GE Healthcare OEC does not provide training to independent service organizations or to contracted individuals. The training programs in this package are only available to direct and full time employees of the hospital or medical facility using GE OEC products. TRAINING PROGRAM DESCRIPTION: The GE OEC Training Program is designed to provide the hospital Biomedical or Clinical Engineer with the knowledge and skills necessary to properly maintain OEC equipment. You receive world-class instruction from a team of experienced Technical Trainers who have spent years working with our devices. All our courses include in-depth theory with ample hands-on lab and system troubleshooting time. The course will provide all necessary Planned Maintenance (PM) activities, applicable hardware and software adjustments and system alignments. Each topic is supported with in-depth labs designed to reinforce the theory covered. Your knowledge and skills are again reinforced and evaluated with troubleshooting events, daily interactive discussion, and comprehensive exams. PREREQUISITES for 9900 Elite Courses: Two-week Course: No pre-requisite. This class provides c-arm (generator) and workstation theory, PM, all system alignments/calibrations, troubleshooting, and software installation. One week Course: Requires previous attendance to a 9800 Level 2 (two week) surgery factory course. This course provides workstation theory, PM, and software installation, but does not cover the generator or workstation. PREREQUISITES: The individual attending GE OEC equipment training must have a solid understanding of the basic fundamental principles of X-ray imaging. GE OEC equipment training courses are designed with the idea that all students attending have at least one year X-ray imaging experience in addition to the following: Good understanding of digital and analog electronics Good understanding of microprocessors and computer circuits Good understanding of MS-DOS operating system Good understanding of basic X-ray imaging principles *NOTE - All participants should bring applicable Personal Protective Equipment (PPE), including all employer-provided personal dosimetry to ensure accurate exposure collection. Labs are conducted on functional, ionizing-radiation producing equipment. Our facility is a secured location, ID badges will be issued and will need to be visible for the duration of the trainee s stay. Student receives a Certificate of Completion Student receives Continued Education Units (CEU) *. Continued Education Units are a nationally recognized standard of achievement designed to acknowledge and provide a record of an individuals continuing education accomplishments. These credits and certificates will be issued upon the completion of all hands-on and written tests given during class. TUITION POLICY: Tuition fees are due one week before the first day of class. Hotel accommodations are included in all customer tuition fees. (See Hotel section of this package for more information.) Please see the training schedule at the end of this package for course dates, length and tuition fees. Training purchased with a system must be completed within 12 months to guarantee enrollment at the negotiated rate. Training credits redeemed after 12 months will be charged the then current pricing. If training is not completed within the applicable time period GE Healthcare s obligation to provide the training will expire without refund. CANCELLATION & SUBSTITUTION POLICY Student substitutions can be made at any time No cancellation fee if GE OEC receives the cancellation request at least 21 working days prior to the class start date. A cancellation fee of 20% of the course tuition will be charged if GE OEC does not receive the cancellation notice within 21 working days. 100% cancellation fee after 14 days. GE OEC Reserves the right to cancel classes with < 3 occupants. Attendees will be Notified 21 days in advance of cancelation by GE OEC. HOTEL ACCOMMODATIONS & TRAVEL ARRANGEMENTS: GE OEC will make your hotel reservations upon course registration. You will be staying at the Marriott Residence Inn, which is located less than a half mile from GE OEC. The Salt Lake City International airport is approximately 1-1/2 miles from the hotel. The hotel provides free shuttle service to and from the Salt Lake City airport. The hotel also provides shuttle service to and from the GE OEC training facility (upon request). A rental car is recommended since GE OEC Medical Systems is located in a business park with few restaurants and clubs within walking distance. This hotel is a non-smoking only location. GE OEC pays for your hotel accommodations (room & room tax only) according to the following schedule: All hotel reservations must be made through GE OEC. COURSE REGISTRATION: Registration is on a first come first serve basis. To register, you must complete the registration form located in the back of this package and mail or fax it to the Training Registrar, along with the following documents. Registrations cannot be taken over the phone. Your tuition fee will be applied to the next available class or fully refunded if requested in a timely manner (see Cancellation and Substitution Policy above). GE OEC reserves the right to cancel or reschedule training classes. You will be notified if a class is canceled or rescheduled. Every attempt will be made to give you as much notice as possible. CONFIDENTIALITY OF GE OEC MEDICAL SYSTEMS, INC. INFORMATION (Non-Disclosure Agreement) Effective August 1, 1997 The undersigned hereby acknowledges that GE Healthcare OEC, Inc. informs me that in connection with my proposed activities, GE Healthcare OEC will disclose to me certain information relating to medical diagnostic equipment developed and manufactured by GE Healthcare OEC that constitutes GE Healthcare OEC confidential proprietary information. I hereby agree as follows: 1. For purposes of this agreement, "Confidential Information" shall mean all technical information relating to medical diagnostic equipment disclosed to me by or on behalf of GE Healthcare OEC, either in writing or orally. Confidential information shall not include (a) information already in my possession at the time disclosed by GE Healthcare OEC, (b) information that is now or later becomes part of the public knowledge or literature as a result of any action or inaction on my part, or (c) information received by me other than from GE Healthcare OEC or on GE Healthcare OEC behalf where disclosure of such information does not violate any obligation of confidentiality of any other person to GE Healthcare OEC. 2. I will not disclose confidential information to third parties without GE Healthcare OEC consent and will take reasonable steps to prevent disclosure of confidential information to third parties by my employees and agents (if any). 3. This Non-Disclosure Agreement allows the undersigned to use the Confidential Information solely to perform service work on GE OEC products installed at the Hospital listed below, or Hospitals within the Hospital s system or network of affiliated Hospitals (must be owned by the same corporation). I agree that I will not actively engage in service work on any GE OEC products installed at other institutions other than the one listed below, or Hospitals outside of the Hospital s system or network. In the event that my employment with the Hospital listed below is terminated, I agree also to not service any GE OEC products for a two-year period following my termination. 4. This agreement shall be binding upon and insure to the benefit of the successors and assigns of GE Healthcare OEC and shall be binding upon my executors, administrators, heirs, successors and assigns. I agree that, in the event of any breach of this agreement by me, money damages would be inadequate to compensate GE Healthcare OEC. GE Healthcare OEC may seek and obtain any equitable relief available to it, including but not limited to restraining orders and injunctions. If GE Healthcare OEC is successful in any action or proceeding it brings to enforce or interpret this agreement, GE Healthcare OEC shall be entitled to reasonable attorney's fees in addition to any other relief to which GE Healthcare OEC may be entitled.
