Associate Degree in Nursing (AAS.ADN) Licensed Practical Nurse to Registered Nurse Completion Student Checklist Complete the following required steps by February 1 of the year for which you are applying. Complete an Intent to Enroll for Lake Land College Select AAS.ADN.TRK as the academic program. This will ensure you are assigned a nursing academic advisor. It will also generate a Lake Land email account; prospective students are expected to check this email account on a regular basis for nursing department correspondence. Complete a Nursing Department Intent Form indicating interest in the Associate Degree in Nursing Program. These are available online, in the nursing department, or in Counseling Services. Receipt of this form will prompt the nursing department to start a file and correspondence will begin. The department will review student information to determine eligibility to take the take the HESI A2 Admission Assessment Exam. To be eligible, the applicant needs satisfactory scores in 2 of 3 categories on one of the following: (College Placement and ACT scores are valid for 5 yrs) o Compass: English 81, Reading 83, and Algebra 39. This is the Lake Land College Placement test. To schedule, call the Tutoring and Testing Center at (217) 234-5301. o ACT: Reading 21, English 19, and Math 19. o OR Completion of at least 2 of the following 3 courses (or their equivalents): RDG 050 (or completion of 30 college credits in good standing), ENG 007 or higher, and MAT 005 or higher. Within 2 weeks of receipt of completed intent form, applicants will be notified (via Lake Land email) regarding eligibility to take the HESI A2 Admission Assessment Exam. To be considered for the Associate Degree Nursing Program the applicant must obtain a minimum acceptable score of 75 on this exam. NOTE: this exam was changed in 2016 students taking the previous exam are required to attain a score of 90. Upon receipt of this email, it is the students responsibility to schedule and pay for the exam with the Tutoring and Testing Center in Webb Hall at (217) 234-5301. The cost is $50, payable to Lake Land College, by cashier s check or money order only. Payment can be delivered in person or by mail to Lake Land College Tutoring and Testing Center. Payment must be received at least two weeks before the test allow ample time if utilizing the US mail to send payment. This exam is a three hour, proctored, online test offered the first Friday of September, October, November, December, and January. Students will be tested in reading comprehension, vocabulary general knowledge, and math. A study guide is available to assist in preparation for the exam: HESI Admission Assessment Exam Review, 3rd ed. (2012). ISBN: 978-4557-0333-3 Submit a copy of your current LPN license. You will not be considered unless a copy of your license is on file. Submit a letter from your current employer (on agency letterhead) indicating that you are currently working and your role in direct patient care.

Request an official waiver for ADN 040, ADN 042, BIO 100 and HED 102. Associate Degree in Nursing (LPN to ADN) Complete the following pre-requisite prior to the February 1 st deadline, with a grade of C or better: Chemistry either one full year in high school or CHM 101, CHM 111, CHM 120, or CHM 150, or equivalent. Chemistry must be completed within the last 5 years. Students enrolled in Chemistry during the spring semester of the application deadline will not be considered for admission. Send college transcripts to Lake Land College Admissions & Records Office. Contact the office at (217) 234-5434 to confirm receipt. Then request a Transcript Evaluation for AAS.ADN.TRK from the Admissions & Records website www.lakeland.cc.il.us/admissions to ensure courses that may qualify you for a waiver are evaluated. Complete co-requisite courses required of first year ADN students with a grade of C or better prior to admission. On February 1 st, a review of files will begin to determine students for Fall admissions. Admission scores are calculated utilizing the following rubric approved by the nursing department. To be considered for admissions, a current LPN license and verification of employment must be on file in the nursing office by February 1 st. ADN Admissions Rubric for LPN to ADN Students Intent Form-Associate Degree in Nursing 2.5 points for each 6 month period. (Jan 1 June 30 & July 1 Dec. 31) 0 to 10 points HESI A2 Admission Assessment Exam (minimum score of 75 required to be considered for admission). Verification of pre-requisite course completion. Either one full year high school chemistry or CHM 101, 111, 120, 150 or higher. Must have been completed with C or above and within the last 5 years. Aggregate GPA for completed co-requisite courses Co-Requisites points Points earned based on grades for courses completed. Verification of current employment as an LPN. Letter from employer required. TOTAL Minimum of 75 points Chemistry (within 5 years) 2 to 4 points 4 pts=a; 3 pts=b; 2 pts=c D or F not accepted. 0 to 44 points Essential Qualifications/Functional Abilities: Students applying to the Nursing Programs at Lake Land College must be able to meet the technical requirements of the program and not pose a risk to the well-being of patients, other students, staff or themselves. Students applying to the program will need, at a minimum, to be capable of performing all Functional Abilities required of the nurse. The student will need to maintain and demonstrate these abilities through the duration of the program.

Study! Prepare well for classes to earn the best grade possible, which increases the admissions score. Prepare for the HESI A2 Admission Assessment Exam because this is the best way to increase the number of admission points. Is there a waiting list? No The ADN program is a selective program with an objective, competitive admissions process. Each year, applicants are accepted based on a point system. How often are students admitted? Students begin the program in the fall semester only. Are there weekend or evening nursing classes? No currently most coursework is completed between the hours of 6 am and 4 pm, Monday through Friday. Can I apply if I am out of the Lake Land College district? The Illinois Community College Board requires that all in-district applicants be considered first. Out-of-district students will be considered using the same admissions rubric only after all eligible in-district students have been considered. How long is my HESI A2 Admission Assessment Exam score valid? The HESI A2 Admission Assessment Exam scores are valid for five years from the application deadline. If you take the exam more than once, the most recent score is always the score used for admission.

Do I need to repeat my science classes if they have expired? Yes. All sciences must be repeated if they have expired. No alternative exists for Microbiology or Chemistry, if expired, although you may take a higher level Chemistry course, if desired. If you have completed both Anatomy and Physiology I and II (BIO 225 and 226) with grades C or better, an alternative to repeating the expired class or classes is to enroll in ADN 061 Health Assessment (2 semester hours). Where is the ADN program offered? The ADN program is offered on the main campus and at the Effingham Kluthe Center. Accepted students can register their preferred site on a first come, first serve basis. How often will I be in school? Students can expect to attend class 3-4 days a week. The day off varies depending on the clinical assignments. Most clinical assignments require students to BEGIN as early as 6 a.m. Where are clinicals located? Students will have the opportunity to do clinicals in more than one local hospital/clinic/nursing home. So, students have multiple experiences upon graduation.
