These questions are designed to provide us with an understanding of how well the pilot program is working and how it can be improved. Your responses will not be individually shared with the Government, nor will you be identified in any way to anyone not on Westat s evaluation team. In answering the questions, please consider only this establishment. By that, we mean the business location, branch or division at your current address. Please do not include information about other offices or sites of this firm. This survey includes a number of questions about employment verification at your establishment. Your answers are very important to us.
