This summary contains an overview of the benefits provided to eligible employees of ASME. Eligibility: HEALTH CARE OPTIONS PICK WHAT S BEST FOR YOU! Full-time employees and part-time employees who work at least 30 hours per week are eligible for group health insurance benefits on the first day of the calendar month following one calendar month of employment. Medical: ASME offers a choice of two innovative health insurance programs through Aetna, providing you with the opportunity to select the plan that best suits your needs. Open Access Managed Choice POS: You do not need to designate a specific PCP and there is no referral required to see a specialist. There are both in and out of network components to this plan. HSA Open Access Managed Choice POS: This is a consumer directed high deductible plan with lower employee contributions. You will have the ability to control the cost of the care you receive through shopping around for the best prices for medication, etc. A savings account is set up and partially funded by ASME through employer contributions. In the HSA plan, preventive care is covered at 100%. You will then pay for medical expenses out of pocket (including prescriptions) until your deductible is satisfied. The funds roll over from year to year if they aren t used, and if you change jobs or retire from ASME, these funds are yours to use for qualified medical expenses. Prescription Coverage: Both plans have comprehensive prescription drug coverage. In addition to coverage at your regular pharmacy, there is also a mail order option for all maintenance medication which is available at discounted Aetna rates. Dental: ASME offers employees a choice of two Aetna dental plans. Aetna Dental DMO (Dental Maintenance Organization): You choose a primary dentist from a dental DMO network. There are no patient charges for most preventive procedures, no deductibles and no annual maximums. This is an in-network only plan. Aetna PPO (Preferred Provider Organization): You can see any dentist in the PPO network, or you can see an out-of-network dentist. -1-
Employee Contributions: Although most of the cost of the Group Health Insurance Program is paid by ASME, employees make pre-tax contributions through payroll deductions. Employee contributions will vary based on the combination of medical and dental plan options you choose. The ranges for bi-weekly employee contributions are: Single: $58.00 - $84.00 Employee + One Dependent: $173.00 - $253.00 Family: $222.00 - $346.00 UnitedHealthcare Vision: This benefit offers one eye exam per year in addition to various discounts on frames, contacts and lenses. You also have the flexibility of going to retail chain providers or private practive providers for eye exams. Flexible Spending Accounts: Flexible Spending Accounts allow eligible employees to set aside money on a pre-tax basis in an account to pay for eligible dependent care or health care expenses not covered by your insurance. Benefits VIP: Benefits VIP is who we rely on when we have any health care claim issues. Benefits VIP is a service being offered through our insurance broker. Aflac: Aflac Voluntary Benefits: ASME offers staff the option of enrolling in two Aflac plans that are paif got by employees with pre-tax dollars. Accident/Indemnity Advantage: AFLAC helps provide for an unpredictable future by paying cash benefits for accidental injuries. This plan will pay above and beyond what health insurance will cover in the event of an unforeseen accident. The payout ranges based on what type of medical services are provided. Personal Cancer Indemnity Plan Rider: AFLAC will go beyond the coverage that health insurance provides when there is a Cancer diagnosis. There are different levels of payouts depending on the treatment being given. There is also a wellness benefit payable individuals get specified annual screenings. Details of benefit payments are provided in the Aflac brochure. Optional Cancer Indemnity Rider: In addition to the Cancer Indemnity Plan, employees can opt to buy an additional Cancer Rider as well. This Cancer Rider expands coverage to additional will cover additional illnesses such as stroke, heart attack, or end-stage renal failure. -2-
WORK-LIFE BALANCE IS IMPORTANT TO US Paid Holidays: ASME has 11 paid holidays. Paid Time Off (PTO) and Disability Bank: ASME s PTO affords eligible employees a flexible approach to time off by providing one pool of days for time off from work. PTO provides time for vacation, personal business, emergencies, religious or other holidays not included in ASME s holiday schedule, appointments and illness of less than six days. New hires accrue 1.5 days per month starting on the first of the month following three calendar months of employment. This is equivalent to 18 days per year from when your accrual begins. After three years of service employees accrue at the rate of 24 days per year. You may accrue up to one times your annual earning rate in your PTO bank. In addition to PTO time, eligible employees accrue.5 days per month of Disability Bank time. The Disability Bank is available for any illness or injury that requires six or more consecutive days of absence from work and can be used beginning on the sixth day. There is no limit to how much Disability Bank time is accrued. Flex Time: ASME offers full-time employees a program of flexible work hours called Flextime. With supervisory approval, flextime gives you greater flexibility to work a schedule that best meets your personal needs within ASME s core hours. Telecommuting: ASME considers participation in our telecommuting program to be a viable alternative work arrangement in cases where the job, the individual, and supervisor are best suited to such an arrangement. Telecommuting allows a staff member to work from home for all or part of a regular workweek. Telecommuting is a work alternative that may be appropriate for some (but not all) jobs. Casual Business Attire: ASME has a casual business attire dress code. In general, casual business attire is clothing that should reflect an image of professionalism, should not be distracting to others and should be consistent with ASME guidelines. Wellness: ASME offers a variety of initiatives meant to help people achieve their wellness goals. From partnering with our wellness vendor WellCall, who provides health coaching at no cost for employees and their family members, to our annual health fair, there s something for everyone. We also try to provide employees with tools to help them stay healthy by offering some free on-site programs such as flu shots, cholesterol screenings and seminars on various health and wellness topics. -3-
Pension Plan: FINANCIAL SECURITY Employees are covered by ASME's defined contribution pension plan. ASME contributes 3% of salary into an individual account for each eligible employee. Employees become vested for pension benefits after 3 years of service. ASME pays the full cost for this benefit. Thrift Savings Plan: ASME offers eligible employees the opportunity to save for their future with this program. New employees are automatically enrolled but can choose to opt out if they do not want to participate. Contributions to the plan are on a taxdeferred basis through payroll deductions, and after one year of service ASME will match employee contributions up to 2% of their salary. ASME will increase the matching percentage after both five and ten years of service. College Savings Plan: ASME offers employees the opportunity to have a voluntary payroll deduction directed to a 529 College Savings Plan through Upromise. A 529 college savings is a tax-deferred savings plan that enables you to invest for college free of federal and, sometimes, state income taxes. You can use this investment to pay for tuition, certain roomand-board expenses, books supplies, and other qualified higher-education expenses. ASME IS HERE FOR YOU JUST IN CASE Life and Accidental Death and Dismemberment Insurance/Group Travel Accident Insurance: Employees are insured for an amount of Life Insurance equal to a multiple of their annual earnings. An additional benefit equal to the Life Insurance benefit is payable in the event of an accidental death. Employees are provided an additional Life Insurance benefit while traveling on company business. ASME also pays the full cost for all of these benefits. Short Term Disability Insurance: Disability benefits are available after seven calendar days of continuous disability, including maternity. disability benefits may continue for up to 180 days. Short term Long Term Disability (LTD) Insurance: LTD benefits commence after 180 days of continuous disability. Eligible employees are insured for 60% of monthly earnings. Benefits may continue up to age 65, and ASME pays the full cost for this benefit. ASME also provides an option to begin LTD benefits at 90 days of disability instead of 180 days. Employees pay for this optional coverage through a payroll deduction. -4-
Employee Assistance Plan (EAP): ASME s Employee Assistance Plan (EAP) is a benefit that is fully paid for by ASME, for employees and their household members to help identify and find resources to solve personal problems which, if not resolved, could adversely affect the employee s job performance. The EAP also includes information for things such as Child Care, Senior Care, and Financial and Legal Resources. Participation in the EAP is completely confidential. There is no waiting period before being able to use the EAP and you do not need to be enrolled in ASME medical insurance plan. GETTING AHEAD Tuition Reimbursement Plan: After completing 1 year of service, full-time employees are eligible to participate in the Tuition Reimbursement Plan. The plan provides 100% reimbursement for tuition and related expenses, up to a $2,500 annual maximum. Courses must meet policy guidelines. Job Postings: Designed to provide career growth to employees and strengthen ASME s workforce by matching employees goals with those of the Society. Open positions are posted on our Intranet. Training/Seminars: ASME encourages staff to develop their job-related skills. For example, we periodically offer in-house training programs. Management also supports staff attendance at external training workshops and seminars, such as those offered through AMA and other vendors where appropriate. THE BEST EMPLOYEES MAKE THE BEST REFERRALS Employee Referral Bonus: Many of our employees have referred people they know for open positions at ASME. If an employee refers someone who is hired and successfully completes three months of employment, the employee will be rewarded with a $1,000 bonus. -5-
