BSN Program News Summer, 2014 Edition Required Health Records DUE DATE APPROACHING To be submitted as a single package by July 15, 2014 Health Form All students admitted to the BSN Program must have a current complete Health Form on file by July 15, 2014 of the sophomore year. RN to BSN students must also have these forms on file by July 15. Students without the current complete Health Form will not be permitted to begin clinical (BSN students) or engage in any activities involving agencies other than the School of Nursing (RN to BSN students). (See below for further information on consequences of not meeting deadlines). Be sure to use the SON health form which can be found on SON website or in the main nursing office. The health care provider must complete a physical examination for the student and certify the student s emotional and physical fitness for carrying out nursing responsibilities. Any deviations and treatments must be noted. Immunization records are also required. Signatures are required in several places, so the student should make sure form is complete before leaving the health care provider s office. The School of Nursing abides by the requirements of the clinical facilities. CPR Verification* By July 15th, starting in the sophomore year of the BSN program, or upon admission to the RN to BSN program, students must obtain and maintain a current CPR card (Adult, Infant, and Child). Failure to have a current CPR card on file in the nursing office will result in the student being declared ineligible to begin clinical (BSN students) or engage in any activities involving agencies other than the School of Nursing (RN to BSN students). A photocopy of the actual card is required, front and back and signed by student and CPR instructor. (Do not send original card, it will not be returned!) Only the following courses are acceptable: 1. The American Heart Association Basic Cardiac Life Support for Healthcare Providers. 2. The American Safety Health Institute CPR Pro for the Professional Rescues. 3. The American Red Cross course with both the Adult CPR and AED course AND the Infant and Children CPR course. 4. The American Heart Association Heartsaver First Aid, module C (adult and child CPR with mask), module D (adult and child AED) and module E (infant CPR with mask). 5. IOHSA (International Occupational Health and Safety Association), BLS for Healthcare Provider (adult, child, and infant), and AED course. It is the student s responsibility to make sure an active CPR card is on file in the nursing office at all times. If the student attends clinical (BSN students) or engages in activities involving agencies other than the School of Nursing (RN to BSN students) without an active CPR card all clinicals or activities engaged in during this time will be given an unsatisfactory grade. ANNUAL TB TEST At the current time there is a Nation-wide Shortage of TB Tests, and many health departments and clinics are no longer giving tests unless an individual is at risk or symptomatic. If student is unable to get TB test done students should have their health care provider complete the attached TB Screening Questionnaire adapted from one used by Cabell Huntington Hospital. If the student is deemed at risk they will be sent to the health department or their primary care provider for follow-up. New Sophomore students and ne admit RN to BSN will need to provide us with documentation of last TB test they had done. When the TB Test shortage is resolved students will be informed and required to get their annual TB Test and provide results to SON. *Please note, students who have not turned in the appropriate documentation by July 15 will be declared ineligible to participate in clinical and will start the semester with an unsatisfactory in all clinical courses. Any clinicals missed due to lack of documentation will also be counted unsatisfactory. Two unsatisfactory grades in clinical will result in a failure of the course. Include legal name and 901 number on ALL Documents to be turned into the SON. 1
