FY2016 Open Enrollment Starts 5/01/2015 Ends 5/22/2015 Effective Coverage Date 07/01/2015 Contact Information Sonia Solivan Ext. 2219 Katy Johnson Ext. 2425 Email: benefits@clcillinois.edu Make an informed decision about your health & welfare plan selections and review the resources available in the Human Resources Benefits Open Enrollment Page. http://dept.clcillinois.edu/per/openenrollment.html Guide to Enrolling for Health Benefits through Employee Self-Service Step-By-Step Instructions The steps outlined in this guide will assist you with enrolling into FY2016 Health Benefit Plans at the College of Lake County. The guide includes the following parts: Part 1. Accessing Employee Self- Service. Part 2. Reviewing Benefit Plans. Part 3. Enrolling in FY2016 Benefit Plan. ** Note that the screen shots used in this guide are just an example and displayed benefit plans and coverage amounts may differ from what you will see on the screen when you navigate through Employee Self Service.

Part 1. Accessing Employee Self Service. 1.1. You will need Internet access, a Username and a Password to connect to CLC Employee Self-Service. Contact the IT Help Desk at helpdesk@clcillinois.edu or 847-543-4357, if you need your Username and/or Password. 1.2. In the Web Browser, type the following web page address: http://clcweb.clcillinois.edu/. 1.3 Click on myclc If you are not logged in already, the following screen will appear. Enter your CLC Username and Password, and click Login button. College of Lake County Human Resources Department Page 2

On the left hand side of the screen, click Employee Self Service. 1.4 Home page of the PeopleSoft center system will open. Click on the Main Menu link then the Self Service link. (Menu Items displayed on your screen may differ from what is displayed in the image below). College of Lake County Human Resources Department Page 3

1.5. Click on the Benefits link then Benefits Summary. (Self Service sections displayed on your screen may differ from what is displayed in the image below). Part 2. R eview Current Benefit Plans. 2.1. Click on each underlined link to review your current Health Benefit selections and covered dependents. After reviewing your specific Benefit plan information, click on the Return to Employee Benefit Summary link at the bottom of the page to return to the Benefit Summary page. Click on each blue link to view information about your current Benefit plan selections. If you are currently enrolled in the Flexible Spending Plan (medical and/or dependent care) and would like to continue enrollment, you must re-enroll for FY2016. Part 3. Enrolling in FY2016 Benefit Plans 3.1. Once you are ready to enroll into Benefit plans, navigate to the Benefits Enrollment page by: - Selecting Benefits at the top of the page then Benefits Enrollment. 3.2. Click on the Select button to access the Benefits Enrollment page and to begin enrollment in your Benefit plans. Your Event Status should show as Open until you have submitted your Benefits Enrollment. Once your selections are submitted, the Event Status will update to Submitted. If initially the Event Status is not Open, please contact the Benefits Department at benefits@clcillinois.edu. 3.3. Your benefit options will display under the Enrollment Summary heading. Click on each Edit button to review and edit your Enrollment Summary. Even if you do not want to make changes to your current Benefit plans, you still must click edit on all listed plans. Your current coverage selections are listed under Current and defaulted/updated selections are listed under New. You must click on the Edit button See Step 3.4 for details New FY16! All employees now have the choice to waive Dental and Vision Coverage! You must click on the Edit button See Step 3.5 for details College of Lake County Human Resources Department Page 5

3.4. Medical/Dental/Vision Insurance Plan options available for you will be displayed, as well as the associated cost based on the enrollment level of coverage. Note: Change in medical plans! You have the choice between HMOI Network Only and PPO Standard. (The Network Only PPO is no longer available) The selected radio button (see below) indicates your current benefit plan choice. If you are currently enrolled in the Network only the radio button will indicate HMOI Network Only. Please ensure that you make a choice! If you wish to change your current plan, click the radio button next to the option of your choice. Dependents Review the names in the Enroll Your Dependents section. All eligible dependents will appear in this section. If the dependent is currently covered, they will already have the check box selected. Under the enroll column either: - check the box next to the name to ENROLL additional dependent(s). - or uncheck the box next to the name to REMOVE dependent(s). Checked Radio Button indicates Selected Medical Plan What can you change in the Plan? - Your Benefit options - Enroll/Remove eligible Dependents - If enrolling eligible dependents for the first time, you must provide documentation showing dependent status and complete a dependent/beneficiary form. Contact HR for forms or click on http://dept.clcillinois.edu/per/ope nenrollment.html Eligible Dependent Name appears here College of Lake County Human Resources Department Page 6

Once your selections have been made, click on the Continue button, the system will display the page with your selections and estimated cost. Eligible Dependent Name appears here Click on the Update Elections button to store your choices or Discard Changes. Once you click the Update Elections button you will return to the Open Enrollment page where you can continue with enrolling in other Benefit plans. Important! You must repeat the Steps for Dental and Vision plans, regardless of whether you plan to enroll or waive. College of Lake County Human Resources Department Page 7

3.5. Flexible Spending Medical Reimbursement and Dependent Care Reimbursement. You must edit all flexible spending accounts listed on the Enrollment Summary page, regardless of whether you plan to enroll or waive. If you are currently enrolled and would like to continue enrollment in the Medical Reimbursement and/or the Dependent Care Reimbursement, you must re-enroll for the new FY2016. Important! By default the FY2016 selections for flexible spending accounts will be Waived. If you decide to enroll in the flexible spending account, you must indicate so by clicking on the applicable radio button. If you decide to enroll, you must enter the annual dollar amount you would like to contribute. Once you click on the Update Elections button, system will display the page with your selections and estimated cost. See the display on next page. Once you click on the Update Elections button, you will return to the Open Enrollment page, where you can continue with enrolling/waiving other Benefit plans. Important! You must repeat this Step for the Dependent Care Reimbursement plan regardless of whether you plan to enroll or waive. College of Lake County Human Resources Department Page 9

3.6. Submit your Health Benefit Plan selections. Your enrollment changes will not be complete until you click on the Submit button at the bottom of the Open Enrollment page. When your Benefit Enrollment screen reflects the desired information under the New: for Medical, Dental, Vision, and Flexible Spending (Medical Reimbursement and/or Dependent Care Reimbursement), then click on the Submit button to finalize your enrollment. Review your New Health Benefit Plan selections for FY2016 that will be effective on 07/01/2015 Review the summary of your estimated cost per-pay-period. *If you would like to a print out of your selections, just hit File>Print in your internet browser before you hit submit. 3.7. Confirm Your Submission *If you are finished with your benefit choices click Submit. *If you are not ready to submit your benefit choices click Cancel to return to the Enrollment Summary. Once you click on the Submit button, system will display the confirmation page. You will receive a confirmation statement from HR the week of May 25 th. 3.8 Sign out of Employee Self Service. Click on the Sign out link to disconnect from Employee Self Service. College of Lake County Human Resources Department Page 11
