All information included in the mailing will also be available at our Student Orientation Day. Please mark your calendars for the following important dates: Enrollment dates for students new to Perry Township and Perry Meridian Sixth Grade Academy are July 10 th, 11 th, 18 th, and 23 rd. Locker information will not be handed out until the first day of school, but banks of lockers will be available to practice opening using a combination lock. Enrollment dates for students new to Southport 6 th Grade Academy are July 19 th, 23 rd, 24 th, and 25 th. No enrollments will take place on the day of orientation. Back to School Registration/Orientation Information (Continued) Perry Meridian Middle School 789-4100 Monday, July 15 th : Guidance Office opens for enrollments. Tuesday, July 23 th Orientation There will be two orientation sessions for all new and returning seventh and eighth grade students. It is very important that all students attend one of these sessions. All students will receive their class schedules, fee statements, and locker assignments by mail in late July. By attending one of these orientations, parents and students may pay fees, open their lockers, speak to counselors and be able to walk through their schedules. The orientations will be held on Tuesday, July 23 th from 1:00-3:30 p.m. and 4:30-7:00 p.m. Tuesday, August 13 th : Back to School Night 7:00 p.m. to 8:30 p.m. Southport Middle School 789-4600 On Tuesday, July 23, 2013, Southport Middle School will open its doors to all seventh grade students and new eighth grade students and their parents. Three special orientation programs will be held for these families who are new to SMS. Students whose last names begin with A-L should attend at 9:30 a.m. and students whose last names begin with the letter M-Z should attend at 1:30 p.m. An additional program will be held that evening at 6:30 p.m. for students and parents who were unable to attend earlier in the day. After a formal program in the auditorium, students will be able to familiarize themselves with the location of their classrooms and lockers. Returning eighth grade students may also visit their classrooms and lockers that day, but starting one half hour after the above times. Fee statements, class schedules and locker assignments will be mailed in the middle of July. Physical Education clothing will be available for purchase, $15.00 for all sizes except for 2X and 3X, which will cost $17.00. Parents will have the opportunity to pay for textbook rental fees. Students who are new to the Southport Middle School attendance area should contact the school office at 789-4600. New enrollees should plan to bring their last report card, immunization record and last standardized test results. Also, a copy of their birth certificate and the student s social security number will be needed. Back to School Night Thursday, August 1 at 6:30 p.m. Perry Meridian High School 789-4400 New Enrollments Counselors will be available to enroll new students from July 23-26. Please call 789-4430 to schedule an appointment. Freshman and New Student Orientation Thursday, July 25, 2013, 5:30 p.m. 7:30 p.m. Stations will be set up in the foyer and in front of the IMC for the following: The Treasurer will be accepting textbook rental payments. A computer will be set up for any parent wishing to pay with credit card online. The bookstore will have a station selling gym clothes. The students will be able to pickup schedules at the guidance station. Other stations will be available to purchase spirit clothes, athletic passes, etc. Also, there will be Parent/Student organization stations available. Students will be able to tour the building. Various clubs will be present. Uniform Dress Guidelines Elementary: 1. Solid color collared shirt (with sleeves) or turtleneck, tucked in. 2. Khaki or navy bottoms, no denim. 3. Knee-length pants (shorts or Capri's). Shorts may be worn up until Fall Break and after Spring Break. 4. Dresses, skirts and jumpers must be near knee length 5. Winter Wear solid color sweater or sweatshirt allowed; must be worn over collared shirt or turtleneck. 6. No flip-flops, no wheelies. 7. Spirit wear determined by each building. 6th Grade Academy and Middle Schools (Grades 7 & 8): 1. Solid color collared shirt (with sleeves) or turtleneck, tucked in. 2. Any items with a hood will not be permitted. 9. Hats, head coverings, sunglasses, curlers, or slippers may NOT be worn at any time during the school day. 10. Clothing, jewelry with double meanings, sexual references, satanic symbols, or advertising for alcohol, drugs, and tobacco are NOT permitted.

Medication Policy When a student requires medication at school or during a school sponsored event, the following must be in place before medications can be administered to the student: Prescription Medications: Must have the M.D. prescription on the container Parent/guardian must sign release for school to administer Original container (pharmacy will provide a container for school) OTC Medications Must be in original container Must have dosing information on package Must be age appropriate (i.e., not adult Tylenol for a child) Parent/guardian must sign release for school to administer To carry medications at school, the student must have: Written documentation from M.D. of the medical condition and validation that student understands medicine use and misuse Authorization note from parent/guardian Medications can travel with student to/from school with parental consent. Parent Center On-Line Have you used the Parent Center online? On the district s website, www.msdpt.k12.in.us, click the Parent Center icon. Parents can pay their child s textbook rental fees and pay for school lunches, view their child s marking period grades, view the electronic teacher grade book, attendance and discipline information. To access the Parent Center, go to the district s website, www.msdpt.k12.in.us and enter your login information. An ID number and personal identification number (pin) are needed to gain access to the Parent Center. Your pin can be found on the textbook rental invoice that will be mailed to parents. K-5 Students A statement listing all textbook rental fees and supply fees is scheduled for mailing to each student s home on or about July 20, 2013. The textbook rental and supply fee for the 2013-2014 school year is $95.00 for kindergarten and $157.00 for first grade through fifth grade. If payment is made by check, the check must be payable to the student's school (with your child s name in the lower left corner of the check.) You may also pay fees online using a credit card. See page 11 of this bulletin for quick and easy instructions to pay by credit card. If you have not received your statement or need further information, please call your child's school. Gr. 6-12 Students MSD Perry Township high school and middle school fees are assessed on a per course basis. An invoice statement, listing all school course fees, is scheduled for mailing to each student s home on or about July 20, 2013. This statement will itemize textbook rental charges along with workbooks, consumable workbooks, supplemental charges and supply fees ending with a grand total payable to the school your student attends. The Metropolitan School District of Perry Township will once again be accepting payments for textbook rental and student lunches for the 2013-2014 school year, via the school district s online payment system. Online payments may be paid by credit card: Visa, Mastercard, and/or Discover Card are the credit cards that will be accepted. See page 11 for quick and easy instructions to pay by credit card. In order to access the online payment system, one must have access to the Internet. If payees do not have access to the Internet from home, work, or elsewhere, computers will be available for use during the designated orientation or registration times before the start of school. The online payment system will be available for use between the hours of 5:00 a.m. and 11:30 p.m. daily. Once connected to the school district website, one must simply enter login information at the Parent Login area. The minimum amount accepted for lunch payments will be $25.00. Students will be able to use money posted to the cafeteria account the day after an online payment is made. MSDPT Free & Reduced Meal Application You may now fill out the MSDPT Free and Reduced Meal Application online. For your convenience, the Free or Reduced Lunch Application for the 2013-2014 school year may be completed on-line from the link below. Step by step instructions make filling out the Free and Reduced Meal Application online a simple and accurate process. You may apply online anytime after July 1. Click here for the online application. You can also locate the Free and Reduced Meal application by going to www.msdpt.k12.in.us. Click on the Parents and Community Tab and then click on Food Service. Click on the FREE AND REDUCED MEAL APPLICATION icon. Accept the end user language agreement, and then click Start to proceed to the application. When the application is successfully completed and submitted it will be processed within 5 days. Click on Food Service and then click on printable Free and Reduced Application on the left side of the page.
