LAMBERT LACROSSE SCHOLARSHIP 2015 APPLICATION The Lambert Lacrosse Scholarship Fund was created in 2013 to assist deserving male and female students who have participated in Lambert Lacrosse programs with college expenses and to encourage their participation as adults in community youth lacrosse programs and/or as players at the college level. The number of awards and award amounts may vary annually depending on funding availability. This scholarship, which is renewable up to four consecutive years, is open to graduating seniors at Lambert High School (LHS) who have participated as a player on either the boys or girls lacrosse team for at least one season and who are planning to attend an accredited U.S. college or university as a full-time student in the fall of 2014. Applicants must also be planning to participate on a lacrosse club/team or to serve a local lacrosse program while seeking a degree. Additional eligibility requirements are as follows: Minimum 2.8 cumulative GPA Commitment to become a volunteer coach, board member or official in a community lacrosse program for two years when life circumstances permit Demonstrated financial need (may be considered but not primary consideration in selection process) Please fill out the information below completely and print clearly.
