Online Enrollment Instructions Beneficiary Designation Open enrollment for faculty, administrative professionals and other eligible non-classified staff is completed via the Campus Administrative Portal which requires your eid to sign in. Before following these instructions, please make sure you are connected to the CSU network. 1. Sign into CAP with your eid a. If you need help resetting your epassword, please do one of the following: Call Human Resources at (970) 491-MyHR (6947) Take a picture ID to the Library s Technical Help Desk b. If you receive this error: Your ebusiness Suite Account has expired, call Human Resources at (970) 491-MyHR (6947). c. If you receive this error: Could not display page, you may not be connected to the CSU network. Click here for instructions on connecting to the network. 2. On the right side of the screen, under Quick Applications, choose Human Resources. 3. When the Oracle Applications Home Page opens, choose CSU Employee Self- Service from the left column. 11/2014 Page 1 of 12

Online Benefits Enrollment: Individuals and Beneficiaries 7. On the next screen, ensure that all your current individuals and beneficiaries are included in the list. a. To add a new beneficiary or individual, click Add Individual. b. If information for a CSU employee needs to be updated, ask that person to contact the HR Service Center at (970) 491-MyHR (6947). c. If information for an individual not employed by CSU needs to be updated, click the pencil icon in the Update column to the right of the person s name. 11/2014 Page 3 of 12
Add Individuals and Beneficiaries 8. Complete all applicable fields in the Name and Relationship area. If you are naming the individual as just a beneficiary, choose Beneficiary in the Relationship area. 9. Enter address in the Address Information area, or choose Shared Residence if the individual s address is the same as yours. 11/2014 Page 4 of 12
10. Complete all three fields in the Miscellaneous Information area. a. The SSN is required for individuals to be covered on insurance. If no SSN is provided, the person can be used only as a beneficiary. Beneficiaries do NOT require a SSN. 11. To add an estate, select Beneficiary for the Relationship 12. 13. Choose Female for Gender and enter 01-Jan-2001 in the Date of Birth Field 14. To designate a 501(c)3 charitable organization as your beneficiary, select Beneficiary-501(c)3 Charitable Organization for the Relationship area. And enter the name of the organization, fill in the mandatory address fields, and list the applicable tax identification number 15. Click Apply when the information is complete 16. Your updated list now includes the new designation. Repeat the above steps until your list is complete 11/2014 Page 6 of 12
Designate/Change/Update Beneficiary Designation 17. After adding all individuals, click Next at the bottom the Individuals and Beneficiaries page 18. On the Select Program page, make sure CSU Benefits Plan (Cost Share) is elected and click Next 11/2014 Page 7 of 12
19. On the Benefit Enrollment Tab-which shows current benefits enrollment and beneficiary designations, click on Update Benefits 11/2014 Page 8 of 12
20. The next screen is the Update Enrollments screen, where you can change any benefit plan enrollment (medical, dental, vision, life insurance, flexible spending accounts), find the plan that you want to change and select the new enrollment, once all enrollments are completed, click the Next button 11/2014 Page 9 of 12
21. The next screen is the Cover Individual page, where eligible dependents are designated for coverage (medical, dental, vision, life insurance). Make sure that any individuals that should be covered by clicking the checkbox under the Cover column. Click Next when your coverage selections are complete. 11/2014 Page 10 of 12
22. Update Beneficiaries Page Any individual (spouse, domestic partner or child) or beneficiary (individual, organization, trust or estate) that has been added to the table is listed here for you to designate as your beneficiary on plans. The employee is the beneficiary for all spouse, domestic partner and child policies. 23. Enter values in whole percentages (i.e., three beneficiaries might be designated at 33%, 33%, and 34%) for each beneficiary you wish to cover. 24. If you have not designated beneficiary percentages properly for any plan, the system will return an error message when you click the Next button. The error indicates which plan does not equal 100% of allocations. 11/2014 Page 11 of 12
25. If a beneficiary you want to designate on a plan is not listed below, click the Add/Edit Beneficiary button at the bottom of the page, and follow steps 8 16 above. 26. When all beneficiaries have been selected, click Next Confirmation Statement 27. Congratulations! You have successfully completed your benefits enrollment process for changing/updating beneficiaries 28. If any additional action on your part is required, it will be listed in the "warning" box near the top of the screen. 29. Be sure to print a copy of the Confirmation Statement for your records. You may also choose to print the current summary page by clicking the Printable Page button at the top of the screen. 30. Click Finish to complete your enrollment elections for the retirement program and return to the Insurance/Retirement Program page which reflects any changes made in the preceding steps. 31. To make further changes to your insurance plans, click Update Benefits. 32. To review and/or make changes to your retirement program, click Return to Ins/Retirement Programs.
