Hospital lists As an Allianz Worldwide Care member, you have access to leading private hospitals throughout the UK, Channel Islands and Isle of Man. Included on our hospital lists are a comprehensive range of private hospitals and NHS hospitals, with private patient facilities. Our range of hospitals are selected to offer our members excellent geographical coverage, a full range of treatments and high standards of quality of care. As a member of a company scheme, your hospital list will have been selected by your employer. The list chosen will be indicated in your Table of Benefits, under the Policy endorsements section. Under your plan, you are eligible for in-patient and day-care treatment in any of the hospitals in your list. There are no restrictions on where you can have out-patient treatment. There are three hospital list options: offers a great range of private hospitals and NHS private facilities throughout the UK. in addition to all of the hospitals in the list, it includes a broader range nationally, with a wider range of London based hospitals. covers all hospitals and facilities in the UK. When the list is chosen, members have peace of mind in knowing that if they choose to attend any in-patient or day-care facility in the country, then all eligible treatments will be covered in full. Treatment outside of network Please note, except where the hospital list has been chosen, you are only eligible for treatment in one of the hospitals on the list chosen by your employer or at any NHS hospital. On the rare occasion that the treatment you require is not available at one of the hospitals on your list, please contact our Helpline (LoCall 0203 5642 546) and our Medical Services team will assist you in arranging treatment at an alternative hospital. Please be aware that pre-authorisation of treatment at a hospital which is not on your list must be sought in advance, or your claim may not be paid by Allianz Worldwide Care when it is submitted for payment. treatment across all the hospitals in our network, and the average hospital cost calculated will be the maximum amount we will pay towards your treatment. This could leave you with a shortfall that you will be required to pay. If the actual cost of the treatment is less than the average calculated cost, we will pay the hospital costs in full. We also cover specialist fees up to the limits in our fee schedule. Treatment in an NHS hospital Unless stated otherwise, all NHS hospitals are considered to be included in all hospital lists (, and ). You may be treated as a private patient in a private patient unit or in a pay bed. However, it is important to note that, in some cases, pay beds may not differ from standard NHS accommodation and their availability is at the discretion of the hospital. Alternatively you may choose to be treated as an NHS patient. If you choo se to have your treatment as a public patient in an NHS hospital, you may be eligible for an in-patient cash benefit for each night you are in hospital subject to the maximum number of nights stated in your Table of Benefits. Psychiatric hospitals Dedicated psychiatric hospitals have been clearly marked in the tables included in this document. These hospitals only treat conditions related to mental health; other types of medical treatments are not provided. Changes to the hospital list Allianz Worldwide Care constantly monitors and reviews the services provided and treatment costs of hospitals on our lists. Therefore, from time to time, we may make additions or deletions to our hospital lists. The most up to date list can be found on our website at: www.allianzworldwidecare.com/ukmembers Please ensure that the proposed hospital is on the list, prior to booking your treatment. If you receive treatment on an in-patient or day-care basis in a hospital that is not included in your hospital list, we may decide to calculate the average hospital cost of equivalent 2
Pre-authorisation Why is Pre-authorisation required? All in-patient and day-care treatments require pre-authorisation, so please contact us prior to arranging treatment. If you have to go to a hospital, in the vast majority of instances we will settle the bill directly with the medical provider, subject to any co-payments, deductibles and benefit limits. As with all health insurance policies, your plan with us will only cover treatment that is medically necessary and charges that are usual and customary. Therefore, it is vital that you contact us prior to treatment so that we can confirm medical necessity and appropriateness of costs. In addition, Pre-authorisation will help us to provide you with a better service in the following ways: To arrange for direct settlement, we can assist you more quickly and efficiently if the following steps are taken: For planned treatment: 1. Please download a Pre-authorisation Form from our website: www.allianzworldwidecare.com/ukmembers You and your physician will need to complete the relevant sections of the Pre-authorisation Form. 2. Once fully completed, please send the Pre-authorisation Form to us at least five working days prior to treatment so that we can ensure there will be no delays at the time of admission. You can submit it via: Scan and email to: medical.services@allianzworldwidecare.com Fax to: + 353 1 653 1780 or Post to the address shown on the Pre-authorisation Form If treatment is due to take place within 72 hours, our Helpline (LoCall 0203 5642 546) can take Pre-authorisation Form details over the telephone if you have the required information to hand. In the case of planned treatment, we will have time to communicate with the hospital to facilitate smooth admission and arrange for direct settlement, offering you cashless access to hospitals for in-patient and day-care treatment. Your treatment can be overseen by our Medical Team. What happens if Pre-authorisation is not obtained? If pre-authorisation is not obtained and the treatment received is subsequently proven to be medically unnecessary, we reserve the right to decline your claim. We are also unable to guarantee direct settlement of your medical bills if pre-authorisation has not been sought. Out-patient Network For more information in relation to our Physiotherapy, Dental and Health Assessments direct settlement networks, please refer to your Signature Employee Benefit Guide or our website: www.allianzworldwidecare.com/ukmembers What is Pre-authorisation? Pre-authorisation is required in advance of certain treatments. Following approval by Allianz Worldwide Care, cover for these required treatments and the costs associated can then be guaranteed.
