Department of Health and Family Welfare ONLINE DISABILITY CERTIFICATE GUIDELINES FOR APPLICANTS

The Disability form is designed to fill up the personal details of the people suffering from various types of physical disability and is not cent percent perfect to perform their activities. This disability certificate is to be issued by the medical centers or hospitals located at different districts of the State to which the respective candidate belongs to. The basic purpose of issuing this certificate is to provide relaxation to the disabled candidate in every field may it be education or any professional stream in order to bridge the barrier. Here is a guideline, so as to instruct how to fill the form so as to avoid any error and rejection of the same. Enter the URL http://www.odisha.gov.in/disabilityportal of the respective site in the menu bar to browse and display the following Home Page of the application- 2

In the above displayed screen, click the Application Form link which redirects the user to the following page For first time users, click on the Click here link to register. The following registration page will appear, where the applicant would enter his details for registration. After the registration is successful, applicant would receive his Login Credentials. 3

Now click on the Applicant login link Now click on the Continue to login link and enter your credentials 4

After Successful login, your Status page would be displayed, Then click on the Apply for Certificate link and continue to fill the form, 5

Basic information page In the above displayed disability screen, the first section provides the space to fill-in the full name of the applicant- Enter the Surname, First Name and Middle Name of the applicant in the respective textboxes provided as they appear in the certificates. In the next section, enter the Father s and Mother s Name of the applicant in the respective textboxes provided. Select the applicant s Date of Birth in the DDMMYYYY format from the drop down list. Select the applicant s gender from the Sex list. 6

For uploading the photograph and signature of the applicant, go to the Photo section. There are two options for uploading photo- if there is webcam facility in the system, then choose the Webcam radio button. On doing so, the system camera gets activated and the applicant snap can be clicked and saved there itself. In case there is no webcam facility, then the user can choose the File radio button. On doing so, the user can upload the scanned copy of the photograph from the file where it is saved. For uploading signature, the same has to be scanned beforehand and saved at a particular location. Click the File radio button and browse for the signature where it is saved. On doing so, the signature gets uploaded in the specified space. Personal Details page 7

In the Personal Details column, select the Caste of the applicant along with his/her Blood Group from the respective drop down list. Select the Occupation of the applicant entering the name of the Identification mark (if there is any). In the Address for Correspondence section- Select the name of the District to which the applicant belongs from the drop down list. For the selected District, choose the name of the Block/ULB along with entering the PIN code of the location. Enter the Address Details providing the Telephone no. of the locality with the code. Provide the name of the Street/Sahi, Village/City, Police Station and Post Office where the applicant resides. Provide the e-mail address and Mobile no. of the applicant for communication. In the Permanent Address section- If the Permanent Address is same as that correspondence address. then select the checkbox for the same. Accordingly, the data gets filled-up in the respective fields, else fill in the location details for the Permanent address. In the Details of Educational section- 8

Select the name of the Highest Qualification attend by the applicant from the drop down list. In the Disability Information column- Select the radio button either for Single or Multiple disability For Single disability, Primary and Community health center is applicable and accordingly the same health centers get populated when single is chosen. For Multiple disability, District Health Center is applicable. Select the type of Disability Category under which the respective applicant is covered along with selecting the Period since when the applicant is suffering from the same i.e. either Birth or Year. If Year is selected, then enter the year since when the disability attacked the applicant. Click the Add button to include additional disability information. If ever the applicant has applied for issuing the disability certificate, then choose Yes radio button else choose No. 9

If Yes is selected, then select the name of the Authority to which the applicant has applied for the certificate. Select the name of the District to which the applicant belongs along with the Status of the application from the respective drop down list. On previous application, if the applicant is already been issued with the disability certificate, then choose the Yes radio button and upload the scanned copy of the certificate. In the Document Details column- Select the Document Type that can be uploaded for address verification. Select the Date of Issue of the respective document. Accordingly click the Choose File button to browse and upload the scanned copy of the document. After filling in all the relevant details, click the Submit button. Confirmation Page 10

A confirmation screen is displayed where the Username/Registration No and Password generated. These credentials are provided to the applicant which has to be restored for future use. The user can generate printout of the application form for future reference by clicking the Print Applicant Form. For authentication, the applicant has to login into the system using the credentials received either through SMS or online using the candidate login tag as provided in the default screen. For checking the status of the application, the candidate can login using the credentials allotted to him/her through the user space. For user login, click the Login button at the top right corner of the screen which displays 2 options. Select the Applicant Login as highlighted below- 11

On doing so, the user is redirected to the Applicant Login screen- Enter the applicant s User Name or Registration No. and Password as generated after submitting the application in the respective textboxes provided. Click the Login button. 12

The user is redirected to the Form Activity page after login. To modify and update the candidate s disability details, click the Edit ( disability data can be modified only within 48hrs of submitting the application. ) option. The After checking all the details, click the Logout option to exit the page. Re-apply Case In case, the application is rejected at the time of evaluation by the physical committee, the respective applicant can re-apply for the disability certificate. For Re-applying, click the Reapply option in the home page as shown below- 13

After choosing the Re-apply option, the login screen is displayed- In the above displayed screen, the applicant can login using his/her credentials as generated on submitting the application and click the Reapply option. For re-applying, no more verification is required. The page is directly redirected to the apply screen. Similar process is to be followed for the Re-apply case as in new application. But applicant can only edit their Photograph, Signature, necessary documents and disability information. 14
