San Juan College High School Application for Class of 2020 Thank you for considering San Juan College High School (SJCHS.) Before completing the application, please give careful consideration to the information below to determine if SJCHS is a suitable educational experience for you. In completing the application, please make special note of the expectations you and your parent/guardian will agree to if selected to attend SJCHS. The primary purpose of this high school program is to provide an opportunity for students to graduate with a High School Diploma and a two-year transferrable college degree or associate s degree, in four years. Eligibility: The student population of SJCHS reflects and values the diversity of its participating school districts. In situations where the number of applications exceed the number of allotted seats (60) all completed applications will be reviewed and a random lottery will select the students. SJCHS is seeking students who meet one of the following criteria: Students who are first generation college students. Students who would benefit from accelerated academic instruction and a college learning environment. Students who are seeking a nontraditional high school setting. In addition, the following are key attributes of successful SJCHS students: Maturity, motivation, and the ability to make responsible, independent, and productive choices which contribute to success and growth, both at the high school and college level. Desire to be intellectually challenged and experience rigorous high school and college courses. Ability to work cooperatively, making education a priority. Students must be engaged in communication, leadership, critical thinking, and problem solving skills. Desire to effectively and appropriately use technology toward educational purposes and enrichment. Attending SJCHS is a commitment to your future and your education! 1

As a San Juan College High School Student, I agree to: Make education a high priority in my life, participate in a positive manner and work to achieve and exceed my potential. Be punctual and maintain exemplary attendance in all classes. Take responsibility for my own learning, behavior and success. Devote adequate time, every evening, for homework, studying, and reading. Show respect toward everyone including: teachers, peers, staff, and community members. Behave in a manner that shows respect for the college, school facilities, and equipment. Maturely handle the freedoms and scheduling provided in a college setting. I understand that my admittance to this program is a privilege and not a right. Demonstrate the ability to make mature, independent, and productive choices while accepting responsibility for those choices. Understand that SJCHS is a high school without competitive athletics. SJCHS will be considered my high school, and I realize I cannot participate in athletics or activities at another high school, including my residential high school. Maturely handle flexible scheduling and make productive use of unscheduled time. Adhere to the technology agreement of SJCHS, FMS, and SJC. Complete all SJCHS graduation requirements, including all required State and District testing. Complete all SJC Career Pathway requirements. Keep my parents informed of my academic progress at SJCHS. In addition, I understand that I may be sent back to my residential school if I exhibit any of the following: poor attendance, failing grades, lack of effort, failure to meet SJCHS academic standards, and/or failure to adhere to SJCHS / FMS and San Juan College policies. I understand that SJCHS will use a random lottery should the number of applicants exceed the 60 slots. SJCHS will use the same process to determine a waiting list, should one be necessary. PART II: Student Letter of Interest Coming to SJCHS is a commitment to your future and your education. Therefore, the selection committee would like to know more about you and your future goals. Please write a letter of interest explaining why you feel SJCHS is the right choice for you. At a minimum, include responses to the questions listed below. SJCHS PARENTAL AGREEMENT/CONTRACT: If my child is accepted, I agree to the expectations below: Provide adequate time each evening for my child to do homework, study, review, and read. Support regular attendance, call the school on any day of absence and provide proper documentation upon the students return. Attend and participate in parent/school functions. Stay current regarding SJCHS activities by reading materials sent home and visiting the school website often. Expect and follow up on progress reports and report cards. Encourage and expect that my child will complete their college degree, even if they have to take college classes after their high school graduation date. Contact the school if I have questions. Understand that SJCHS is a high school without competitive athletics. Recognize that my child is responsible for adhering to the technology agreements of SJCHS, FMS, and San Juan College. Accept the collaborations and inter-disciplinary curricula as set forth by SJCHS and SJC. Understand that college employed instructors teach SJC Courses and any disputes regarding grades, course content, or other issues are to be addressed with SJCHS School Administration. Understand that my child may be referred back to his/her residential high school if he/she exhibits any of the following: poor attendance, failing grades, lack of effort, and/or failure to adhere to the SJCHS and SJC school/college policies and academic expectations. Understand that SJCHS will use a random lottery to identify the students who will be admitted to SJCHS. PART VI: Student Agreement (Please initial to the left of each statement & sign below) I realize that being admitted to SJCHS provides a chance to foster my educational opportunities for success. By signing below, my parents and I understand that I am committing to at least one full academic year of attendance at SJCHS. The SJCHS staff will complete an End of Year Assessment of each student to determine their potential future status at SJCHS. I understand that this is an adult educational setting, therefore, my behavior must be conducive to learning without disruption. I agree to act as a young adult who can handle this level of adult work and interaction while using SJCHS/SJC facilities. I agree to abide by the rules and regulations established by SJCHS and SJC. I understand that daily attendance is required for academic success. I agree to follow the SJCHS and SJC attendance policies and be present whenever classes are in session. I understand that maintaining a cumulative college program GPA of 2.0 is required by SJCHS to earn an associate s degree. Therefore, I agree to work to maintain a C or above in all of my high school and college classes. I understand SJC employs instructors to teach SJC Courses. Any disputes regarding grades, course content, or other issues should be addressed to SJCHS High School Administration first. I understand the successful completion of one of the following: a service learning/graduation/internship/capstone project is required of all SJCHS students, and I agree to participate fully in all aspects of the project. Release of Information: I agree as a SJC and SJCHS student, regardless of my age, that SJCHS may release information regarding my enrollment, academic progress, discipline matters, or attendance to my parent(s) or legal guardian(s).
