IMPORTANT NOTICE -- ACCESS LIVING S HOUSING SERVICES Access Living tries very hard to keep this housing list updated. Since Access Living is not a residential housing center, we do not take on the responsibility of finding you a place to live but we work to offer you the tools that will assist you find adequate housing. In your housing search, we can assist you in the following ways: Providing you with an updated list of subsidized units as well as market rate units Schedule an appointment with you at your convenience, to assist you on a one-on-one basis with your housing search Allow you the use of Access Living s telephones, video phones, fax machines and computers to assist you in your search Assist you with making telephone calls to housing providers if due to your disability, you are unable to do so Provide you with public transportation fare to and from Access Living, as needed Assist you with free legal advice and possible representation if you believe you have been discriminated against because of your disability in your search for housing We also conduct monthly housing support group meetings where we invite speakers that provide affordable housing programs to explain the process that is required to apply for housing. If you want to attend the housing support meetings contact Access Living at (312) 640-2121 voice or (312) 640-2102 (TTY) and give us your name and address, we will send you a flyer. If you have access to a computer, you can also check our website www.accessliving.org for a posting of upcoming housing support meetings and other independent living services. Access Living also has a housing advisory board that meets once every 4-6 weeks to discuss advocacy strategies to increase the availability of affordable, accessible housing in Chicago. If you are interested in participating on that committee, please call us. Most of the housing providers listed here have a waiting list and some of these lists might already be closed. We advise that you be persistent and call often to find out if a waiting list is open and place your name on all open lists. Some providers know about Access Living s efforts to assist individuals with disabilities find adequate housing and send us notices about when they will open their waiting lists. Some providers also send us rental s for us to give out. During the summer months we receive an average of 500 calls per month from people that are seeking subsidized or affordable housing. Given our limited resources it is at times hard for us to answer all phone calls within 48 hours so we ask that you be patient, but persistent and call us back. 3

Scattered Site Supportive Housing for Families ELIGIBILITY: Families with at least one child under 18 Must be currently homeless, which is defined as: Living in an emergency shelter Living in a place not meant for human habitation, such as a car, park, or abandoned building Living in transitional housing, and was previously in shelter or place not meant for human habitation Fleeing, or attempting to flee domestic violence, dating violence, sexual assault, stalking, or other dangerous or life-threatening conditions related to violence A member of the household has a documented disabling condition, such as a physical disability, mental illness, substance abuse disorder, or developmental disability. SERVICES: The program will provide an apartment with a permanent subsidy. Residents must pay a portion of their toward rent. Residents will receive supportive services to help them maintain housing, increase their skills and, and become more self-sufficient. Services may include case management, referrals for health services, therapy for adults and/or children, tenant education, and financial literacy. Families also have opportunity to participate in community-building activities for parents and children. Services are available in English and Spanish.
