Indiana Avenue 99216 Home Institution ACICS ID Code _00016074 --------------------------------------------------------------------------------------------------------------------- Host Institution ITT Technical Institute, Indianapolis, IN, Online Division Host Institution Address 9511 Angola Court, Indianapolis, IN 46268 Host Institution Accrediting Body Accrediting Council for Independent Colleges and Schools Host Institution ACICS ID Code 00016040 This consortium agreement enables enrolled students in an eligible program of study at ITT Technical Institute, Spokane Valley, WA or any of its branch campuses (Attachment A), an ACICS-accredited institution of higher education (the Home Institution ), to enroll in online courses delivered by the ITT Technical Institute, Indianapolis, IN, Online Division (the Host Institution ), that apply toward the academic requirements of a certificate, diploma and/or degree program at the student s Home Institution. The Host Institution must hold institutional accreditation from an accreditation agency recognized by the U.S. Department of Education; any portions of the program to be offered by the Host Institution must have been approved by the Host Institution s accreditation agency for online delivery. This consortium agreement allow students pursuing certificate, diploma and/or degree programs at the Home Institution to register for online courses delivered by the Host Institutions, subject to any applicable limits established by the Home Institution s state licensure and accreditation. These online courses must: (a) meet the learning outcomes established for the equivalent resident course at the Home Institution; (b) allow students to earn academic credit that is awarded by the Home Institution; and (c) not equal or exceed 50% of the educational program of study offered by the Home Institution. All online courses delivered by the Host Institution have been developed in accordance with the following standards: 1 of 7

Appendix H: Principles and Guidelines for Nontraditional Education as established by the Accrediting Council for Independent Colleges and Schools (ACICS). Distance Education Requirements as established by the state in which the Home school is located. All rules and regulations governing distance education and student financial assistance programs authorized under Title IV of the Higher Education Act of 1965, as amended. Coursework will be delivered in an online format via distance education to students enrolled at the Home Institution and supported by services offered by both the Home Institution and Host Institution. The Host Institution will maintain control of the learning management system consistent with the requirements and expectations outlined in this agreement. The Home Institution retains ultimate responsibility for the delivery of its educational programs and the satisfaction of its students. Monitoring of Curriculum and Instruction The Home Institution must have access to reports, generated by the Host Institution, that assist with monitoring student attendance and academic performance. The Home Institution is responsible for accessing the reports, monitoring student attendance and academic performance, and ensuring appropriate interaction with the student. The assessment of learning outcomes of students enrolled in resident programs at the Home Institution must be specifically addressed in the Campus Effectiveness Plan (the CEP ). The CEP must identify how data will be collected, set forth the rationale for using these data, provide a summary and analysis of the data collected, and explain how the data have been used to improve educational process. Curriculum Revision Upon completion of an online course, all students are requested to complete a student end-of-course evaluation to assess curriculum and instruction. The results of these assessments are used for continuous quality improvement of online courses and support services offered by the Host Institution. Teach-out Plans The Home Institution maintains full responsibility for properly administering any teachout plans that may be necessary if an educational program is going to be surrendered. The Home Institution must ensure that all required courses are made available to students consistent with any approved teach-out plan, which may include offering online courses, where appropriate. 2 of 7

Student Grievances The Home Institution is responsible for providing its students with a grievance policy and procedure for addressing their concerns and complaints. Any student complaints arising from registration for an online course will be handled through the published Student Complaint and Grievance Procedure, in full collaboration with the Host Institution. Disclosure The Consortium Agreement must be disclosed in the catalog in accordance with applicable accrediting body, state and federal regulations. The following description must be included in the Home Institution s school catalog: Online Courses - Any or all of the courses in a program that are marked with a + in the program outline for that program in the Curricula section of the school catalog may be taught either completely in residence at the school, completely online over the Internet as a distance education course or partially in residence and partially online, as determined by the school from time to time in its discretion. In order to help students become familiar with fundamentals of taking courses online over the Internet, the school may determine that a portion of the first online course that a student takes in this program must be taken online at the school in a supervised setting. Distance education courses are delivered online over the Internet through an asynchronous learning network. There is a prescribed schedule for completion provided for each of these courses. Support materials for each distance education course are sent to the student. These materials may include course syllabus, textbook, CD-ROM and other printed documents required for the distance education course. Students are assigned a cohort group for each distance education course. Online interaction within their assigned group and with the instructor is through discussion board and e-mail systems. Any student who is registered to take a distance education course will be assigned a unique login identifier and prompted to create a unique password. The unique login indentifier and instructions on how to create a unique password will be sent via e-mail to the student s e-mail account at the school. The student may not share his or her login identifier or password with other students or any person at the school. A copy of the school s privacy policy can be obtained at http://www.itt-tech.edu/privacy.cfm/. A student will not be charged any fees for verifying the student s identity. Online Student Preparation - Prior to starting any of the distance education courses taught online over the Internet in any program, the student is required to complete the online student preparation, which describes the protocols that the 3 of 7

student must follow when taking a distance education course online over the Internet. Online courses are supported through Faculty Services provided by the Online Division of the ITT Technical Institute located in Indianapolis, IN. Administration of Consortium Agreement The Consortium Agreement provides the legal basis required by the U. S. Department of Education to enable the Home Institutions listed herein to disburse Title IV Student Financial Assistance to a student enrolled in courses delivered by the Host Institution. The Home Institution agrees to perform or assist with performing the following designated functions: 1. Satisfy relevant criteria and standards of its accrediting body. 2. Facilitate the review and evaluation of the online resources provided by the Host Institution as requested by the Home Institution s accrediting body. 3. Award the student s degree, diploma, certificate and/or other recognized credential. 4. Designate and accept coursework of the Host Institution as equivalent to coursework at the Home Institution. 5. Establish enrollment agreements for each student and determine the period of enrollment for the agreement. 6. Maintain academic records and monitor Satisfactory Academic Progress. 7. Determine eligibility for and disbursement of Title IV Student Financial Assistance to eligible students who take courses pursuant the Consortium Agreement. 8. Approve student participation in conjunction with the Host Institution. 9. Specify beginning and ending enrollment dates for Title IV purposes. 10. Charge tuition to students taking online courses from the Host Institution at the same rate of the residential courses delivered by the Home Institution. 11. Define enrollment status (e.g. full time, half-time, less than half-time) for Title IV purposes. 12. Report enrollment information to the National Student Loan Data System (NSLDS). 13. Establish the student s cost of attendance in conjunction with the Host Institution. 14. Confirm that the student remains eligible for Title IV funds at time of disbursement. 15. Meet all deadlines established by the Host Institution for enrollment information prior to course starts. 16. Accept expense allocation for online courses offered as part of Home Institution resident programs. 17. Notify student of Satisfactory Academic Progress policy requirements. 18. Provide student support services. 19. Administer, in conjunction with the Host Institution, orientation to online courses. 20. Adhere to the Host Institution s defined drop/add period for courses delivered by Host Institution only. 21. Define the applicable refund and repayment policy for course additions/withdrawals. 4 of 7

22. Establish, in conjunction with the Host Institution, the student s last day of attendance, if applicable. 23. Disclose required consumer information. 24. Provide Institutional grievance policy and procedure for any student complaints regarding online courses provided by the Host Institution. 25. Maintain student financial aid and academic and attendance files. 26. Administer any approved teach-out agreement in the event an educational program is surrendered. 27. Disclose online learning opportunities and the description of services in the Home Institution s school catalog. 28. Appoint a responsible individual to oversee the online initiative on campus and to serve as a point of contact for participating students. The Host Institution agrees to perform the following functions: 1. Provide enrollment documentation, through internal systems (S3), to the Home Institution. 2. Provide student attendance in online courses to the Home Institution through the internal S3 system. The Home Institution is able to determine if the student ceases attendance in the course(s). 3. Provide end-of-session student course evaluation results to Home Institution through internal system access (Cognos). 4. Facilitate a forum with the Home Institution for the collective review of online curriculum. 5. Appoint qualified faculty for all online courses and be responsible for faculty orientation, in-service and faculty development activities. 6. Provide full disclosure for all faculty credentials to Home Institution as published in the Host Institution catalog. 7. Provide full disclosure of all facility and technological infrastructure requirements for online courses to the Home Institution as published in the Host Institution catalog. 8. Disclose relevant consumer information. 9. Make available to the Home Institution all information required for any related student complaints. 10. Maintain responsibility with the Home Institution for student orientation to online courses. 11. Maintain attendance records for courses taken and submit them on a regular basis to the Home Institution. 12. Provide pre-arranged access to the accrediting body of the Home Institution for purposes of evaluation of the online delivery of instruction to students being awarded credit by the Home Institution. The Home and Host Institution reserve the right to revise or amend this consortium agreement, as operational or regulatory needs require changes to be made. Such changes will be communicated promptly to the accrediting body. 5 of 7
