6) Indicate any extracurricular activities or other accomplishments that you would like to have considered. Example: Community Service. This could be in school or out of school. Include participation in Fine Arts, etc. It is strongly suggested all students write a short essay, outlining accomplishments, future goals, community service, and planned area of study. You may also talk about why you need financial assistance. Include any information that will help the reader get to know you. Include this with the application form as it gives organizations who choose the recipient(s), a snapshot picture of the applicant. You only have to complete one general application form to be considered for all awards; however CUPE, THE ROYAL CANADIAN LEGION and the RUTH HARVEY AWARD require additional applications. Please see Ms. Offutt, Ms. Bryant or Mr. Russell for these. 7

STATEMENT OF NEED If you are attending school out of town, it is not unreasonable to expect to spend a minimum of $15,000 for your First year. Below, briefly describe any circumstances regarding financial need of which we should be aware. Example: You have another brother or sister in university. The information provided here will be strictly confidential. It is strongly recommended that you include an introductory essay discussing who you are, what you plan on doing, School achievements, extracurricular activities, community involvement, and any extenuating circumstances for financial need. The essay will be copied and included with your application when given to sponsors. The essay is sometimes the deciding factor. You must complete this application and make an appointment (very brief) with Ms. Offutt, Ms. Bryant or Mr. Russell to go over this information to make sure it is complete when you hand it in. NOTES: a) Students need to be aware that if they receive an award administered by Prince Rupert School District #52, they will be required to submit their Social Insurance Number with their proof of registration. This is so award payments are reported on the T4A. b) By signing this application, I am also giving the awards coordinator permission to photocopy my application and provide general grade point averages for sponsors who request it for selection purposes. CHSS 2015 SCHOLARSHIP & BURSARY APPLICATION PROCEDURE The CHSS 2014 Scholarship & Bursary Booklet will be available online at http://sd52.bc.ca/chss/ under Grad info by April 10, 2015. It is designed to present the financial awards made available by a number of Prince Rupert organizations and service clubs to graduating students who are planning to attend a postsecondary institution. These organizations are to be thanked for their generosity and for their recognition of the real need that exists for all students who embark on further education. The following page of this booklet provides some indication of costs that may be incurred at a post-secondary institution. These figures have been provided by your counsellor and are intended as a guide only. Check post-secondary websites for updated costs. The financial awards described in this booklet have been divided into three categories, as follows: Awards: Bursaries: Scholarships: presented upon fulfilment of specific criteria as outlined in each award description. presented on the basis of fulfilment of criteria and demonstrated financial need. presented on the basis of academic achievement or combined academic, athletic and citizenship achievements. Students applying for bursaries must present proof of financial need (statement of need). Students applying for scholarships must present a transcript of secondary grades as well as a short written description of their athletic and/or leadership qualities when these are specified in the scholarship applied for. 1) All students wanting to be considered for awards must fill out the application form with supporting documents and attach a list of awards for which they are applying. 2) Please make sure to set a time to see Ms. Offutt, Ms. Bryant or Mr. Russell in order to review and clarify all information on your application. 3) Deadline for application is April 29, 2015. 4) Scholarship, bursary and award recipients will be determined by the Scholarships and Awards Committee or some organizations will choose their recipients and notify the school. 9

PRINCE RUPERT BETA SIGMA PHI The Beta Sigma Phi Sorority of Prince Rupert proposes to provide bursaries of $600.00 to high school graduates of Charles Hays Secondary School during the spring convocation. The following are the criteria upon which this award may be granted: 1. The candidate(s) must be a graduate of Charles Hays Secondary School. 2. The candidates must be a daughter or son or legal ward of an "active member in good standing" of a Prince Rupert Beta Sigma Phi Chapter for at least three consecutive years immediately prior to the bursary application. Proof of legal guardianship may be required. 3. The candidates must be proceeding to a recognized University, College or Technical School. 4. The bursary must be applied for by November two years following graduation (eg. graduated in June 2014, bursary must be applied for by November 2015). 5. Confirmation of registration in a recognized Learning Institution (see 3 above) for a minimum of 30 days is required from the institution before monies will be distributed. 6. The bursary will be distributed within 30 days of verification of registration as outlined in Criteria #5. 7. Eligible students may apply for this award by completing and submitting the general application form provided in this booklet. 8. A cover letter is to be submitted along with the general application form. 9. Recipients will be verified by a Beta Sigma Phi bursary committee. NOT OFFERED THIS YEAR 15

Bob Quast Memorial Award The Bob Quast Memorial Award has been established to provide an award of $750.00 annually to the successful candidate. Criteria: 1) The candidate must be a graduate of Charles Hays Secondary School. 2) The candidate must have enrolled in a post-secondary institution and the award will be provided upon receipt of registration at a recognized university, college, technical school, or emergency services academy (firefighter, police, paramedic). 3) This award must be used within 24 months of graduation. 4) The candidate must have achieved a C+ or higher in every grade 11 & 12 course in which they were enrolled (candidates for this award are disqualified if they have dropped any grade 11 or 12 course more than halfway through its completion). 5) The candidate must be strongly dedicated to at least one extracurricular activity. 6) The candidate must possess an exemplary record with respect to attendance and punctuality. 7) The candidate must display excellent citizenship at the school and/or community level. 16

FILIPINO-CANADIAN ASSOCIATION OF PRINCE RUPERT The Filipino Canadian Association of Prince Rupert will provide $1000.00 in award money to a student or students (divided equally if more than one is eligible) graduating from Charles Hays Secondary School. The following conditions must be met before this award may be granted: 1. Preference will be given to the son, daughter, or dependent of a member in good standing and active in the association and with the residency of three consecutive years. 2. The candidate/candidates must have enrolled in a post-secondary institution and submit a copy of the registration and receipt of tuition to the Association within a year. 3. The school officials will recommend students and the Board of Directors of the Association will decide on the recipient/recipients based on the pre-set criteria. 17

HOSPITAL EMPLOYEE S UNION AWARD The Prince Rupert Local of the Hospital Employee s Union is sponsoring a $250.00 award at Charles Hays Secondary School. Criteria: 1. Preference will be given to dependents of H.E.U. members. If no dependent applies, then graduating students entering post-secondary training in the health care field will be considered. 2. The candidate must be a graduate of Charles Hays Secondary School or Pacific Coast School. 3. Payment of this award will be made upon confirmation of enrollment and proof of registration in a post-secondary institution. 4. This award must be used within twelve months of graduation. 5. The Prince Rupert Local of the Hospital Employee s Union will be involved in the selection process. 6. This award must be used within 24 months. Eligible students may apply for this award by completing and submitting the general application form available with this booklet. 18

KNIGHTS OF COLUMBUS COUNCIL 5282 Knights of Columbus Council 5282 wish to advise that they will award two bursaries to the graduates of Charles Hays Secondary School for 2014. 1. Two - $500 bursaries (five hundred dollars) Pre-requisites 1. Applicant will graduate from Charles Hays Secondary School in Prince Rupert. 2. Successful graduation from Grade 12. 3. Applicants will continue their education in a recognized post-secondary institution. Classification Criteria 1. Evidence of service to local community (1-10 points) 2. MAKING HEALTHY CHOICES - AWARDS for DRUG-FREE YOUTH Community Prevention Education Continuum (CPEC) and the Prince Rupert Sikh Missionary Society have joined to provide $1000 in award money to two Grade 12 graduating students ($500 each) in the Prince Rupert area who have made the decision to be drug, tobacco and alcohol free during their high school years. CPEC is designed as a sustainable community-lead, police assisted, comprehensive youth drug prevention strategy (da.bc.rcmp.ca). The smart and healthy choice to avoid the use of illegal substances, focus on academics and healthy school and community activities is a choice worthy of commendation. To be eligible for these awards, students must meet the following criteria: 1. Have not used alcohol, tobacco or illegal drugs during their lifetime; 2. Write a one page essay (type written and double spaced) that describes specifically how you have lived and modeled a drug free life; why you made the choice to be alcohol and drug free; and what role youth can play in drug free prevention efforts; 3. List in detail specific drug prevention activities you have been involved in and list your extracurricular activities both in school and in your community; 4. THE ROGER OBATA ACHIEVEMENT AWARD (2005-2014) The Friends of Roger Obata Committee (Toronto, Ontario), is pleased to sponsor one award of $1000.00, to be given for the next ten years to one CHSS graduating student. Different recipients are to be selected each year if the criteria is met. Mr. Obata, a Canadian of Japanese descent, was born in Prince George, British Columbia, in 1915 and was raised in Prince Rupert. He never forgot his early education and friendships made and often mentioned them in his memoirs, interviews and conversations. Mr. Obata, a successful businessman, received a degree in engineering from the University of British Columbia, and served with the Canadian Army during the Second World War. He is known for his outstanding contributions to the Japanese Canadian community over a period of 50 years, especially in the area of human rights. He was a valued member of the strategy team that signed the historic Redress Agreement with the Government of Canada in 1988. Mr. Obata was named to the Order of Canada in 1990 for his human rights activities, and was an example for all Canadians who love justice and freedom. Prince Rupert Chapter Canadian Parents for French-Prince Rupert Chapter (CPF-PRC) provides bursaries for School District #52 Grade 12 French Immersion student graduates and also students who have successfully completed Core French 12, and who are entering post-secondary university, college, trade, vocational or technical education. BURSARY AMOUNT: dependent on the available funds and on the number of applicants. The maximum is $500 per student. Candidates must: 1. Currently be a member or a dependant of a current member of Canadian Parents for French (CPF) prior to bursary application. Length of CPF membership and contribution to CPF activities will be factors in prioritizing recipients for available funds. 2. Be a Grade 12 French Immersion Program graduate, or have completed Core French 12, in School District #52. 3. Complete and submit the general bursary application form in the School District #52 graduation booklet in the designated month of your Grade 12-graduation year. 4. Provide a covering letter stating why you should be considered for a bursary, including your goals and aspirations and how you have worked to promote the French language within the community. Demonstrate financial need in your cover letter. 5. We must receive from your recognized accredited post secondary education facility, a confirmation of both your registration and a statement of your attendance for a minimum of 30 days before November of the year following your Grade 12 graduation. The selected student will have taken one or more of the following courses: Drama 9 and/or 10 Theatre Performance 11 and/or 12 Directing & Scriptwriting 12 Peer Tutoring (in a drama class) The selected student will have participated in two or more of the following: The school musical/major fall production Drama festival Fine arts revue Community theatre This award will be given to a student who plans to continue his/her education in a post-secondary institution (including college, university, performing arts school, technical or trade school) within two years of graduation, and who is likely to continue to be involved in theatre in the future. CHARLES HAYS SECONDARY SCHOOL PARENT ADVISORY COMMITTEE BURSARY The Charles Secondary School Parent Advisory Committee will be providing 4 bursaries of $600.00 each to graduating students. The successful candidate will display admirable citizenship through school spirit, sports or club activities and community and family involvement. PRINCE RUPERT PORT AUTHORITY BURSARY 1. Details of Award a) Two bursaries in the amount of $1,000 each to be offered to graduates of Charles Hays Secondary School. The bursary is tenable for the first year at any college or post-secondary (non-university) institution. b) The purpose of the bursary is to encourage and financially assist high school graduates in the Prince Rupert area to obtain a trades, technology or business career. 2. Conditions of Award a) Applicants must have successfully completed a secondary school program necessary for admission to a college of the Applicant s choice, before commencement of the college. b) Before payment of the bursary, the successful candidate must be accepted and enrolled in the college of his/her choice. c) The bursary is payable in two equal installments. The first installment of $500 will be paid after the Authority receives proof of registration. The second and final installment of $500 will be paid after the Authority receives proof of registration for the recipient s second semester. 3. Acceptance of Other Awards Winners of the Authority bursary will be permitted to hold additional scholarships and bursaries, and to receive similar assistance for the same period from other sources. 4. Selection of Bursary Winners A committee approved by the respective high schools will select the bursary winners. The main factors governing the selection will be college admission ability as determined by the selection committee. 62

SEAFEST JIM CICCONE MEMORIAL 3 ON 3 BASKETBALL CHALLENGE BURSARY The Prince Rupert Special Events Society is sponsoring one $500.00 bursary to Charles Hays Secondary School of School District 52 annually, as funds allow. The funds are a result of the Jim Ciccone Memorial 3on3 Basketball Challenge that is held annually during the Seafest community festival. This bursary is in respect of Jim Ciccone s contribution of volunteering for his community. The Jim Ciccone Civic Centre was named in his memory in 1996 in recognition to his long and outstanding service to recreation, his service as an elected alderman (city councillor), as a director on the board of the All Native Basketball Tournament, and his many fine achievements as a homegrown athlete and coach. Jim Ciccone was born in Prince Rupert in 1952 and graduated from Prince Rupert Secondary School. He received provincial honours as a basketball player for the Rainmakers and went on to play basketball for the Simon Fraser Clansmen. Criteria: 1. The candidate must be a graduating student from Charles Hays Secondary School in the current year. 2. The candidate must have been an active player of a senior Charles Hays Secondary School basketball team, during grade 10, 11 or 12. 3. The candidate must have been a volunteer for a minimum of two (2) Prince Rupert Special Events Society community festivals in the previous 12 months. The amount of volunteering time will determine the selection of the candidates. 4. The Prince Rupert Special Events Society will confirm the candidate s participation with the community festivals to the CHSS bursary selection committee prior to the student s nomination. 5. The candidates must write a paragraph expressing what basketball has meant for you, what your most memorable game was and why it was, and why you would like to receive this bursary. 6. This bursary must be requested within sixteen months of graduation, by November 1 st of the year following the Grade 12 graduation. Example: Graduate of June 2014, the latest you can request your bursary is November 01, 2016. 7. Candidates must be graduates of Charles Hays Secondary School. 2. Candidates must enroll in a post-secondary institution and take courses of study for at least one year's duration, and MUST ATTEND IN THE YEAR OF GRADUATION. 3. NOTE: for grade 12 students who were in the ACE IT PROGRAM. We have made a special consideration. IF YOU WILL NOT BE ATTENDING SCHOOL IN SEPTEMBER OF YOUR GRADUATING YEAR due to the trade hours that may be required to continue, we are prepared to assist you with the cost of the books, tools and clothing you had in this program. On receiving actual receipts, we would present you with a cheque which would represent the first half of a bursary payment. You would have to attend school the following January to receive your bursary balance. 4. Financial needs of the candidates will be a primary consideration when granting this bursary. 5. TD CANADA TRUST BURSARY TD Canada Trust proposes to provide $2000.00 in bursary funds. The following are the criteria upon which this bursary may be granted: 1. The candidate must be eligible to receive either a Dogwood or a School Completion Certificate from Charles Hays Secondary School. 2. The candidate must meet eligibility criteria as outlined in the Ministry of Education Special Education Services Manual. 3. The recipient must be furthering their education in a field of their choice beyond high school. This may include technical and trades which include hair dressing, first aid, etc. 4. THE THOMAS PILFOLD MEMORIAL BURSARY The Thomas Pilfold Memorial Bursary, established by the members of the International Longshore and Warehouse Union (ILWU) Local 505, will be awarding 2 bursaries in the amount of $1000.00 each. 1. The candidates must be a graduate of Charles Hays Secondary School and/or Pacific Coast School. 2. First preference will be given to dependent children covered under the Health and Benefit plan of Local 505 members. Second preference will be given to grandchildren of Local 505 members. WILLERS MEMORIAL SCHOLARSHIP On the recommendation of the Employee Committee, Ridley Terminals Inc. will sponsor an annual scholarship to two (two) students graduating at Charles Hays Secondary School or Pacific Coast School, to be awarded as follows. Ridley Terminals Inc. offers two (2) Bernard Willers Memorial Scholarships of Two Thousand Five Hundred Dollars ($2500) each, to be awarded annually to the students who meet all of the following conditions: 1. Each award shall be made to the dependent of an RTI Team Member with the highest academic standing amongst eligible dependents at Charles Hays Secondary School or Pacific Coast School and who satisfies conditions 2. and 3. below, OR where no RTI dependent is eligible at one of the institutions, the awards shall be made to two RTI dependents at the other institution, who satisfy conditions 2. and 3. below, OR where less than two RTI dependents are eligible, the award shall be made to the student with the highest academic standing who meets the conditions set out below and who, in addition, is not the recipient at the same time of any other individual award in excess of Five Hundred Dollars ($500.00). 2. Is graduating in the year of the award. 3. Intends to pursue a full time course of study at a post-secondary institution, beginning within twenty-four months of graduating. 4. The amount of the award will be paid by RTI to the selected recipient on presentation of proof of registration at a post-secondary institution within the period specified. A resident of Prince Rupert and District; 2. Must be enrolled in and provide proof of registration for a second term of studies at a post-secondary institution within the first two years of graduating from high school; 3. Must be studying an area of the arts: dance, creative writing, music, drama or visual arts. 4. The Community Arts Council will be involved in the selection of the recipient. 5. PRINCE RUPERT PORT AUTHORITY LEGACY SCHOLARSHIP The Prince Rupert Port Authority will award two annual scholarships of $5,000 each to two grade 12 graduates. Preference will be given to those applicants pursuing a career that will support the transportation industry and would include such disciplines as international business, trade, logistics, marketing, engineering, communications, finance, accounting, law, etc. Preference will also be given to those candidates that demonstrate a desire to practice their chosen profession in Northern British Columbia communities following post-secondary graduation. Criteria: 1. Applicants must graduate successfully from Charles Hays Secondary School. 2. Provide evidence of strong academic achievement. A copy of his/her secondary school transcripts must accompany the application. 3. Provide evidence of school and community involvement. This evidence must include some tangible aspects of social responsibility and commitment of the applicant to service in his/her community. 4. (A) An essay must be submitted with a maximum limit of 800 words. The topic will be the following: Transportation is the Foundation of Society Today. The essay should include insightful analysis and research. (B) A brief explanation of the reasons for the applicant s chosen field of post-secondary endeavours as well as the rationale for desiring to pursue one s career in Northern British Columbia, should be included with the application. 5. Candidates may be requested to participate in an interview with the adjudication committee. 6. Payment of this scholarship will be made upon proof of registration in an accredited postsecondary school. Proof of registration will include a tuition receipt and a confirmed timetable of courses. The $5,000 amount will be disbursed in two installments - $2,500 for the first semester (September to December) and $2,500 for the second semester (January to April) consecutively. Proof of registration must be provided for BOTH semesters. 7. The recipients must be going to a post-secondary school in September of his/her graduating year and must be attending the institution for a minimum of one full-time academic year. 8. PRINCE RUPERT REGIONAL HOSPITAL SCHOLARSHIP Prince Rupert Regional Hospital will make available the Prince Rupert Regional Hospital Scholarship. Scholarship standing will be of prime importance in the awarding of the scholarship. The Prince Rupert Regional Hospital Scholarship of $500.00 is available to a student eligible for graduation from Charles Hays Secondary School, Pacific Coast School or to a former graduate of the high schools. The candidate must be enrolled in a program in the health care field at either a community college or a university. SCHOLARSHIP On the recommendation of the Board of Directors, Ridley Terminals Inc. will sponsor an annual scholarship to two students graduating from Charles Hays Secondary School or Pacific Coast School, to be awarded as follows: Ridley Terminals Inc. offers two (2) Ridley Terminals Inc. Scholarships of Two Thousand Five Hundred Dollars ($2500) each, to be awarded annually to the students who meet all of the following conditions: 1. The award shall be made to the student with the highest academic standing who meets the conditions set out below and who, in addition, is not the recipient at the same time of any other individual award in excess of Five Hundred Dollars ($500.00). 2. Is graduating in the year of the award. 3. Intends to pursue a full time course of study at a post-secondary institution, beginning within twenty-four months of graduating. SHOPPER S DRUG MART SCHOLARSHIP Shopper s Drug Mart, Prince Rupert, has donated the following scholarship. The annual Shopper s Drug Mart Scholarship has been established for graduating students of Charles Hays Secondary School. It is intended to help offset expenses the students may incur in pursuing their post secondary studies. This scholarship will be given to a student who has shown an aptitude for the Health Sciences, and who is planning to take a program of studies in the Health Sciences. A student going into Pharmacy would receive the highest consideration. The student must be intending to pursue an educational program of eight or more months in duration at any recognized Post Secondary Educational Institute or Training School within 18 months of the date of the award. This scholarship will be awarded each year at the Charles Hays Secondary School Graduation Ceremony. The scholarship is worth $500.00. One half of the annual scholarship will be awarded to the recipient upon proof of registration, and the other half will be awarded if the student is still registered in the second semester. DOGWOOD DISTRICT/AUTHORITY AWARDS (DISTRICT SCHOLARSHIP PROGRAM) APPLICATION Beginning in the 2006/2007 school year, the District Scholarship Program name has been changed to the Dogwood District/Authority Awards. The allocation of these awards has increased from approximately 500 awards to 3000 awards pro-rated to school districts/authorities, based on district enrolment. The award amount remains the same at $1000 distributed in two parts ($500 cheque and $500 voucher to be used towards post-secondary education). These awards are intended to help students in furthering their post-secondary education. Purpose: The Dogwood District/Authority Awards are available to students who demonstrate superior achievement in any of the following areas: Fine Art (Visual Arts, Dance, Drama, Music) Applied Skills (Business Education, Technology Education, Home Economics) Physical Activity (not limited to Physical Education) Second Languages (including Aboriginal Languages) with IRPs School/Community Service The school selection committee also will consider the following factors when determining the recipients for these awards: Attendance Punctuality Work habits Cooperation and positive contributions to the school and community. NORTHWEST COMMUNITY COLLEGE SCHOLARSHIP Northwest Community College offers 2 annual entrance Scholarships of $2000.00 to high school graduates from publicly funded schools in Prince Rupert. Selection will be based on the highest GPAs obtained by Entrance Scholarship applicants from each of the above communities. The applicants school and community contributions will be considered. The following conditions apply: A minimum GPA of 3.00 on all grade 11 and 12 courses shown on the BC secondary school transcript. Applicants are expected to graduate from grade 12 in the year that they enter the college. A deferral of up to one year may be requested.
