TAKE YOUR MARK Timeline and Summary of Activities As you begin your Houston Methodist journey, be sure to continually review the following timeline so you know what activities you need to complete and by when. Enrolling for Benefits Benefits enrollment is a crucial step in your Houston Methodist onboarding. Enroll through MARS via mymethodistbenefi ts.com within 30 days of your benefits eligibility date to ensure you and your family are covered for 2014. You can enroll from anywhere. U From a Houston Methodist Facility U From Home (or non-houston Methodist location) Enroll from HomE U During an Onboarding Event You will be able to enroll for benefits and have your questions answered at the Onboarding Events in February. See page 20 for dates and times. You must be connected to a Houston Methodist network to access. Important! You must register for MARS Home Access before you can enroll from home. For enrollment requirements and step-by-step instructions on how to complete your benefits enrollment and/or add dependents or beneficiaries, refer to your 2014 Benefi ts Guide, found in your Welcome Packet. If you are benefits eligible you must enroll for benefits by March 2, 2014, to have benefits coverage for 2014.* Your Houston Methodist Badge For security, please wear your badge above your waist while working at Houston Methodist locations. It also contains your Employee ID number (see placement below). For time and attendance, please use your badge to clock in and out of work. For purchasing power, the badge may operate like a debit card at some locations, including the cafeteria. Funds will be deducted from your paycheck when purchasing items. If your date of hire is February 1, 2014, your director should have provided you with your Houston Methodist network ID, email account, and employee badge. WHAT LANE ARE YOU IN? Defining Employee Status and Benefits Eligibility Houston Methodist uses Standard Hours to define employee status and determine eligibility for certain benefits. Standard Hours are the hours you are typically scheduled to work in a week (e.g., 20, 24, 28, 32, 36, 40, etc.). Standard Hours are also used to calculate your Paid Time Off (PTO), Life Insurance and Long-Term Disability (LTD) benefits. For more information about how your Standard Hours and benefit options work together, please refer to page 7 of your 2014 Benefi ts Guide, found in your Welcome Packet. Employee Classification Range of Standard Hours Benefits Eligible Full Time 32 to 40 Yes Part Time 1 to 31 Yes (20 to 31 Standard Hours ) PRN 1 to 40 No Temporary* 1 to 40 No *Expected to work for less than six months Benefits Eligibility Employees with Standard Hours of 20 or more, who are not classified as temporary or PRN, are eligible for benefits. How Your CHRISTUS Service Enhances Your Houston Methodist Benefits As you review your new benefit plans, you may notice that a few of our programs use Years of Service to calculate the amount of coverage or benefit you will receive. Based on the benefit offering, your CHRISTUS service may enhance your benefits at Houston Methodist. See the chart below for a more detailed explanation of how your CHRISTUS service plays a role. Defining Dependents As you prepare to enroll for benefits, review the criteria for covering dependents. For your reference, the following table breaks down the requirements for different dependent types. Verifications through ConSova Houston Methodist partners with the ConSova Corporation to verify dependent eligibility and Spousal Surcharge for the Medical Plan. See page 9 for more details on the Spousal Surcharge. These verifications are part of a multi-pronged approach to help control rising Medical Plan costs by ensuring that only eligible dependents are covered. The ConSova Corporation will contact you if you cover dependents in the Medical Plan or if you enroll a spouse who is identified as not being eligible for his/her employer s medical plan. 6 DIVE IN

St. Catherine and St. John physicians who currently contract with UHC have been transitioned to the Houston Methodist Preferred Network as of February 1, 2014. Understanding the Spousal Surcharge Spouses typically cost Houston Methodist more per member than our own employees and have hospitalization costs of two times that of our employees. As a way to control Medical Plan costs and keep premiums as low as possible, we include a Spousal Surcharge of $100 per pay period. The Spousal Surcharge only applies to spouses enrolled in the Houston Methodist Medical Plan who are eligible for medical coverage through their own employer s group-sponsored health insurance plan. The surcharge does not apply to spouses who are self-employed or covered by Medicare. Need vision coverage? Make sure you elect it during enrollment! At Houston Methodist, we offer vision coverage separate from our Medical Plan, giving you and your family the choice to opt in for coverage or not. When are my benefits effective? ID Cards and Retroactive Premiums Your Houston Methodist Medical, Dental, Pharmacy and Vision Plan ID cards will arrive 10 to 15 business days after enrollment. Prescription Drug Program The Prescription Drug Program is administered by CVS Caremark, with a retail network of more than 64,000 locations, including most chain stores such as CVS, Walgreens and Wal-Mart. Filling Your Prescriptions Maintenance Drugs Short-term medications may be filled for a 30-day supply at a retail in-network pharmacy. Long-term medications, for maintenance and non-maintenance medications, may be filled for a 90-day supply at a retail CVS pharmacy or through Caremark s mail services program. Maintenance drugs must be filled for a 90-day supply once you have obtained two one-month (up to 30-day) supplies of medication. Make sure your prescriptions are still covered! A CONFIDENT JOURNEY Stay Afloat with Life and Accidental Death and Dismemberment (AD&D) Insurance Houston Methodist provides Basic Life and AD&D coverage to you free of charge. This way, in the event of your death, your beneficiaries do not have to pay tax on the benefit distributed to them. The IRS determines the amount of imputed income based on your benefit amount and age. Added to your earnings, your paycheck shows this amount (see page 16) as Imputed Life in the Imputed Income Section, and the associated taxes are deducted from your paycheck. What s New for You? Retirement Plan Options Houston Methodist provides two retirement plans to help you prepare for retirement. Both plans are administered through Fidelity. Only the 403(b) Plan requires action. To enroll in the 403(b) Plan, visit netbenefits.com/methodist or call 800.343.0860. You can also use this site to access your information and set up your beneficiaries. If you have another retirement account and would like to roll over and consolidate your funds, Fidelity can provide help. They will be onsite at the Onboarding Events to assist. But, you must take the first step. Rollovers will not happen automatically. Enrollment Participation is voluntary. You may enroll through Fidelity starting February 4, 2014. Automatic beginning February 1, 2014, for eligible employees. Your Contributions Up to 60% of your allowed compensation, up to the IRS contribution limit of $17,500. If you turn age 50 by December 31, 2014, you can elect an additional $5,500 catch-up contribution. Allowed compensation includes base salary, overtime, shift differentials and call time. You do not contribute to the DCP. Houston Methodist s Contributions Vesting On the first 4% of your contribution, Houston Methodist will match half the amount (up to 2% of your allowed compensation). Your contribution is matched every pay period, so the sooner you sign up for the 403(b), the sooner you can start receiving the Houston Methodist match! Employee contributions are immediately 100% vested, so the money you contribute to your own plan is always 100% yours from the day you start. Houston Methodist matching contributions vest after three years of service.* After the close of the plan year, typically March of the following year, Houston Methodist will make an annual contribution based on your continuous service date (which reflects your years of service) and base salary. A REWARDING JOURNEY Paid Time Off Houston Methodist recognizes that what you do on a daily basis requires hard work and commitment. While the work we do is critical, your personal time away from work is essential. Our Paid Time Off (PTO) program is designed to help you achieve balance and encourage you to take time for yourself so you are able to Lead your Life. You earn PTO on a bi-weekly basis. Your PTO accrual rate is based on your PTO Category, Years of Service and Standard Hours. There is no restriction on the amount of PTO you can accumulate in your PTO Balance. For your Annual PTO Accrual schedule, see page 29 of your 2014 Benefi ts Guide, found in your Welcome Packet. What s New for You? Accrue PTO based on Standard Hours No limit to the amount of PTO you can accumulate in your PTO Balance Earn PTO for Houston Methodist holidays separate from your biweekly accruals Two types of PTO sell programs Important! You will start with a balance of zero PTO hours. But don t worry, CHRISTUS will cash out your remaining PTO Balance and include it in your last CHRISTUS paycheck on February 6, 2014. We suggest holding on to these funds in the event you must take an unpaid day off before you have a sufficient PTO Balance with Houston Methodist. Holiday Benefits Eligible Houston Methodist employees receive up to eight PTO hours for each of the following holidays: New Year s Day Labor Day Good Friday Thanksgiving Memorial Day Day after Thanksgiving Fourth of July Christmas Day Regular, non-exempt employees who work Thanksgiving Day, the day after Thanksgiving, Christmas Day or New Year s Day will be paid time and a half (holiday premium). If you work a holiday, your holiday hours will be awarded and remain in your PTO Balance. Also, if Christmas Day or New Year s Day falls on a Tuesday, you will receive hours for both Monday and Tuesday! The same principle applies if Christmas Day or New Year s Day falls on a Thursday. The Friday after will also be awarded as a holiday. Holiday premium will not be paid for these extra holidays. Selling Your PTO Hours After your first year with Houston Methodist, you will be eligible to participate in one or more of our four opportunities to sell PTO throughout the year. Navigating Your Paycheck The Pay Cycle Starting February 1, 2014, you will transition to the Houston Methodist payroll. You can expect to receive your paycheck on the same bi-weekly schedule you do now; however, Houston Methodist will deposit your pay on Thursdays instead of Fridays. Bonus Structure, continued Hand Hygiene Program an annual bonus linked directly to one of Houston Methodist s critical quality and safety standards keeping your hands clean! Keep your hands clean by washing your hands and/or using hand disinfectant before and after contact with a patient or patient s environment. This behavior reduces hospital-acquired infections and helps maintain the health of our patients and employees. Houston Methodist has a team of secret shoppers that observes employees to measure participation. As part of the Hand Hygiene Program, eligible employees can earn up to $150 in 2014! Management Incentive Plans (MIPs) an annual bonus for Houston Methodist management employees based on a set of shared financial, quality and patient satisfaction results. To ensure we are all working toward the same goals, the patient satisfaction and quality metrics correspond with that of the Patient Satisfaction Bonus and the Hand Hygiene Program. The Compensation Department sends monthly performance updates to management employees. We will provide additional details about MIP during the New Manager Orientation course. Focal Point Review (FPR) Cycle a time to review employee performance and organizational results and to reward eligible employees for results achieved. The review cycle begins with performance evaluations and ends with the delivery of merit increases. The cycles are defined by employee type and typically take about three months to complete. A Note about Shift Differentials Non-exempt employees in eligible positions who work the evening and/or night shifts may be eligible for shift differential compensation. Shift Timing Transition from 2nd to 3rd Shift Second shift or evening shift differential Third shift or night shift differential Starts at 2:45 p.m. and ends at 11:15 p.m. Starts at 10:45 p.m. and ends at 7:15 a.m. Employees working second shift who stay past 11:15 p.m. will receive third shift differential beginning at 11:15 p.m. You will move to the Houston Methodist shift differential policy. GETTING PREPARED In-Services Completing your in-services is an important part of your orientation to the Houston Methodist culture, procedures and environment.
