Let s prepare for your appointment When you arrive at Carolina Eyecare Physicians to meet your doctor for the first time, there are a few things you should bring with you: The paperwork you have been given to fill out We know that sitting in a reception area filling out forms can be a hassle. It s so much easier to recall all of the details you need to answer those questions when you can fill out the forms at home and bring them with you.

We try to provide these forms in advance of your appointment because we want to make your visit as pleasant and hasslefree as possible. If you have not received forms from us, don t worry. They are also available on our website, where they can be downloaded to your home computer and printed out no hassle! If you have not filled out your paperwork ahead of time, it s a good idea to arrive 15 to 30 minutes early to allow time to take care of this. Your insurance card and photo I.D. It will take us only a moment to make copies of these for your file. Most people use a drivers license as the photo I.D. Someone to drive you home after your appointment If possible, you should not drive yourself home because your eyes may be sensitive to light for several hours after your examination. We want you to arrive home safely!

YOU will receive a very thorough, comprehensive eye exam Your examination will include a wide variety of very hightech tests. We will be using eyedrops to dilate your eyes (make the pupils larger) so the doctor can see the inside of your eye. You can expect to be at our office for approximately 1-1/2 to 2 hours. Your doctor will explain the results of your examination, any recommendations for treatment, and will take the time needed to answer all of your questions. We want YOU to ask questions but we realize that it can be difficult to ask the right questions when you don t know much about how the eye works. That s why we have provided a wealth of information about the eye, how it works, eye problems and eye procedures and it s all available on our website, CarolinaEyecare.com. The more you know, the better questions you will be able to ask and the more helpful your doctor s answers will be!

After YOU see the doctor At the end of your appointment, you will be escorted to the check out area. If you need to return to the office for treatment or a follow-up visit, we will help you make that appointment during the checkout process. Carolina Eyecare Physicians accepts Medicare assignment and participates in most insurance plans. Payment for the portion of the fee not covered by insurance is expected at the time of checkout.
