DATA BACKUP / RESTORATION WARNING Please read carefully. Please note that since computer hard drives are mechanical devices, they will ALL eventually fail (regardless of the age of the computer). A failure occurs when a hard disk drive malfunctions (for various reasons) and the stored information cannot be accessed. Desktop Services strongly recommends storing Monash data on a Monash data server (e.g. Novell). The advantages of storing Monash data on a server is that your data is secure, backed up, accessible from any computer with Internet access and can be shared. While not as cheap as an external hard drive, the data server solution is much more reliable and flexible than a computer hard disk, which provides a single point of data loss. If you decide to keep Monash data on your laptop, please ensure that you backup regularly and check the reliability of these backups regularly by doing test restores of random data. Also, you should keep your backups secure and in a separate location to the source data. Have you backed up your data? Yes No Is there any data on this computer you want to have backed up? Yes No [If you select no, all data will be overwritten when the computer is reimaged] If yes, where is your data kept? Eg. My Documents or Documents (for Macs) (Please provide an accurate list of all folders you need backed up or restored. Only data you list below will be backed up. All other data will be lost in the reimage. NOTE: While every effort will be taken to ensure the integrity of your data, we cannot guarantee that we will be able to backup or restore all data. Page 4 of 5

NOTES: Personal Data We are unable to backup and restore personal data (eg photos, videos etc) Music/Audio Files The University's Information Technology Use Policy - Staff & Other Authorised Users clearly states that staff should not: convert a CD to an audio format, such as MP3, and using it on a PC; download a film, MP3 recordings, or software from the internet using University internet access or computers; http://www.policy.monash.edu/policy-bank/management/its/it-use-policy-staff-andauthorised.html Therefore, we will not backup and restore commercial music files. Any work related sound files must be stored in a separate folder so that they can be backed up. My work-related sound files are stored in: DATA RESTORE Do you want the data burnt to DVD or restored back on the reimaged computer? (NOTE: Backup DVDs cost $50/DVD.) Restore Data on hard drive Backup data to DVD and restore data to hard drive Backup data to user-provided storage device (e.g. external portable hard drive) NOTE: While every effort will be taken to ensure the integrity of your data, we cannot guarantee that we will be able to backup or restore all data.
