Upon completion, you ll be ready to succeed as a licensed cosmetologist. PHASES (SIX PHASES) Introduction/Alpha Unit You ll start by learning the fundamentals of cutting, styling and chemical restructuring of hair; skin and nail care; makeup; and the related sciences. Lectures, demonstrations and workshops concentrate on developing skill and accuracy and reinforcing classroom knowledge. You ll be introduced to retailing, client servicing and personal development skills. This unit also covers state safety requirements. Beta I and Beta II Unlock your creativity as you explore the latest trends and techniques in hair cutting, styling, coloring, permanent waving and chemical restructuring. Time in the clinic allows you to become increasingly confident in your professional abilities, and you begin to learn vital employment and career-advancing skills such as successful interviewing and resume writing. Gamma I and Gamma II Get ready to launch your career with in-depth training in client service, time management, self-promotion, goal-setting, merchandising and entrepreneurship. Prepare for an exciting future in skin care with Aveda Institute Des Moines. Our Esthiology curriculum provides 600 hours of skin care training with an emphasis on using pure flower and plant essences in treatments. Upon completion, you ll be ready to succeed as a licensed esthetician. PHASES (TWO PHASES) Introduction Learn the fundamentals of dermanalysis, facial manipulations and equipment and plant aromaology as you study the related sciences of anatomy and histology, along with state safety requirements. Combining theoretical knowledge and hands-on experience, this unit puts you on your way to a career in skin care. Alpha Apply your knowledge through clinic experiences while you increase your understanding in the classroom. In this unit, you ll refine your skills and gain the expertise you need to meet the Aveda Institute s skin care and makeup service standards and state safety requirements. Aveda Institute This is the place! Located in the Jordan Creek Town Center, the Aveda Institute Des Moines is the ideal setting for your professional education. Guest Service Areas A diverse array of clients come to the Aveda Institute for beauty and wellness services. As a student, you will have the opportunity to perform a spectrum of hair, skin and nail services in a virtual salon setting, under the supervision of your instructors. Resource Library and Administrative Offices A resource library has books on styling, motivation, health and wellness for your reference. Team leaders and admissions personnel are also available to respond to your questions and concerns. Student Classrooms Classrooms of all sizes have been designed to provide the proper environment for different types of learning and activities. Aveda Experience Center A retail center for Aveda hair, skin, flower and plant Pure-Fume and body care, makeup and lifestyle products. The store gives you the opportunity to practice your client service and retailing skills. 14 15

Admissions Are you ready to begin? If you re excited about the prospect of training at the Aveda Institute, here s all you have to do to get started. 1 Have an informational interview with an Admissions Representative. You will receive a written notice of acceptance or rejection. 2 Submit a letter of intent describing your reasons and goals for enrolling in the program. 3 Submit a letter of recommendation. (from someone other than a family member). 4 Submit a copy of your high school diploma or transcripts or a copy of your General Education Diploma(GED). 5 Submit a completed enrollment application (to be provided by your Admissions Representative) with a non-refundable $200 registration fee. Transfer Students Applicants for transfer into cosmetology and esthiology are considered on an individual basis. Aveda Institute may, at its discretion, refuse transfers if admission requirements, including tuition, cannot be met. The Aveda Institute, in its admissions, instruction and graduation policies, practices no discrimination on the basis of race, religion, color, financial status, sex, national origin, age, veteran or sexual orientation. The Aveda Institute does not recruit students that are currently attending or admitted to another school offering a similar program study. Sources of Financial Assistance Cash or Credit Card The Aveda Institute Des Moines offers a $1000 discount to Cosmetology students and a $350 discount to Esthiology students who pay their tuition in full 30 days prior to first day of classes. Financing Please see Student Services Coordinator for additional payment options that are available.. Sallie Mae/Personal Loans Students may be eligible for financing up to 100% of their tuition. See Student Services Coordinator for more information. Tuition Waiver for Multiple Programs Aveda Institute offers a $800 waiver on tuition if you enroll in a second program. You must begin the second program no more than six months from your first graduation date. Private Scholarships A number of organizations and corporations offer scholarships. Check with our Financial Aid representative or with your employer or parents employers for available scholarship programs. Veteran s Benefits If you are an honorably discharged veteran of the armed forces or a dependent of a deceased veteran, you may apply for veteran s educational benefits. Student Services Placement With a network of many salons, spas and experience centers nationwide, the Aveda Institute can help you begin your professional career. We ll help you gain the knowledge you need with biannual career fairs and self-promotional instruction. Nonetheless, the Aveda Institute is primarily an institution of learning and does not guarantee job placement. Individual Guidance If you experience personal challenges, Aveda Institute encourages students to contact Student Services for a list of local crisis hotlines, which provide personal counseling referrals to a network of professionals. Student Activities While at the Institute, you ll have the opportunity to participate in a variety of events and activities that are educational, interesting and just plain fun.. Alumni Association Aveda Institute Alumni Association allows graduates to keep in touch with classmates, receive advanced training and placement assistance.

Academic Information Student Evaluation Your progress at the Aveda Institute Des Moines will be evaluated on each Phase of your program. Each phase consists of: written, practical, projects, clinic or model days, quizzes, and homework. Grading Scale 100 97 A+ High Honors 96 95 A Honors 94 91 B 90 85 C 84 80 D 79 Below F Satisfactory Progress/Probation 1 Students must maintain an 80% average in each phase to meet satisfactory progress. 2 Students must maintain 95% attendance in each phase to meet satisfactory progress. For a complete description and specific number of days that can be missed in each phase, please refer to your Student Manual. Access to Cumulative Records Students, and parents of students under the age of 18, who are in regular attendance at Aveda Institute Des Moines, have the right to inspect and review the student s educational, financial and attendance records to ensure they are accurate, factual and do not violate the student s privacy or other rights. Students and/or parents should schedule an appointment with the administrative team leader to review the student s records. Student Information Release Policy Aveda Institute Des Moines requires written authorization from a student or parent/guardian (in case of a minor) or graduate in order to release academic, attendance, enrollment status, financial and/or any other information to agencies, prospective employers, or any other party seeking information about the student. 20 21

Graduation and Licensing Requirements Graduation Requirements To receive a diploma from Aveda Institute, you must: 1 Meet the minimum course requirements. Complete 2100 hours for 2 Cosmetology, or 600 hours for Esthiology. 3 Complete tuition and fee obligations. 4 Meet satisfactory progress requirements. 5 Meet service quota requirements. 6 Participate in extracurricular activities including career fairs, student shows, student recognitions and other activities. 7 Complete Final Practical Exam. Additional Hours If a student, after completing the required number of hours for the course, requests additional instruction, it may be provided at the discretion of the administrative team leader, at an hourly rate to be paid by the student in advance. Hourly rate is determined by dividing the current tuition by the number of hours in the course. Licensing Requirements To receive a license in the state of Iowa, a cosmetology or esthiology student is required to: 1 Submit a Diploma issued by a licensed school of Cosmetology Arts & Sciences (including date of graduation). 2 Complete application form and licensure fee (payable to the Iowa Board of Cosmetology Arts & Sciences) and submit to the Board Office. 3 Successfully complete the NIC examination with a score approved by the Board. 4 Complete an exit interview.

Policies and Standards To help you achieve excellence in cosmetology and esthiology, we have established these guidelines to ensure fairness, understanding and positive work habits among our students. Standards Because every student is a future employee, manager or entrepreneur, you must meet these standards of professionalism, which will prepare you for the demands of your future career: 1 Maintaining a professional appearance is vital to success. A professional appearance includes: a. The uniform provided in the student s kit is to be clean, neat and worn during all classroom and clinic-floor hours. It should not be torn, stained or altered. If it does not meet these standards, the student has 24 hours to replace it from Aveda at the student s expense. The uniforms supplied are to be worn with full length black dress pants and solid black shirt. No logos, graphics, or prints will be allowed with the exception of official Aveda shirts. Official Aveda shirts that are black are acceptable. b. Identification badges are to be worn as issued during all clocked hours to identify students to clients. If badge is lost, the student must replace it immediately at a cost of $10. c. Footwear should be professional in appearance and worn at all times. No flip flops or tennis shoes are allowed. d. Aveda Institute Des Moines reserves the right to maintain an esthetic standard for all students, including professional personal hygiene and grooming and, to the extent appropriate, makeup, appropriate facial hair and standard dress code adherence. e. Students who, in the reasonable determination of Aveda Institute Des Moines, are not dressed professionally will be dismissed. 22 23

To help prepare you for the workplace, Aveda Institute operates much like a professional salon environment. Late arrivals, absences and other interruptions in your training have a significant effect on your achievement just as they would if you were an employee in a salon, day spa or other professional environment. By law, we must keep track of your training hours for licensure or certification. 2 Students are expected to conduct themselves in a professional manner at all times and be mindful of the following: a. To maintain a learning environment for all students, anyone who is disruptive in the classroom or on the clinic floor (rudeness, foul language or other unprofessional behavior) may be dismissed for the day. b. Food, candy and gum are allowed in the lunchroom area only. Beverages can be consumed in the lunchroom and classroom areas only. c. Because Aveda Institute Des Moines is a smoke-free facility, smoking is not allowed on school property. d. So as not to interrupt the educational process, students will be notified of emergency phone calls only. Minor Standard Violations Minor violations include assigned area violations, property misuses, client service violations, unprofessional behavior, dress code violations and any disruptive behaviors determined by instructors and team leaders as interrupting or preventing the regular operation of the school or interrupting or preventing the education of other students. Anytime during the student s program the violation of a minor standard may result in lost hours, and repeated violations may result in suspension from the school or termination. Minor Standard Suspension Students who are suspended by the Review Board have up to 30 days to provide the Board documentation that the stipulations have been met. Once the Review Board has agreed that the student is prepared to comply with the professional standards of the school, the student may return to school; however, a record of suspension will be recorded in the student s permanent file. While a student is suspended, no clock hours may be earned, and tuition charges are suspended. If a student on suspension fails to respond in 30 days, the student is automatically terminated. Major Standard Violations Major standards include using controlled substances/alcohol, defacing or destroying property, stealing, falsifying documents, committing fraud, abusing and/or causing physical harm to others and violating local, state and federal laws. At anytime during a student s program, the violation of a major standard will result in termination. Attendance Policies Absences If students encounter situations that mandate missed hours, such as emergencies and illnesses, each field of study has a predetermined emergency pool of hours that can be used for class or clinic time missed. The pool of hours has been established as the maximum amount of time that students can be absent and still achieve the quality of skills that will be necessary in the workplace. Students are required to maintain 95% attendance rate in each phase. Please refer to your student manual for the maximum number of hours that can be missed in each phase. Cosmetology: Cosmetology students can be absent for a total of 105 hours, only 30 of which can be missed on Saturdays. After Cosmetology students have been absent for 95 hours or have used their 30 hours on Saturdays, they will receive a warning letter communicating that their pool of hours has only 10 hours remaining /or that they have exhausted their Saturday hours. After 105 hours and 15 minutes of absences or 30 hours and 15 minutes missed on Saturdays, the student is terminated without appeal. Esthiology: Esthiology students can be absent for a total of 30 hours, only 10 of which can be missed on Saturdays. After Esthiology students have been absent for 20 hours or have used their 10 hours on Saturdays, they will receive a warning letter communicating that their pool of hours has only 10 hours remaining or that they have exhausted their Saturday hours. After 30 hours and 15 minutes of absences or 10 hours and 15 minutes missed on Saturdays, the student is terminated without appeal. Please refer to your student manual for the maximum number of hours that can be missed in each phase. 24 25

Late Arrival All students arriving late must: clock in; proceed to the student service office to complete a late arrival form. When students are tardy more than 3 times in a phase, they will be dismissed for the day. Students arriving after 8:15 will be dismissed for the day. Early Releases Students leaving early must: obtain an early release form to be signed by instructor. Student should then scan out for the day and proceed to the student service office. Notice of Expected Absence Students planning to be absent must: obtain and complete a notice of expected absence form, have form signed by instructor, proceed to student service office and drop off completed form. Time Record Policies It is a state requirement that the school provide an accurate system for recording all students times, services and class hours. Students are ultimately responsible for tracking hours on a daily basis. Aveda Institute Des Moines will honor documented daily time worked, up to 10 hours. This is in accordance with the state of Iowa. Always scan in/out on the time clock at the beginning and end of each day, and at the beginning and end of each lunch break. Holidays Aveda Institute recognizes the following days as legal holidays: New Year s Day Memorial Day Independence Day Labor Day Thanksgiving Day December 25 Days off due to legal holidays are recorded as such and extend the enrollment contract.

Cancellation and Refund Policies Notice of Cancellation and Refund; Buyers right to Cancel The Contract and the student s enrollment can be terminated by written notice from one party to the other. Said notice shall be mailed, postage prepaid, either registered or certified, return receipt requested, to the other party at the address set out in this contract or delivered to the Student or the Administrative Director in person. The refund policy is as follows: B monies returned with the exception of the non-refundable registration fee. Thereafter, a student will be liable for: 1. The non-refundable registration fee plus; 2. The cost of any textbooks or supplies accepted plus; 3. C The student refund may be more than that stated above if the accrediting agency or federal refund policy results in a greater refund. The school shall acknowledge in writing any valid notice of cancellation within 10 days after the receipt of such notice. Any moneys due the applicant or student shall be refunded within 30 business days of the official withdrawal date or the date the school discovers that the student has officially withdrawn. The official withdrawal date is determined by the postmark date on the written notification or the date said notice is delivered to a school director in person. Veterans Refund Policy If the student receives veteran s benefits for attendance in a state or federally approved course and fails to enter the course, withdraws or if the student is terminated at any time prior to completion, that student receives a refund of all tuition fees and other charges which exceed a pro rated portion of the total charges. The pro rated portion is determined by the ratio of the number of days or hours of instruction completed to the total number of instructions days or hours in the course, in accordance with Chapter 36, Title 38 US Code Section 1776. An established registration fee in an amount not to exceed $10 need not be subject to pro-ration. Where the established registration fee is more than $10, the amount in excess is pro rated. Re-Admission All students who withdraw or are terminated from their program may reapply for admission, unless termination was due to a major violation. A re-registration fee of $200 is charged and a letter of commitment must be submitted for re-admittance.
