Taylor Unit Our Mission Statement We provide effective and appropriate individualised rehabilitation for each patient, using a holistic approach and taking into consideration the hopes and expectations of the patient and their relatives/carers. We aim to provide an environment where our patient s their self esteem and dignity are maintained and enhanced so that they can be encouraged to achieve their optimum potential for living. What Is Rehabilitation? Rehabilitation is a form of treatment which aims to help people to recover as fully as possible from neurological illnesses. Where this is not possible, rehabilitation aims to help people achieve quality of life through maximising individual s capabilities. Neurological rehabilitation offers a service to people who have suffered from a neurological illness, which involves the impairment of the brain, spinal cord and nerves. Conditions commonly treated are head injury, multiple sclerosis, and stroke. Rehabilitation is provided by a team to ensure that all aspects of care are considered. As part of this team you and your relatives/carers will be involved in discussions regarding the nature of your condition. Therapeutic recovery can vary from weeks up to many months. Discharge arrangements are focused on the individual needs of the patients, which range from minimal to highly complex care packages. In some exceptional circumstances a patient may need to be transferred to Royal Albert Edward Infirmary, Wigan for medical emergencies. This will only be considered if essential to maintain an individual s health and wellbeing. We are unable to transfer to any other hospitals in this emergency situation. Goal Setting and Family meeting During your stay on the unit you will be invited to attend a goal setting meeting. This is where you will be given the opportunity to discuss your expectations with the rehabilitation team and set goals for your treatment. Family members/carers can be invited to these meetings at your request. Patients and family are always welcome to approach a staff member for any clarification or concerns. One family member will be identified as the main contact for passing on information about your care and progress (with your consent). This means that we are not then dealing with multiple requests for information from relatives. Discharge planning will be addressed with all concerned when the rehab process has reached a plateaux or your needs can be met in the community. What You May Need To Bring With You Upon admission to the unit you may want to consider bringing some of the items below: Clothing (day & night) Outdoor coat Well fitting shoes/trainers Toiletries (including bath/shower gel, toothbrush, toothpaste, deodorant, razor, shaving foam, denture adhesive & pot if required, comb, etc) Photographs/meaningful objects Mobile phones are allowed on the unit, although you are asked to use them with respect. Patients are asked to make arrangements with your relatives/carers for your personal laundry to be done at home. The Team System When you first come to the unit you will be allocated to one of 2 teams. Each team will consist of a consultant, along with a small team of Doctors, Psychologist, Nurses, Occupational Therapists, Physiotherapists, Speech and Language Therapist and Support Workers. Treatment is an ongoing process and all staff and patients are actively involved. You may hear the term inter-disciplinary/multi-disciplinary - these refer to the members of the team and how they integrate and work together in order to provide the most effective treatment possible. The Team Medical team On the unit there are Consultants who specialise in neuro-rehabilitation as well as other Doctors at different levels of training. A medical ward round takes place once a week, where you will have the opportunity to see the Consultant and Doctors to discuss any medical aspects relating to your care. The medical input on the unit is 9:00 am 5:00 pm weekdays, excluding bank holidays. If any medical problems arise out of these hours patients are then referred to the Doctors on call at the Royal Albert Edward Infirmary. After discharge most patients are given a follow-up appointment with the Consultant. The out-patient appointment is for the rehabilitation clinic. Nursing team The Nursing team are available to assist and advise on most matters twenty four hours a day. On admission a named Nurse will be identified. The named Nurse is also responsible for arranging progress meetings where you and your relatives will be invited to discuss your progress or any problems. The Taylor Unit is an established area for the training of Student Nurses and other Allied Health Professions who may work with you under supervision. Key Therapist (this may be a Therapist from any therapy profession) Alongside your named Nurse each patient will be assigned a named key Therapist. They will introduce themselves to you and should be approached for an update on your progress. The named key Therapist will work alongside your named Nurse to co-ordinate your care from admission to discharge. Therapists Therapists are available on the Taylor Unit, between the hours of 8:00 am 4:00 pm each weekday. Occupational Therapist The specific role of the Occupational Therapist includes the assessment and teaching of daily living skills, these include washing and dressing, cooking and shopping. The Occupational Therapists work closely with the family, and carry out home visits to assess the environment to which the patient will return on discharge. If adaptations or aids are required, the Occupational Therapists will work with Social Services to make any changes that maybe needed I your home and organise these for you. Patients may be asked to contribute financially to aspects of their treatment, e.g. shopping and community trips. Physiotherapists The Physiotherapists will assess each patient on admission to the unit and identify an appropriate programme of physical therapy for each individual; this may be completed within daily activities or individual therapy sessions. Relatives may be encouraged to observe and join in therapy sessions where appropriate. Taylor Rehabilitation Unit Page 4 of 9

It would be helpful if you could bring with you on admission any aids you are using such as walking aids, wheelchairs and any other splints or appliances that have been given to you. Speech and Language Therapy The Speech and Language Therapists will assess any communication difficulties you may be experiencing. They can help you and your family or carers to overcome and/or adapt to a range of communication problems. They also assess swallowing problems and advise on safe ways to eat and drink. If you are currently using a communication book/chart, or have an electronic communication aid, please bring it with you onto the Unit. Clinical psychologist The Clinical Psychologist aims to help individual patients and their families recognise, understand and cope with many of the invisible effects of a new or progressive disability. These may include difficulties with memory, thinking, concentration, feelings of anxiety, anger, and frustration or depression. The Clinical Psychologist can explore the possible effects of disability on a person on physical relationships with the patient and their carers. Emphasis is placed on encouraging self management and learning to deal with problems, whilst planning positively for the future. Rehabilitation Support Worker (RSW) The Rehabilitation Support Worker works with the Therapists and Nursing staff on the unit. The RSW will continue treatments already established by Physiotherapists and Occupational Therapists. Social & Educational Facilitator The SEF co-ordinates activities on the unit to encourage social interaction and participation. They may also assist Therapists in further community activities. Your Daily Timetable In the morning you will be given the opportunity to complete your personal care routine by a member of your team, this may be incorporated into your rehabilitation programme. Treatment is focussed around normal movement principles. This encourages you to relearn normal patterns of movement and incorporate your affected limbs as normally as possible. A personal plan will be developed to meet your individual needs. Some treatments used will focus on teaching you alternative ways to increase your independence and ability to carry out daily tasks. Your rehabilitation may include taking an active role in daily household tasks, joining in with group activities which may include community activities for social and treatment reasons. Mealtimes We operate a protected mealtime policy, and do not allow visiting at these times. 08:00 11:00 12:45 13:15 16:45 17:15 We also discourage visitors from bringing perishable foods in for patients unless prior agreement has been made with the ward manager. We apologies for any inconvenience this may cause. Visiting Times Visiting times are as follows: 14:00 16:00 & 18:30 20:00 7 Days a Week Although the number of visitors is not restricted, we would ask that you recognise that large numbers of people can pose a hazard on the unit due to the limited space available. We also operate an Open Visiting policy, whereby one named family member is able to be on the ward from 8:00 am 8:00 pm each day. Please note that at times they may be asked to leave momentarily for reasons of privacy, dignity and confidentiality. Planning Your Discharge To ensure a smooth, trouble free discharge, planning for your return home will begin on admission. If it is necessary you will be referred to a Social Worker and screened for Continuing Health Care. You will be informed of this. Discharge needs are assessed on an individual basis. Discharge will always be discussed and planned with you, your relatives/carers and team members. Discharge planning may include plans for day leave or over night stay. If you have ongoing rehabilitation needs which can be met in the community or as an outpatient, you will be referred on to the appropriate service on discharge. There may be a waiting list for these services. Parking The car parking available is Pay & Display. Concessionary parking is available for your visitors if you have been on the unit for more than two weeks, please see staff for details. Taylor Rehabilitation Unit Page 6 of 9

Bus Services The 597 bus will travel from Leigh bus station and bring you into the hospital grounds, for more information telephone Greater Manchester Public Transport Information Ltd. 0161 228 7811. Chaplain & Church Services Chaplain services can be available on request. The hospital chapel is situated on the main corridor. People of all religions are welcome. Smoking WWL NHS Trust operates a No Smoking policy, which is strictly enforced within all its buildings. Illegal substances Use and/or storage of illegal substances in the unit are strictly forbidden. If such substances are discovered, legal advice will be taken and patients will be asked to leave the unit. Alcohol The consumption of alcohol in the unit is also strictly forbidden. Alcohol following head injury can be potentially lethal and may induce convulsions. It may also exacerbate the reaction to certain medications given to prevent epilepsy. Please ask the nurse in charge if you are unclear on this matter. Zero Tolerance Everyone has the right to be treated with dignity by those they come into contact with. Unacceptable behaviours will invoke the use of the Trust s zero tolerance policy.
