding First Midwest Bank-Line, please refer to the Instant Answer Center, our knowledge database at FirstMidwest.com/GetAnswers. Please refer to the separate Important Dates Schedule for specific information regarding availability of Automated Telephone Banking. Online Banking Your Online Banking and Bill Payments will transfer to First Midwest s Online Banking system making the transition easy and convenient. You will receive a separate communication prior to the effective date with details pertaining to your login, password and initial access. We strongly recommend that you sign on to Peoples Bank s Online Banking and save all transaction history and any other information that you deem important for the upcoming tax season. Please refer to the Important Dates Schedule for specific information regarding Peoples Bank s Online Banking availability. After the Effective Date, please visit Firstmidwest.com/OnlineBanking to get started. Upon your first login to First Midwest Bank s Online Banking using the User Name and Password information you will receive prior to the transition date, you will be asked to verify your identity by selecting one of two methods: 1. Receive a single-use code via phone call or SMS message to a cell phone number on file; or, 2. Answer a series of security questions sourced from public information. You will then be presented with the terms and conditions that apply to your use of the service. Please read these terms carefully. You must also consent to receive disclosures electronically. Once you have successfully completed these steps, Online Banking will be ready to use. You can also download and begin to use First Midwest Bank s Mobile Banking App available at no cost for Apple and Android smartphones. Just search for First Midwest Bank in the appropriate app store and look for the purple app icon. First Midwest Bank s Online Banking system contains many similar features to Peoples Bank s Online Banking service including transfers, alerts and Bill Pay. Plus, there are several new features available to you in First Midwest Bank s Online Banking system: Additional account alerts including information regarding debit card purchases, account balances and other security alerts. Access to estatements. The ability to perform external transfers with the following limits - $2,500 for a single item, $7,500 daily, and $10,000 monthly. Access to the financial center dashboard, an intuitive interface which will provide maximum control over your account. If you have debit card transactions that have not cleared your account by the time of the transition to First Midwest, the balance you will see within First Midwest Bank s Online, Mobile and ATM systems may be higher than what is actually available in your account(s). This will correct itself over the first few days following the completion of the transition. All other accounts will be charged a daily overdraft fee through analysis after the second (2nd) calendar day the account is overdrawn. Additional Services for Business Accounts Business Internet Banking All Peoples Bank Internet Banking users will receive First Midwest Bank Business Express Online Banking Services. You will receive additional information regarding our service, including a new company ID and password, prior to transition. Your Business Express Online Banking service will be available on the Effective Date. Refer to the Important Dates Schedule for the date on which clients will no longer be able to access their account information via the Peoples Bank Business Internet Banking system. We strongly recommend that you download any account history, bill payment payees and ACH template information (if applicable) you wish to retain prior to the Transition Date. Automated Phone Banking We are excited to offer you 24/7 access to First Midwest Bank-Line beginning on the Effective Date. You can access First Midwest Bank-Line by dialing 1.800.322.3623. The first time you access First Midwest Bank-Line, you will need to enter your account number. When prompted for your 4-digit Personal Identification Number (PIN), enter the last 4-digits of the Tax Identification Number for Business accounts. We recommend changing your 4-digit PIN at your earliest convenience for security purposes. You may also contact First Midwest Bank s Customer Care at 1.800.322.3623 for assistance. Any fees assessed through analysis will be charged on the first business day of the following month. GENERAL ACCOUNT INFORMATION. Personal and Business Debit and ATM Cards You will be receiving a new First Midwest Bank Debit or ATM Card with a new card number to replace your Peoples Bank Debit or ATM Card. If you have any bills that are being paid automatically via your Peoples Bank Debit or ATM Card, you must notify the company of your new Card number and expiration date. In case of errors or questions about your electronic transfers, please refer to the Retail Account Agreement and Disclosure or the Business Account Agreement and Disclosure (both contained in the CIT Notice). You will receive your new First Midwest Bank Debit or ATM Card prior to the Transition Date. Prior to receipt of your new card, you will receive a mailing informing you of your new Personal Identification Number (PIN). Please note that your PIN will change; therefore, a PIN notice mailing will be mailed to you prior to the Effective Date. You will need your PIN to activate your new First Midwest Bank Debit or ATM Card. Simply follow the instructions that accompany your new Card to activate it. You may also change your PIN at this time. Our momentum starts with personal commitment. Commitment that gives us and our clients the power to make their lives better. Their companies stronger. You must begin using your new First Midwest Bank Debit or ATM Card for purchases and stop using your Peoples Card on the dates outlined on the Important Dates Schedule. On the Effective Date, you will have free access to First Midwest Bank s network of over 100 ATMs plus 50,000 surcharge-free ATMs worldwide in the Allpoint network (Debit Cards only). If you have debit card transactions that have not cleared your account by the time of the transition to First Midwest, the balance you will see within First Midwest Bank s Online and ATM systems may be higher than what is actually available in your account(s). This will correct itself over the first few days following the completion of the transition. Allpoint ATM Access Like People s Bank, First Midwest Bank has joined forces with Allpoint, America s largest surcharge-free ATM network, expanding our network from 100 local machines to over 50,000+ surcharge-free ATMs nationwide. ATM Deposits Peoples Bank s ATMs will not be able to take deposits for a period prior to the Effective Date. If you deposit funds to cover checks or current account activity during this period, please do so at your current Peoples Bank branch to ensure funds are available as quickly as possible. Refer to the Retail Account Agreement and Disclosure or the Business Account Agreement and Disclosure (both contained in the CIT Notice) for complete details regarding our Funds Availability Policy. Please refer to the separate Important Dates Schedule for specific information on ATM Deposits. Overdrafts and Returned Items For purposes of overdraft processing, First Midwest Bank determines your account balance at the end of each business day. To determine your account balance, we begin with your ledger balance (when funds are credited before collection occurs) and (i) subtract any holds on your account; and (ii) add any deposits available for withdrawal that day (based on our Funds Availability Policy) and any credits to your account. From this amount, we then subtract any items received for payment during that business day, in a defined posting order, and any fees and charges assessed by the Bank against your account. An overdraft occurs when during this determination process, the Bank receives an item for payment and your account balance is not sufficient to pay the item. You may be charged an Overdraft Fee or Returned Item Fee for each item that overdraws your account, whether the overdraft was caused by you or another person authorized to use the account or by fees applicable to the account. You may refer to the Overdrafts/Returned Items, Processing Order and Fees and the First Midwest Bank Funds Availability Policy found in the Retail Account Agreement and Disclosure and the Business Account Agreement and Disclosure (both contained in the CIT Notice) for additional information. Overdraft Privilege Overdraft Privilege protects you from the additional costs and inconvenience of an overdraft on your account by giving you a reserve for emergencies and unexpected situations. If you are eligible for this program you will receive a separate notification after the transition date. Funds Availability Our policy is to make funds from your cash, wire transfers, electronic direct deposits and other electronic transfers, available to you on the business day we receive the deposit. Funds from other deposited items, such as checks, will be made available on the first business day after we receive the deposit. Refer to the Retail Account Agreement and Disclosure or the Business Account Agreement and Disclosure (contained in the CIT Notice) for complete details. Interest Calculations First Midwest Bank uses the daily balance method to compute interest. Under this method, interest is calculated by applying a periodic rate to the full (collected) balance in your account each day. The collected balance is the balance that reflects the deposits you have made and for which First Midwest Bank has actually received credit. This occurs when First Midwest Bank collects the funds (receives credit) from the other bank. Direct Deposits and Electronic Funds Transfers for Personal and Business Accounts All electronic funds transfers will remain in place. In most cases, everything will be automatically changed, and you will have no interruption in your direct deposit or transfer. Beginning on the Effective Date, all future direct deposits, preauthorized or other electronic transfer relationships that you establish should use First Midwest Bank s routing number 071901604. Personal and Business Account Transfers All established transfers will remain in place. Please refer to the separate Important Dates Schedule for specific information related to Personal and Business Account transfers. Personal and Business Account Statements You will receive a statement for transactions prior to the Effective Date from Peoples Bank. After the transition is complete, the next statement you receive will generally arrive around the same date that you have been receiving your current statement. In some circumstances, savings account statement dates may vary. IRA statements will be generated annually at the end of the calendar year. Any service fees incurred will be assessed on a monthly basis. You can view free check images after the Effective Date for all of your personal checking accounts using our free Online Banking service. First Midwest Bank will continue to store your cancelled checks. Please refer to the separate Important Dates Schedule for specific information about Personal and Business Account Statements. Certifi cates of Deposit (CDs) CDs will automatically renew into the CD terms and conditions of First Midwest Bank on the date of renewal. CD account holders will receive advance notice of maturity and a renewal notice along with complete details regarding the terms for renewal of the CD. CD statements will only be produced if there is a combined statement relationship. Upon maturity, please call First Midwest Bank s Customer Care at 1.800.322.3623 or visit your Personal Banker to discuss the CD solution that s right for you. Individual Retirement Accounts (IRAs) An individual with multiple IRAs may consolidate or aggregate the separately calculated Required Minimum Distributions (RMDs) and withdraw the total amount from any one or more of the IRAs. If you are taking the RMD from another institution, you must complete a Waiver of Distribution from First Midwest Bank, which informs us that you will meet your RMD with another institution. If you do not meet your RMD with First Midwest Bank and have not completed a waiver form, a check for the required amount will be sent to you at the beginning of December of the year the distribution is required. We have waived the RMD for you this year to make the transition to First Midwest Bank as convenient as possible. The custodianship of the IRA will change from Peoples Bank to First Midwest Bank on the Effective Date. Please refer to the separate Important Dates Schedule for specific information on the Effective Date for Individual Retirement Accounts (IRAs). Safe Deposit Boxes Please refer to the Retail CIT Notice for changes to your Safe Deposit Box Agreement. Clients who continue to maintain a Safe Deposit Box should continue to send rental payments to the branch where your box is located. Any additional pricing changes will be communicated in writing prior to the contract expiration date. Personal Loans and Lines of Credit Prior to the Effective Date, any questions regarding your loan or line of credit should be directed to Peoples Bank at 1.847.368.0100. After the Effective Date, for information regarding your loan or line of credit, please call the First Midwest Bank Customer Care Center at 1.800.322.3623. Loan Payments Mailing All Peoples Bank loan clients will be receiving an additional communication from First Midwest Bank to assist them in successfully redirecting their loan payments to First Midwest Bank. Future loan billing statements produced after the Effective Date will replace current Peoples Bank billing statements and contain the First Midwest Bank loan payment mailing address. For clients with auto debit payments, there will be no change. Your auto debits will continue unaffected by this transition. Tax Reporting Form 1098 Interest paid in 2015 on all open loan accounts that transfer to First Midwest Bank on the Effective Date, will be reported by First Midwest Bank. If your loan with Peoples Bank was paid off prior to the Effective Date, any interest paid in 2015 will be reported by First Midwest Bank. Form 1099 All interest earned and paid during 2015 will be reported by First Midwest Bank. Please refer to the separate Important Dates Schedule for specific information on the dates applicable for Tax Reporting. Wire Transfers Business and Personal clients who currently request wire transfers by email, fax or telephone, will receive a separate communication prior to the Effective Date. A wire transfer authorization will need to be completed, listing the individuals who will be authorized to request a wire transfer from a specific account(s). A Personal Identification Number (PIN) will be issued to these individuals. Beginning on the Effective Date, clients will request a wire directly with First Midwest Bank s Wire Transfer Department by calling the PIN line at 1.888.362.9473 on business days between the hours of 9:00 AM to 3:00 PM. Peoples Bank received international incoming wires through another financial institution on your behalf. For clients who currently maintain accounts at both banks: 1. Your converted deposit accounts will be separately insured from any accounts you already have at First Midwest Bank for six months after the Effective Date. For example, with an Effective Date of December 7, 2015; separate insurance coverage will continue until June 8, 2016. 2. For time deposits, the separate deposit insurance will continue until the first maturity after the expiration of those six months. Refer to the exceptions below. Exceptions that apply to time deposits: Time deposits that mature within the six month period and are renewed for the same amount with or without accrued interest added to the account and the same term as the original deposit will continue to have separate insurance coverage until the next maturity date. For example: Merger occurs December 7, 2015. The six month extension period expires June 8, 2016. Jane Doe s CD matures January 9, 2016, within the six month period. Jane Doe does not make any changes to this account and lets it renew automatically. Jane will receive separate insurance coverage until her NEXT maturity. Time deposits that mature within the six month period and are renewed for a different term or amount will not be insured separately after the six month period expires. For example: Merger occurs December 7, 2015. The six month extension period expires June 8, 2016. John Doe s CD matures January 9, 2016, within the six month period. John Doe changes the length of time the dollars are invested (term). John s separate insurance coverage would end on June 8, 2016, at the end of the six month period. 3. During this six month time period, the depositor must rearrange their deposits if necessary to maintain appropriate levels of FDIC insurance coverage. Clients will stay at the branch approximately 10 minutes to sign a required disclosure, which will list the transfer amount, First Midwest Bank s transfer fee, exchange rate, and other fees. Your transaction can be cancelled for up to thirty (30) minutes and can be communicated either verbally or in writing. All monies including fees would be refunded. 26 X 27 X
