Financial Aid Required Forms Students should access the Student Portal and fill out the following forms: Complete W-4 Tax card Complete OPERS Retirement Exemption Form Complete Student Profile Sheet I-9 Eligibility to Work Form (OGS) Students with a GA, TA or GRS award need to obtain a letter from the Center to apply for a social security number. Please see Bri Dowler in Yamada House 104 to collect your letter. This letter will need to be turned in to ISFS. You MUST be registered for classes before you can receive letter and apply for a social security number. Check your graduate appointment policies! Center for International Studies 38
Service Assignments See your Program Director for your work assignment. It is your responsibility to check in with your supervisor during the 1st week of the semester to set a work schedule. Must maintain at least a 3.0 grade point average Continuation of any award depends on satisfactory work performance Online time sheets will need to be completed by ALL students that hold a GA or TA work assignment.
