The terms are as follows: 1. Number of students to be admitted 12 students. 2. Field of recruitment: Courses offered in the field of dental research by the Graduate School. Applicant Qualifications (1) Academic Background: Completion (or expected graduation by September 2015) of 18 years of formal schooling (medical, dental, pharmacy, or veterinary degree) outside Japan. (2) Health: Applicants must be in good health. (3) Language Ability: Applicants must have sufficient proficiency in English. (4) Applicants not in Japan at time of application must arrive in Japan between September 17 to October 13, 2015. Note: Acceptance will be cancelled if an applicant fails to arrive in Japan by the specified date. 5. International application form for graduate school admission (Prescribed form by the university: Form 3) Applications without a prospective supervisor s signature and seal will not be accepted. To submit the International Application Form for Graduate School Admission 1. After filling out all of the items on the form, including your signature, please make the form into a PDF file and email it to your prospective supervisor. Please request your prospective supervisor to sign the form in the prescribed place and affix his/her personal seal on it, and to then email the completed, signed PDF application form back to you. The signature shows that the supervisor will accept you into his/her department if you pass the selection process. 2. After you receive the PDF back from the prospective supervisor, please check to make sure his/her signature and seal appear on it. 3. Next, please submit the signed PDF version of the application form and the original Excel file version of the "International Application Form for Graduate School Admission" (it contains the information on the PDF file, but not the signatures) to the Graduate School Entrance Exam Team, Admission Section [nyushi.adm@cmn.tmd.ac.jp] by email. IMPORTANT NOTICES IN SUBMISSION OF THE DOCUMENTS - Applicants should download the forms to be submitted (Forms 1-3) from TMDU s website [http://www.tmd.ac.jp/english/admissions/application/graduate/index.html ]. - Application documents, once submitted, will not be returned for any reason. - Applicants are advised to keep in close contact with their prospective supervisor and to discuss him/her their research in the course. - 2 -

- All documents should be typed in Japanese or English on paper of the same size (A4 size, preferably). A Japanese translation is needed if an official certificate is in another language. - Above documents 1 and 2 should be prepared on the prescribed forms provided by TMDU and must be submitted both on paper and by digital file. 7. Selection Methods Acceptance to the Graduate School is determined solely by a review of the application documents submitted. 8. Announcement of successful applicants July 17 2015 (Friday) at 13:00 A list of successful applicants will be posted on the Graduate School Bulletin Board (located in front of Building 6 at Tokyo Medical and Dental University). The list will also be posted on the university website (http://www.tmd.ac.jp). We cannot respond to telephone inquiries regarding application screening or its results. 9. Admission Procedure (1) Period September 24, 2015 ( Thursday) to September 30, 2015 ( Wednesday) (2) Office hours: 9:00 to 17:00 Monday Friday (3) Place Admissions Section, Academic Affairs Division (Building 1 West, 1 st Floor), (4) Tuition and Other Costs (As of FY 2015) Admission fee: 282,000 yen Tuition: 267,900 yen (Six months) 535,800 yen (One year) If the admission or tuition fees are revised, the new amount will apply from the time of revision. Payment of the admission fee should be done using the admission fee transfer request form, and wired from a bank near you. If you wish to pay the fees from abroad, please contact the Admission Section, Educational Affaires Division by email [nyushi.adm@ tmd.ac.jp]. Payment of tuition after admission is done by automatic withdrawal from your bank account. (Details will be provided in the admission procedure.) Others (1) Changes to the contents of an application (including the change of the department you want to enter) are not allowed after the application package has been submitted. (5) If any difference is found between facts and matters as described in your application documents, your enrollment may be canceled. (6) Handling of personal information. 1. Personal information is collected in the application process and selection process, and may be used to carry out health guidance or education after admission. It will not be used for any other the purposes. 2. Regarding personal information collected in the process of selecting entrants, the National University Corporation Personal Information Management Rules will be followed and TMDU will not disclose any such information to third parties without consent of the applicant. (7) Admission will be canceled if an applicant who applies under the condition of expecting to receive a master's degree or of graduating from an 18-year course of schooling before the deadline is not able to receive such degree or to graduate by the deadline (September 30, 2015). (8) Students are encouraged to apply for scholarships other than the Japanese Government (Monbukagakusho) Scholarship. (9) Tuition may be exempted based on student's academic performance and financial status. (10) Students are advised to acquire some knowledge of Japanese culture, customs, weather, and the university system before coming to Japan. The use of English is given priority in study and research; however, some ability to speak Japanese will be needed right away to conduct your daily affairs in Japan. It is strongly advised, therefore, that international students acquire some level of Japanese language proficiency before starting at the university.
