3 A long history of providing access to healthcare The Universities & Colleges Personal Healthcare Scheme has been providing private medical insurance to the higher and further education sector since The scheme was devised and is managed by Jardine Lloyd Thompson Benefit Solutions, leading advisers on Healthcare Solutions in the Tertiary Education sector. Underwritten by AXA PPP healthcare one of the UK s leading medical insurance providers. The scheme is currently one of the largest and most popular voluntary medical schemes in the UK, covering around 5,000 individuals. It is specially designed and costed for the benefit of university and college employees and their immediate families. AXA PPP healthcare gives you peace of mind The Universities & Colleges Personal Healthcare Scheme offers you the choice of when and where you receive treatment which is particularly important, considering the busy life that you lead. All employees of Universities and Colleges and their immediate families are eligible to join the scheme, including children up to 25 years old if they are in full time education. No medical examination is required. Benefits for you and your family The scheme offers child rates including year old dependants, provided they are in full time education. Child dependants aged who are not in full time education can continue in the group scheme at an adult rate. You can even stay in the scheme and continue your policy when you retire. The cover you d expect from one of the leading providers Providing the level of cover that s right for you We provide a range of different plans to suit your individual needs: AXA PPP healthcare Premier, Ideal, Key and Assure. Each plan offers you cover for private treatment. This includes in-patient and day-patient treatment, associated specialists charges, out-patient surgical procedures, radiotherapy and chemotherapy. In addition, each plan excluding the Assure plan provides you with out-patient cover for diagnostic tests, physiotherapy and other therapies (subject to the annual limits on each plan). All our plans also offer you out-patient CT, MRI and PET scans. The underwriting methods available As detailed in the Main exclusions and limitations section of this policy summary your cover is restricted for treatment of medical conditions you had prior to joining. How this applies to your cover will depend on which basis you choose to join the policy. You can join any of the policies either on moratorium or a fully underwritten basis. Moratorium Moratorium means we ll cover you for any new conditions that arise after joining. Plus, after the first two years, you may also be covered for any pre-existing conditions you had in the five years prior to joining. Fully underwritten we ask for medical history declarations on your application form. All pre-existing conditions are excluded for benefit subject to the rules and benefits of the plan. Medical history disregarded in some circumstances you can join Premier or AXA PPP healthcare Key on a medical history disregarded basis, provided your application is received within six weeks of joining your current employer, or within six weeks of the scheme being launched/re-launched by your employer. 4 As an in-patient, you are covered for all your essentials during your hospital stay: accommodation, meals, nursing care, drugs and dressings. You will usually have your own private room, with an en-suite bathroom, television and telephone. We will also pay your specialists fees in full, as long as they are in line with the level customarily charged for the treatment you are given. covered on this policy, has any on-going claims or treatment, and there is no treatment planned or booked within three months of the start date of the policy. Excesses A mandatory excess applies to this policy per person per annum. This means you pay the first 100 of eligible claims once each policy year, for each person covered by the policy. Higher excesses ( 200 and 500) are available. Full details of how the excess is applied can be found in the Additional information section of the membership handbook. The 6 Week Option An alternative way of saving on your premium is by choosing a 6 Week Option plan. This option can cut the cost of private medical insurance by up to 30%. All of our plans offer you the 6 Week Option whereby you agree to be treated by the NHS for out-patient surgical procedures, in-patient or day-patient treatment, if the waiting list is six weeks or less from the date treatment should take place. However there are certain common surgical procedures that are covered immediately for private treatment regardless of the NHS waiting list. In-patient and day-patient treatment explained In-patient treatment is treatment for a patient who is admitted to hospital and who occupies a bed overnight or longer, for medical reasons. We normally settle all eligible bills for this type of care directly with the hospital or specialist, so you will not be out of pocket. Day-patient treatment is treatment for a patient who is admitted to a hospital or day-patient unit because they need a period of medically supervised recovery but does not occupy a bed overnight. Out-patient treatment explained All our plans include cover for out-patient surgical procedures, radiotherapy, chemotherapy, magnetic resonance imaging (MRI), computerised tomography (CT) and positron emission tomography (PET) scans (subject to the annual limits on each plan). All of our plans excluding Assure also cover out-patient consultations, diagnostic tests and treatment by a clinical practitioner, including physiotherapy. Complementary therapies acupuncture, homeopathy, osteopathy and chiropractic treatments by recognised complementary practitioners are also covered on most of our plans when you are referred by your GP (subject to the annual limits on each plan). It is important to note that this outline and the plan information on pages 4 5 is only intended to provide you with a summary of the benefits offered by AXA PPP healthcare s range of policies. It should help you focus your thoughts on the most appropriate type of private medical insurance (PMI) cover for you. All PMI providers will give you this information in the same format. The Assure policy offers you cover as a UK resident, for private medical treatment in the UK, of new medical conditions that arise after you join. This includes in-patient and day-patient treatment, associated specialists charges, radiotherapy and chemotherapy. In addition, it provides you with cover for out-patient surgical procedures, computerised tomography (CT), magnetic resonance imaging (MRI) and positron emission tomography (PET). What is Key? The AXA PPP healthcare Key policy offers you the same cover as Assure. In addition it provides you with cover for up to 400 of benefits for out-patient diagnostic tests, consultations and complementary practitioner and clinical practitioner charges (including Physiotherapy). What is Ideal? The Ideal policy offers you the same cover as AXA PPP healthcare Key. In addition you will have cover for in-patient and out-patient treatment of psychiatric illness. It also provides you with cover for up to 900 of benefits for out-patient diagnostic tests, consultations and complementary practitioner and clinical practitioner charges (including physiotherapy) for each person covered on the policy. What is Premier? The Premier policy offers you the same cover as Ideal. In addition it provides you with unlimited cover for out-patient diagnostic tests, consultations and complementary practitioner and clinical practitioner charges (including physiotherapy) for each person covered on the policy. What are the main exclusions and limitations of AXA PPP healthcare Premier, Ideal, Key and Assure plans? As with all insurance policies, general exclusions apply. The following is a summary of the main exclusions and limitations of the policies. What are the main exclusions and limitations of cover? Ongoing, recurrent or long-term treatment of long-term illnesses (usually referred to as chronic conditions ). General dental procedures. Routine pregnancy and childbirth. In-patient or day-patient treatment or MRI, CT or PET scans not received in a hospital or scanning centre listed in the Directory of Hospitals. We pay most fees in full, but we only pay charges up to the average level if the person treating has in the past charged fees outside the range that is usual for the treatment they provide. Mandatory excess of 100. Treatment of medical conditions that existed, or you had symptoms of, before joining (unless on a MHD basis). Treatment of injuries sustained from playing professional sport. 7 How long will my cover last? Your policy will be arranged for 12 months from the effective date on your membership statement. What do I do if I want to make a claim? We aim to provide you with the highest possible standards of service but there may be occasions when you feel that things have gone wrong for you and you are unhappy with us. If you have a complaint about any matter please contact us and we will do our best to address your concerns. Your feedback is vital to helping us improve. If you are dissatisfied with the outcome of our investigation, you can ask the Financial Ombudsman Service to consider your complaint. If you have a claim against AXA PPP healthcare In the unlikely event that AXA PPP healthcare becomes insolvent and is unable to pay the benefits under your policy, you are protected by the Financial Services Compensation Scheme (the FSCS). The first 2,000 of any claim is protected in full. For amounts above this the FSCS will ensure that policyholders are compensated to 90 per cent of the value that their policy would have paid. Further information about the operation of the scheme is available on the FSCS website: What if I change my mind? You have a 14 day cancellation period. During this period you have the right to cancel your policy and have your premium returned in line with the terms for cancellation at renewal, as detailed in the Legal rights and responsibilities part of the Complaint and regulatory information section of your membership handbook. The cancellation period begins on the day your contract is agreed or on the day you received your full policy terms, if this is later, and will also apply from each renewal date. Staffed by registered nurses, midwives, pharmacists and counsellors, we re there to answer any health related queries you may have. Lines are open 24 hours a day, 365 days a year. The service covers everything from how to alleviate common ailments to how to contact local support groups. On request, Health at Hand can also post, fax or Health Fact Sheets on over 150 ailments and treatments. They re written in plain English and we call you to make sure they have arrived and to discuss the information with you. Health at Hand does not take the place of your GP, nor does it diagnose or prescribe. However, we know that sometimes it s just reassuring to talk to someone other than your GP in complete confidence. Help when you re abroad Immediate emergency in-patient treatment abroad The plan you choose with the Universities & Colleges Personal Healthcare Scheme will provide you with limited medical treatment when travelling abroad of up to 40,000. This cover is only available for emergency in-patient hospital treatment in an overseas hospital. This benefit is available providing you haven t travelled abroad against medical advice, or gone abroad specifically for treatment. Under normal circumstances there is no cover on the policy for treatment received outside the United Kingdom. This cover should not be relied on in place of adequate travel insurance which we strongly recommend you take out whenever you travel abroad. Visit for details of such an arrangement. Evacuation or repatriation service If you can t get suitable or adequate emergency in-patient treatment, we ll arrange for you to be transported to the nearest suitable medical facility or even bring you home for treatment, if necessary. 9 Our guarantee and what to do next Our guarantee Perhaps two of the greatest advantages of taking out an AXA PPP healthcare policy are the confidence and sense of security it can provide. We recognise our members are our business. That means your complete satisfaction is very important to us. We offer you this guarantee: If your application is accepted and you receive your membership documents, you will have 14 days in which to ensure you are completely satisfied with your cover. If you have not made a claim, you can cancel for any reason during this period and owe nothing. In the unlikely event of you wishing to cancel during those 14 days, any money that you have paid or we have collected will be returned, provided you have not claimed. Full details of our complaints procedure will be provided with your membership handbook and are available on request. All our policies are governed by the law of England and Wales and this is the law that is used in the case of any disputes. Your Direct Debit guarantee All banks and building societies that take part in the Direct Debit Scheme offer this guarantee. The efficiency and security of the scheme is monitored and protected by your own bank or building society. If the amounts to be paid or the payment dates change, we will notify you 10 working days in advance of your account being debited, or as otherwise agreed. If an error is made by us, or your bank, or building society, you are guaranteed a full and immediate refund from your branch of the amount paid. You can cancel a Direct Debit at any time by writing to your bank or building society. Please also send a copy of your letter to us. 12 We want to make it as easy as possible to understand everything about your policy Glossary Medicine seems to have a language all of its own. Here are some terms that we use in this brochure and in your policy with which you may not be familiar: ABI Association of British Insurers the trade association for the UK insurance industry. We ve enclosed their independent guide to buying private medical insurance. CT scans Computerised tomography scans. A form of x-ray examination. It is particularly useful for making diagnoses in the head, chest and abdomen. Day-patient A patient who is admitted to a hospital or day-patient unit because they need a period of medically supervised recovery but does not occupy a bed overnight. Diagnostic tests Investigations such as x-rays or blood tests, to find or help to find the cause of your symptoms. Directory of Hospitals A document we publish which lists private hospitals, day-patient units and scanning centres in the UK that are covered by us. Family member The policyholder s current spouse or civil partner or any person (whether or not of the same sex) living permanently in a similar relationship with the policyholder and any of their or the policyholder s unmarried children. Fully underwritten We ask for medical history declarations on your application form. All pre-existing conditions are excluded for benefit subject to the rules and benefits of the plan. In-patient A patient who is admitted to hospital and who occupies a bed overnight or longer, for medical reasons. Medical condition Any disease, illness or injury, including psychiatric illness. Medical history disregarded basis We do not ask for medical history declarations on your application form. All pre-existing conditions are accepted for benefit subject to the rules and benefits of the plan. Because of the extra risk the subcriptions are higher than underwritten groups. Medical underwriting The process by which we assess your medical history and decide on what terms we may offer to cover you. Moratorium We ll cover you for any new conditions that arise after joining. Plus, after the first two years, you may also be covered for any pre-existing conditions you had in the five years prior to joining. MRI scans Magnetic resonance imaging scans. A way of medically mapping the body s tissues. It helps with non-invasive diagnosis and treatment planning of a wide range of diseases, including cancer. It doesn t use potentially harmful ionising radiation such as x-rays. Out-patient A patient who attends a hospital, consulting room, or out-patient clinic and is not admitted as a day-patient or an in-patient. PET scan Positron emission tomography scan. A way of diagnosing and monitoring many serious medical conditions, including heart problems and cancer. A very small radioactive injection is given to the patient and a PET camera is used to measure radioactivity in various parts of the body. Physiotherapy Physical treatment such as massage or exercises. Pre-existing conditions A medical condition, and conditions relating to it, which existed before your cover with us started. Radiotherapy/chemotherapy Treatments for cancer. Specialist A senior and experienced medical practitioner who has attained the status of an NHS consultant or equivalent.
