Leuzinger High School Class of 1963 50 th Reunion Questionnaire We will be having our 50 th Reunion before you know it, and we are looking forward to seeing so many of our old friends! Each classmate attending the reunion will be given the Reunion Memory Book that evening. Part of the memory book will be copies of this questionnaire as filled out by our classmates. But we need your help! It's probably been a while since you've been given any homework, but this assignment is important and fun. You won't be graded on it, but you still need to finish it. We need you to complete the questionnaire and return it to us. By all means, don't put it off until the last minute. As soon as we start receiving your completed questionnaires, we will begin the process of putting together your "Reunion Memory Book". It will contain all kinds of neat stuff, including information from this questionnaire, a few old photos etc. Speaking of old photos, why not get a start at looking into those old shoeboxes of snapshots you took in high school? But wait, I digress back to the questionnaire. It is very important for everyone to complete the questionnaire in some way, even if you are unable to attend the reunion. After the reunion, we will give those of you who cannot attend information how you can also get a copy of the Reunion Memory Book. At the bottom of the questionnaire, you will see instructions on how to return your filled out questionnaire. One more thing, please feel free to skip some questions or to only provide limited information if you prefer. For example, you will see the option to include only your city and state in the address field. The fields marked with (*) are those that are only used by the Reunion Committee to contact you in the future. We need this information to send the Memory Book to you. 1

Go ahead, tell us a bit about your kids and grandkids! You know... how many, their names, ages and the one thing you tend to brag about them when given the chance. Well, now is your chance! No kids or grandkids? How about that favorite niece, nephew, aunt, friend, etc... who makes you smile? Don't forget to tell us about your pets (We have some real animal lovers out there!) Are you retired? If so, how are you spending your time? If you have not retired yet, what is your current occupation? If you are retired, what was your prime occupation? What is the most interesting job you ever held? Where have you lived since graduation? What is the most interesting place that you have visited? What do you feel is your greatest accomplishment since high school? Feel free to brag here if you like! If you could go back to high school, what would you do differently? What is one thing you know now that you wish you knew in high school? What was the color and make of the car you drove in high school, or your first car? Where did you work in high school? What do you miss most about high school? Do you have a favorite high school memory? What was your favorite song in high school? What kind of music do like now? Where was your favorite place to hang out in high school? 3

Where is it now? Do you have a most embarrassing high school moment you would like to share? What news event that occurred during high school stands out to you? Who was your favorite teacher and why? What friends are you most anxious to see at the reunion or at least read their questionnaire? Who did you go with in high school, or who did you date the most? Did that relationship continue after we graduated? We have touched on a few fun topics; if there is something else you would like to share about your life these past 50 years, please add it here! Finally, what did you like best about your high school experience at Leuzinger, and what did you like the least? * Please fill out these fields. See instructions on how to return the questionnaire on the next page. 4

How to Return Your Completed Questionnaire If you received the questionnaire via email, probably the easiest way to return it to us is to Forward the email to donstouder@ca.rr.com. When you forward an email, you can modify it as in the forwarding window within your email program. Doing it this way you never have to leave your email program. You simply go to the questions and type your answers where the lines are. You can delete the unused lines if you want to. This email also includes the questionnaire as a PDF file attachment. Another way to return your completed questionnaire is to print the attached PDF file and write your answers in the space provided. Then mail it to the address below.
