Required for all activities requesting CME credit. A projected budget listing anticipated expenses and income AND a detailed agenda listing topics and presenters are *Submissions received less than 4 weeks prior to the activity launch date may result in an additional CME Rush fee. Obtain disclosures from all planning committee members and presenters Disclosures can be completed online or via paper form. The form must be completed by all individuals in a position to control content (planner, speaker, author, moderator, etc.). Individuals who have identified any financial relationships on their CME disclosure form must have their potential COI resolved prior to the activity. The primary method to resolve COI includes emailing all slides to CME for prospective review and approval. Application Faculty & COI Submit CME application materials and receive preliminary CME approval from the Department of CME Planning documents and supporting materials should be sent to CME no less than 6 weeks prior to the date of the activity*. The preliminary approval process takes approximately 3 to 10 business days depending on the completeness of the application materials. ***An application must be submitted and approved before any event advertisement/ marketing materials are designed/printed/distributed. Begin applying for educational grants (if applicable) Pharmaceutical and medical device manufactures can provide financial support for CMEcertified activities through educational grants. They cannot be involved with any planning, including identification or selection of speakers, topics, etc. All grant letters of agreement must be signed by the Department of CME on behalf of William Beaumont Hospital. Grants & Exhibits Prepare meeting materials/content and provide to CME CME must review and approve all content to ensure compliance. Meeting materials must be educational in nature and not promotion of a specific product or service. CME Information The CME department will develop the required CME information for you. All information on this document is required per ACCME Standards and Beaumont policies. Some information must be presented to learners prior to accessing the content and will be identified as such on the document provided to. May 2015
Process for CME Certification of Enduring Materials Develop post-activity assessment questions/process An assessment of the learner that measures achievement of the activity with an established minimum performance level must be incorporated into the activity. The CME Department utilizes a post-test module to meet this requirement. The Department seeking credit must provide CME with appropriate questions and answer responses for programming into the module. Faculty & COI Identify appropriate bibliographic sources The American Medical Association requires that all online activity provide learners with access to appropriate bibliographic sources to allow for further study. This can be done by providing references throughout the presentation or through a comprehensive reference list at the end of the presentation. Online Evaluation and Certificate Process Evaluations are developed by the Department of CME using Qualtrics online technology. CME certificates are issued online and data is automatically recorded in our CME database. Learners can print their own individual activity certificate or a transcript of all Beaumont CME events they ve attended for a date range. Additional evaluation questions can be added upon request. Begin marketing your event CME must review and approve all marketing materials prior to printing and/or distribution to ensure compliance. ***All advertisement/marketing materials must be reviewed and approved by the CME department before printing and distribution. No mention of CME credits can be made on marketing materials unless official CME approval has been provided by the CME department (one cannot state that CME credit is pending or being applied for or other similar language). Marketing Tools Additional Requirements Within 30 business days following the launch of the enduring material, please provide your assigned CME Coordinator with the following: 1. Final budget detailing actual expenses (due within 30 days) 2. Copy of all grant checks (if applicable) May 2015
Enduring Materials An enduring material is a certified CME activity that endures over a specified time. These include print, audio, video and Internet materials. Examples include monographs, podcasts, CD-ROMs, DVDs, archived webinars, as well as other web-based activities. American Medical Association (AMA) Requirements: To be certified for AMA PRA Category 1 Credit, an enduring material activity must follow the same requirements as a live course. In addition, the activity must provide Clear instructions to the learner on how to successfully complete the activity, including the minimum performance level that must be achieved on the post-test. Access to appropriate additional resources to allow for further study. An assessment of the learner that measures achievement of the educational purpose and/or objective(s) of the activity with an established minimum performance level. The minimum length of an enduring material that can be certified is 15 minutes of content (0.25 credits). We do not count time to complete the post-test or evaluation towards the credit amount. Accreditation Council for Continuing Medical Education (ACCME) Requirements: To be in compliance with ACCME Accreditation Criteria and Standards, an enduring material activity must follow the same requirements as a live course. In addition, If the enduring material is being created as a new standalone activity (not a product of a previous live activity), then a new CME application, etc. must be submitted (same process as a new live activity). If the enduring material is being created from a previous live activity, then you may be able to use much of the information provided on the application for the live activity with a few exceptions: - If the live activity consisted of multiple presentations and the identified gap(s) and objectives were more global in order to apply to the entire conference, then each presentation that is created as a new enduring material must have its own, individual gap and individual objectives (minimum 3) that apply directly to the content of that presentation. One presentation = use same application as for live Series of presentations = specific gap and objectives required for each presentation use Abbreviated CME Application for Online CME Recorded from a Live Presentation Questions for the department/joint provider: 1. Where do you want content to reside? On their own website (if so, what is the URL?) or on the Beaumont CME website? 2. Is the content your own or did you purchase/borrow it from another organization? If it was purchased/borrowed, we must have permission from the other organization on file in order to certify the content. Once Approved: Obtain disclosures from all involved with content (if this is a brand new activity) Submit a copy of PowerPoint slides or video, post-test questions, and additional resources to CME. Unless otherwise requested, CME will create the post-test and evaluation in our Qualtrics software. If Department/Joint Provider is hosting the content on your own site: CME will provide you with the Qualtrics URL to upload into your site Provide CME with the final URL for testing purposes; CME must test the site from start to finish If Beaumont CME is hosting the content on our site: CME will upload the Qualtrics URL to our site CME will test the URL from start to finish and also ask the department/joint provider to test Page 2 of 3
Each online activity is certified for 1-year from the release date initially. Before the 1 st year certification expires, CME will contact the department/joint provider to determine if you wish for the CME certification to be extended for an additional year. If yes, department/joint provider must confirm in writing to CME that the content is still accurate. The website must be updated to add a review date and update the expiration date in the CME information section. CME will charge the department/joint provider an additional certification fee must for the additional year of certification Price TBD (based on % of fee for initial year certification). If no, notify CME and remove the activity from the website. Page 3 of 3
