Application This procedure is to be used for the notification of all pertinent parties as to the impending project to be constructed by Land & Building Services. 2. Definitions Refer to document I-D-04, Projects Definitions. 4. Procedures A. IN-HOUSE CONSTRUCTION PROJECTS 1. Construction Notification a) Upon authorization of the Pre-construction Estimate and prior to any construction start-up, all affected parties to the project must receive proper notification as to the start and anticipated completion dates of construction. b) The Project Coordinator shall discuss trade resource scheduling with the Scheduler to confirm the earliest possible start date and whether work can be completed by the set deadline, with some allowance for change orders during construction. The Project Manager shall be notified of the construction scheduling, who in turn will advise the Design Leader, Customer and Facility Manager. c) The Project Coordinator shall ensure that all shop hazard assessments have been received from the trades involved in the Project, along with detailed work procedures, in strict conformance with the Policy I-B-28, Hazard Identification & Assessment. Any missing or unclear information shall be remedied by the Project Coordinator prior to any work order being issued to the shop in question, with a copy of the project hazard assessment attached to all work orders issued by the Service Centre Coordinator. d) The Notification of Project Construction shall be issued on all Plant Operations projects exceeding $10,000 in value. The Project Coordinator shall generate such notification using the
I-D-29 FG CONSTRUCTION NOTIFICATION & START UP.DOC PAGE Const Notification <rev. date>.doc form (attached as Schedule A), and e-mail the draft electronic file to the Project Manager. e) The Project Manager shall complete the form, obtain the Scheduler s sign-off on the dates listed, and shall arrange with the Projects Clerk for distribution of the form by fax or Campus Mail at least 24 hours before construction start-up. The distribution list is given in Appendix I. f) If the Project involves the removal, encapsulation, or enclosure of friable asbestos materials or the demolition, dismantling, or repair of any part of a building or structure in which insulating materials containing asbestos have been used, or in which asbestos products have been manufactured, the Project Coordinator shall contact the LBS Asbestos Coordinator to see if the work is covered by the University s standing Notice of Project (NOP) with the WCB. If not, the Project Coordinator shall furnish all necessary information to the LBS Asbestos Coordinator to enable a special NOP to be issued to the WCB at least 24 hours before construction start-up. A copy of the completed NOP form shall be posted at the worksite, on the Project Safety Board along with all other Hazard Assessment information. 2. Construction Start-Up a) Site mobilization shall take place as per the earliest start date advised by the Scheduler, involving setting up of necessary site office, hoarding, material storage and other temporary facilities, site signage and work start-up. b) The minimum required site signage shall consist of: i) Project Information Board, including copy of the Notification of Project Construction, building permit (if applicable), and relevant construction plans; and ii) Project Safety Board, including copy of Site Safety Rules (attached as Schedule B), all hazard assessment information, asbestos assessments and notices, warning signs, WCB information and a Project Site Sign-In sheet (attached as Schedule C) for all staff entering the job site, acknowledging that they have read all information posted on the Safety Board. c) A Pre-Construction Meeting shall be held by the Project Coordinator to discuss the schedule, roles and responsibilities, lines of communication, key schedule milestones, inspection schedule, safety and hazard issues, logistics, etc. relevant to the Project. All high risk hazards shall be specifically noted and discussed. In attendance shall be the Project Manager, Site Supervisor, Scheduler, all Trade Heads who will have staff participating in the construction, sub-contractor representatives, Design Leader and key design professionals. The form of agenda to be followed is attached as Schedule D. The meeting shall be minuted with copies distributed to all participants. d) All invitees and participants in the Pre-Construction Meeting shall review and sign-off on the Project Safety Board before leaving the site. The Project Coordinator shall advise the Project Clerk to contact all absentees from the Pre-Construction Meeting to visit the site, review and sign-off on the Project Safety Board and report back as soon as that is complete. Until such is undertaken, no staff of the delinquent supervisory staff shall enter the site. It is the responsibility of each Trade Head and all other supervisors of staff assigned to the jobsite to be aware of the site conditions and to advise their staff accordingly, including directing them to read the Project Safety Board and have them sign-in prior to entering the jobsite. OUTSOURCED CONSTRUCTION PROJECTS 1. Construction Notification a) Upon authorization of the Pre-construction Estimate, contract award to the successful Contractor and prior to any construction start-up, all affected parties to the project must receive proper notification as to the start and anticipated completion dates of construction. b) The Contractor shall forward to the Project Manager a Gantt chart showing the proposed construction scheduling. The Project Manager shall in turn advise the Design Leader, Customer and Facility Manager. c) The Notification of Project Construction shall be issued on all Plant Operations projects exceeding $10,000 in value. The Project Manager shall generate such notification using the Const Notification <rev. date>.doc form (attached as Schedule A), and shall arrange with the Projects Clerk for distribution of the form by fax or Campus Mail at least 24 hours before construction start-up. The distribution list is given in Appendix I. d) The Project Manager shall notify the Contractor to ensure that the Contractor s Safety Program is prepared and submitted to the Worker s Compensation Board and copied to the Project Manager. The safety program shall include, but not be limited to: i) The Contractor s policy statement of aims and responsibilities of the employer, supervisors and workers, with reference to the latest version of the WCB Regulations; ii) Provision for the regular inspection of the premises, equipment, work methods and practices and procedures to be taken to correct any hazardous conditions; iii) Policy on investigation of accidents/incidents and determination of corrective action; iv) Emergency preparedness information; v) Policy on personal protective clothing and equipment standards and first aid services; vi) Formation of a job site safety committee, frequency of meetings and reporting criteria; and vii) Maintenance of records and statistics for tracking and planning purposes and the availability of this information for the WCB, the University s Health, Safety & Environment groups, management and the workers; The Project Manager shall forward a copy of the Contractor s Safety Program to the LBS Health, Safety & Environment group. e) If the Project involves the removal, encapsulation, or enclosure of friable asbestos materials or the demolition, dismantling, or repair of any part of a building or structure in which insulating materials containing asbestos have been used, or in which asbestos products have been manufactured, the Contractor shall also prepare and submit a Notice of Project Asbestos form to Worker s Compensation Board and copied to the Project Manager at least 24 hours before construction start-up. SCHEDULE B UBC PLANT OPERATIONS PROJECT SERVICES DIVISION SITE SAFETY RULES 1. Only approved substantial footwear with safety toes shall be worn. 2. Hard hats must be worn while on site. 3. Hand, eye, ear, respiratory and skin protection is required when exposed to hazards. 4. All floors, ceilings and walls shall be checked against the asbestos inventory to determine the presence of ACM and appropriate work procedures prior to commencing work. 5. Guardrails and handrails are not to be removed unless required by the work in the immediate area and railings so removed must be replaced before the worker leaves the area. 6. No horseplay of any kind will be tolerated. 7. Any worker, as determined by the site supervisor, found to be unfit for his/her duties shall be removed from the site. 8. The site must be kept clean and orderly at all times. 9. No music shall be played which inhibits communication between workers or disrupts building occupants. 10. Ladders must be secured top and bottom. Do not use poorly constructed or defective ladders. 11. Scaffold planks or beams must be of sound material and clear of defects. Do not use poorly constructed scaffold. See that scaffolding is provided with guardrails. 12. Shorts and torn clothing exposing skin are not permitted. 13. All persons entering the site must report to the Site Supervisor or person in charge. 14. All trades personnel must read the site notice boards before commencing work. 15. Personnel not trained in specific UBC policies or procedures related to work practices shall not undertake such work. 16. Persons failing to comply with the above or any WCB regulation and thereby causing hazards to either himself or his fellow workers will be removed from the site. SAFETY COMES FIRST
