OVERVIEW OF YOUR BENEFITS ELIGIBILITY CLASSES I II III Full-time members who generally work 35 or more hours per week. Coverage is for you and your family. Part-time members who generally work on average between 22 but less than 35 hours per week. Part-time members receive benefits for themselves only. Coverage is for the Member only. There is no dependent coverage provided by the Fund. Part-time members who generally work more than 7 hours per week but less than 22 hours per week. Part-time members receive benefits for themselves only. Coverage is for the Member only. There is no dependent coverage provided by the Fund. Note: Contributions are made into the Benefit Fund monthly by Genesis Employers on a per capita basis. Eligibility for Genesis employees is established using four tiers (Member only; Member and Spouse; Member and dependent Children; or Member, Spouse and dependent Children), based upon your Family s composition and the amount that you elect to contribute toward these benefits through payroll deductions. All Genesis Members are considered Eligibility Class I for purposes of establishing eligibility for benefits. Effective June 2009, the following benefits are provided by Aetna subject to an individual or family deductible, co-payments and limitations as described on pages 11-22. Unless otherwise indicated, these benefits are provided to all eligible full-time members and their families and part-time members. Other than in emergency situations, benefits are provided for services rendered by participating providers only. 12

The following Dental benefits are provided by Cigna subject to co-payments and limitations as described. Other than in emergency situations, benefits are not provided for services rendered by a non-participating Dentist. DENTAL Benefits provided for full-time members and their eligible dependents only, and Genesis employees and their eligible dependents based upon your family election. Each individual must select a Cigna dentist as their designated (or primary care) dentist. Other than for emergencies, services must be performed by your designated Cigna Participating dentist. Referrals required for some specialists Co-payments and limitations may apply for some procedures excluding essential oral pediatric services. No annual maximum 19

DISABILITY BENEFITS Member Only Benefit Disability benefits are provided through your employer. LIFE INSURANCE Member must submit proof to the Benefit Fund that disability benefits have been received to maintain your health coverage for up to 26 weeks. Follow the same procedure if you are receiving Workers Compensation. If you need help or advice in filing a Workers Compensation claim, call the Benefit Fund at (646) 473-9200. Member Only Benefit Eligibility Class I During your first year of service, benefit is $2,000. After your first year, based on your years of service and annual earnings up to a maximum of $25,000. ACCIDENTAL DEATH AND DISMEMBERMENT Eligibility Class II During your first year of service, benefit is $1,250. Maximum benefit amount is $2,500. Eligibility Class III Maximum benefit amount is $1,250.
