CASE NO: Petitioner v. DIVISION: Respondent Shared Parenting Visitation Schedule A. The parties shall have shared parental responsibility. Parental responsibility for the minor child shall be shared by both parties, and each party shall hereby affirmatively participate in shared parenting. Shared parenting as referenced in, and required by, 61.13, Florida Statutes (2005), includes, but is not limited to, the following rights and responsibilities: 1. The parents retain equal parental status and have the same rights to, and responsibilities for, the child; 2. Each parent shall promote a good relationship between the child and the other parent. Both parents shall attempt to insure that the child has unhampered contact and free access with both parents. Each parent shall make all reasonable efforts to encourage and facilitate communication between the other parent and the child in person, by telephone and through the mail; 3. The parents shall use all reasonable efforts to maintain free access and to create a feeling of affection between themselves and the child; 4. Neither parent shall do anything to hamper the natural development of the child's love and respect for the other parent; 5. Neither parent shall make, or cause others to make, derogatory or negative comments about the other parent in the presence of the child; 6. Each parent shall confer with the other parent regarding decisions about the child s religious upbringing, discipline, financial matters, moral training, social and recreational activities, and non-emergency medical and dental care; 7. Each parent shall notify the other party immediately of any serious illness or accident affecting the child and to make sure both parents are entitled to authorize emergency medical treatment for the child; 8. Both parents are entitled to obtain information from any person about the child at their own expense. When one parent obtains such information in a written report, a copy of the report shall be given to the other parent; 9. Each parent shall provide the other parent with copies of the child's school work, report cards, testing results, etc.; Initials of Petitioner: Initials of Respondent: Page 1 of 5

10. Each parent shall list the other parent on any and all educational, medical, dental, psychological, etc. forms so that the other parent has access to the child's records; 11. Each parent shall list the other parent on the child s school, child care, or other institution/organization, emergency notification and pick-up authorization cards; 12. Each parent shall keep the other parent informed of his/her current and all future addresses, telephone numbers, and e-mail addresses; 13. It is acknowledged that third parties (e.g. spouses, boy/girlfriends, grandparents, etc.) may become involved in the child s daily life. While the efforts of such third parties are often necessary and invaluable to the parents, the parents shall attempt to communicate with each other in reaching decisions affecting the welfare of the child and they shall endeavor to limit third party interference in their parental rights and responsibilities; and 14. In addition to the visitation as ordered by the Court, the right of the child to have access to the non-primary residential parent for the purposes of visitation shall be liberally construed. Particularly, it is contemplated that the non-primary residential parent may have additional visitation as agreed upon by the parties. B. The shall be designated as the primary residential parent and the shall be designated as the secondary residential parent with the following visitation: 1. Alternating weekends, from (day) at (time) to (day) at (time), commencing on (date). 2. Weekday visitation, every (day) from (time) to (day) at (time), commencing on (date). C. Holidays: In the odd-numbered years, mother has Easter/second half of Spring holiday, Fourth of July, Labor Day weekend, Christmas Day. The Father has Dr. Martin Luther King Day, Memorial Day weekend, Thanksgiving holiday, and Christmas Eve. In the even-numbered years, the schedules are reversed. 1. A holiday that falls on a weekend should be spent with the parent who is supposed to have the children for that holiday, as defined above. The next weekend is to be spent pursuant to the alternating weekends defined above. School holidays not specifically covered herein which fall on a Monday or Friday shall be spent with the parent who would normally have the school-age child for visitation/custody on the weekend adjacent to the school holiday. If the school-age child has pre-school-age siblings, the siblings visitation will also extend over the school holiday. Pick up time is Initials of Petitioner: Initials of Respondent: Page 2 of 5

6:00 p.m. the day before the Friday school holiday. Return time is 6:00 p.m. on the day of the Monday school holiday. 2. No less than one week prior notice shall be provided by the secondary residential parent to the primary residential parent regarding cancellation or adjustment of holiday visitation time as set forth herein. 3. Summer holidays: Each parent shall be entitled to spend two continuous twoweek periods with the children for summer visitation during which the other parent may have telephonic contact on a reasonable basis. The parents shall confer to schedule such visitation. If unable to agree, the secondary residential parent shall have the last two weeks of June and the last two weeks prior to the children returning to school and the primary residential parent shall have any other two week periods not in conflict with the above-stated. 4. HOLIDAY VISITATION HOURS: Hours for parents are as follows: Dr. Martin Luther King begins at 6:00 p.m. the preceding Friday to 6:00 p.m. Monday. Spring Holiday begins the day after school lets out until the day the before school resumes. The break is divided in half, with each parent having an equal amount of time with the children. Easter weekend begins at 6:00 p.m. Friday before Easter Sunday and until Sunday at 6:00 p.m. Mother s Day and Father s Day are to be spent with the appropriate parent from 9:00 a.m. until 6:00 p.m. Memorial Day weekend begins at 6:00 p.m. Friday and continues until 6:00 p.m. on Monday. Fourth of July begins at 6:00 p.m. July 3 rd through 6:00 p.m. July 4 th, unless the holiday falls on a Monday or a Friday, then the adjacent weekend is included. Labor Day weekend begins at 6:00 p.m. Friday and continues until 6:00 p.m. on Monday. Thanksgiving holiday begins at 6:00 p.m. the night before Thanksgiving and continues through 6:00 p.m. the following Sunday. Christmas Eve begins at 6:00 p.m the day after school lets out until 9:00 a.m. Christmas morning. Christmas Day begins at 9:00 a.m. December 25 th and continues through 9:00 p.m. New Year s day. Birthdays. A child shall celebrate his/her birthday in the home of the primary residential parent, unless the birthday falls on a visitation day. The parent not having the child on his/her birthday may have the child from 5:30 p.m to 8:30 p.m. the preceding day. E. Waiting. The children and the parent have no duty to await pick ups for more than one half hour of the scheduled time. F. Cancellations. If a child is ill, the primary residential parent shall give 24-hour notice, if possible, so appropriate plans can be made by the secondary residential parent. Visitations cancelled due to the illness of a child should be made up the following weekend. The secondary residential parent should give 24 hour to cancel. The time canceled by the secondary residential parent is forfeited. The time canceled by the primary residential parent is to be made up the following weekend. G. Flexibility. This schedule provides a basis for minimum contact between the children and the secondary residential parent. However, liberal visiting arrangements are encouraged inasmuch as contact with both parents is important to the children. Changes or modifications will be referred to the Family Mediation Unit for dispute resolution of Hillsborough County, Florida. H. Injunction Prohibiting Removing the Child. The Court hereby prohibits the parties from permanently relocating the minor child greater than 50 miles without a court order, the written consent of parties, or compliance with Fla. Statute 61.13001. However, the parents may temporarily remove the child from the state of Florida without prior court order or the written consent of the other party for the purpose of vacation or holiday travel, if two weeks prior to departing, that parent provides to the other parent a written itinerary which covers dates, times, locations and a phone number where the child can be contacted during the holiday or vacation.
