1 2015 High School Graduate Scholarship Application For any questions please contact your Admissions Representative Scholarship overview Application due date: May 4, 2015 The Ogle High School Scholarship Program looks to award chosen full-time Cosmetology candidates that have assembled solid academic records and are leaders in their communities. Moreover, these exceptional graduates share a great passion in making the world around them beautiful. The program offers two scholarships: The Presidential Scholarship and Beautiful Future Scholarship. Presidential Scholarship Eligibility Criteria 1 HALF TUITION scholarship that will be awarded based upon exceptional academic achievement, exemplary community leadership and an exhibited passion for the beauty industry. Eligibility requirements are as follows: 3.0 weighted GPA 40 extracurricular activities and/or community service hours as approved by your high school Documentable experience in the cosmetology field An application for admissions must be submitted prior to the scholarship award consideration Student must remain in good standing while attending Ogle School Beautiful Future Scholarship Eligibility Criteria 1 $ scholarships that will be awarded based upon high academic achievement, strong community leadership and an exhibited passion for the beauty industry. Eligibility requirements are as follows: 2.7 weighted GPA An application for admissions must be submitted prior to the scholarship award consideration Students must remain in good standing while attending Ogle School 1 Student must begin their program within 45 days from the date of notification.

2 Scholarship Application Process To be considered for an Ogle Scholarship, a Scholarship Application must be completed and all required documentation must be submitted to the Admissions Department at the intended campus of attendance no later than May 4, Scholarship finalists may be required to interview on campus with a member(s) of the selection committee. Scholarship application packet must consist of the following: A completed Ogle Scholarship Program Application including community service profile (required for Presidential Scholarship) An official high school transcript (must be received directly from the high school or in an envelope with an unbroken seal) detailing completed courses and grades through first semester senior year Two letters of recommendation from a community leader, teacher, principal, counselor or other school official o Letters of recommendation should be submitted along with application packet in a separate sealed envelope o Letters of recommendation must be from non-related individuals Applicant Essay (2 part minimum 250 words each) o Why the beauty industry o Describe how you exhibit the following values while a high school student Teamwork Integrity Respect Excellence Service to the customer 2 Any application or other supporting scholarship documentation received after May 4, 2015 will not be presented to the selection committee and will not be used in the selection process.*
