The rooms on the lower level approved for child care meet the 35 sq. ft. requirement for each child. Lower Level: Living Room and Kitchen The bathroom is located on the lower level. The home has adequate fire exits. The lower level utilizes a window as a second exit. The home does not contain a wood burning stove or fireplace. Pets in the home include 2 small sized dogs, 1 large sized dog, and 1 cat. The outdoor play area is fenced and is located at the back of the house. There are no nearby water hazards. There are no other obvious outdoor hazards. There is adequate age appropriate play and learning equipment. The group child care home will operate M-F 6:30a.m to 5:00p.m and will care for children of all ages. Ms. Halfacre was previously licensed to provide child care under a different license number (DG380080475). She has completed the required First Aid and CPR training. She is not a licensed foster parent. There have been no rule variances granted. 2
III. DESCRIPTION OF FINDINGS & CONCLUSIONS The facility is in compliance with all applicable rules and statutes except for the following: R 400.1905 Training. (1) The caregiver shall complete not less than 10 clock hours of training each year related to child development, program planning, and administrative management for a child care business, not including CPR and first aid training. Ms. Halfacre lacked 10 clock hours of training each year. R 400.1941 Heat-producing equipment. (4) Furnaces, other flame or heat-producing equipment used to heat the home, and fuel-fired water heaters shall be inspected by any of the following entities: (a) A licensed heating contractor for a fuel-fired furnace. Ms. Halfacre lacked an updated furnace inspection. R 400.1941 Heat-producing equipment. (1) Operable smoke detectors approved by a nationally recognized testing laboratory shall be installed and maintained on each floor of the home, including the basement, and in all sleeping areas and bedrooms used by children in care. The sleep area lacked an operable smoke detector. A corrective action plan was requested and approved on 05/14/2009. It is expected that the corrective action plan be implemented within the specified time frames as outlined in the approved plan. A follow-up evaluation may be made to verify compliance. Should the corrections not be implemented in the specified time, it may be necessary to reevaluate the status of your license. IV.
