09/04/2014 MEDGEN EHR Release Notes: Version 6.2 Build 106.6.22 Phone: 516-466-3838 Fax: 516-466-3877 Email: medgensupport@comtronusa.com Please find below recent updates that were made to your Medgen EHR system: Navigation: Health Maintenance has been renamed to Clinical Decision Support. Throughout the program you will see this label change. Specifically, you can see this under the Patient Information section of the patient s chart: to Home Screen: From the main home screen the "Message Center" menu option will now display the number of unread messages:. This value will automatically update every minute when new messages are received from another user or patient. The number of unread messages will be visible no matter what portion of the program you are currently in. Transition of Care has been added under the Utilities menu Added ability to quickly access patient s medication list from User Inbox lab review When a patient appointment row is selected on the home screen the system will automatically reselect the row when the screen is refreshed instead of bringing the user back to the top Patient Chart: "Clinical Reconciliation" section has been added to the Patient Information section New patient education and information functionality has been added to the Problems, Medications and Lab Results/Reports in the patient s chart. It is represented with the icon. The tool has been added to obtain educational information about patient s medications, problems and lab results. This feature is also accessible directly from the encounter form and through the patient portal. Demographics: Users now have the ability to set two different types of categories for a patient The patient s categories will visible in various parts of the program whenever the patient is accessed, such as "Open Chart" Patient consent flag added to Demographics page. NOTE: Patient medication history check may not be performed if this flag is set to "No". Patient marital status added to Demographics panel

Patient category icon is now visible when scheduling a patient button added to demographics panel to quickly view location of patient address The SNOMED code list has been incorporated into the Medgen EHR system. The SNOMED code list can be used to represent certain procedures, orders, or diagnosis that are associated with the patient. They may also be used to document completion of certain Clinical Quality Measures (CQM). SNOMED entry can be found on the Demographics page of the Patient Chart under the Demographics Menu: as well as on the Encounter Form menu bar Patient Reports: Under Demographics Patient Reports Patient Information can now be e-faxed Under Demographics Patient Reports Patient Summary Sheet can now be e-faxed Under Demographics Patient Reports Patient Chart now can be e-faxed Summary of Care record can be generated for a patient's chart under Patient Reports and sent to another provider who is using a certified EHR technology, using DIRECT email. DIRECT is the new transmission method adopted by CMS to handle all transfer of electronic patient chart information between providers. If you do not currently have a DIRECT email account and would like one please contact a Medgen representative. Super bills can now be e-faxed. Patient Portal: A patient portal account can now be created without the need to enter a patient email in the demographics section of the patient's chart. Patient Portal account information and login credentials can now be printed and handed to the patient instead of emailed. Printing the patient portal credentials will also count towards Meaningful Use. You now have the ability to create sub-patient portal accounts for an authorized representative of the patient (i.e. spouse, parent, caretaker). The sub account will be able to access the patient portal just like the patient account and also have the ability to send and receive messages.

Patient Recalls: We have redesigned the patient recall entry screen under Demographics Recalls Under Reporting Schedule Reports Patient Recalls, the ability to send e-mail notifications for patient recalls has been added along with printing recall letters. These notifications will count towards Meaningful Use. E-mail templates for patient recall notifications can be generated by using the note template editor under Setup Notes & Forms Notes and Letters. E-mail appointment reminders can be sent directly for an individual patient under the patient s chart Demographics Pharmacy Search: Patient Pharmacy search window has been updated to make pharmacy searching easier and more flexible. This includes having the pharmacy list automatically re-filter as soon as a field is entered without the need to click the "Search" button. button added to pharmacy search to quickly view location of pharmacy Allergies: Allergies are now viewable from the medication search screen when adding a new medication in Patient Information Medications as well as from the Encounter Form For allergies that have been inactivated you now have the ability to mark the allergy as "Entered in Error" to clearly define that this allergy was entered in error and should not be part of the patient chart. Problems: Date Diagnosed field has been added to patient problem entry Medications: option added to medication search list so users may specify which medications they would like to show as default when medication search list is initially loaded. Clinical Reconciliation: Users now have the ability to upload CCDA (Continuity of Care) documents to a patient's chart. A CCDA file is an electronic version of a patient s chart in the form of an XML file and contains information including but not limited to patient s allergies, medications and problems. Using the new Clinical Reconciliation module in Medgen, users may upload these CCDA files and merge the contents of the file into an existing patient's chart. If a patient is referred to your practice from an outside provider Clinical Reconciliation can be used import valuable data about the patient without the need for manual entry. CCDA may be received from an outside provider who is using a certified EHR technology (CEHRT) through DIRECT email. If you do not currently have a DIRECT email account and would like one, please contact a Medgen representative. Performing Clinical Reconciliation counts towards the Meaningful Use measure of Medication Reconciliation.

Scheduling: Daily view in the schedule can now show up to 9 providers side-by-side as opposed to the previous limit of 7. Patient category icon is now visible when scheduling a patient Added ability to export appointment list to excel from Reporting Schedule Reports Appointment List When a patient appointment is switched to the status "Canceled", the system will prompt for a reason why the appointment is being canceled. Encounter Form: Referring provider and Copy to provider may now be viewed and modified from the encounter form under the Encounter Panel. Patient SNOMED entry has been added to the bottom encounter form toolbar. Problem Template selection has been added to the bottom encounter form toolbar. Image dictionary is now accessible directly from the encounter form Image Panel Free text instructions may now be added in the Instructions Panel of the encounter form instead of uploading or selecting a document. "Past Physical Exam" has been added to note assistant to quickly retrieve physical exam sentences from past encounter forms and either replace or append to the current encounter form. "Past Test Results" has been added to note assistant to quickly retrieve physical exam sentences from past encounter forms and either replace or append to current encounter form Note assistant will no longer refresh when selecting between different plan fields in the encounter form. Drop down option functionality has been added to Save Form, Sign Incomplete, Sign Complete on the encounter form for quicker use. You now have the ability sign and create a note or sign and close the chart with a single click.

Problem Templates: User can now force a new line (carriage return) to display between selected note sentences by prefixing the sentences with the following character "- "(dash + space). This can be used to enable sentences within your encounter form to display like this without the need of manually typing the "Return" key on the keyboard: - Sentence one instead of Sentence one, Sentence Two, Sentence three - Sentence two - Sentence three Custom Patient List: The following fields have been added to the Custom Patient List filter Patient weight Patient blood pressure Patient SNOMED codes Category 2 Appointment Provider Appointment Office Transition of Care: Using this new module, providers can upload / receive CCDA (Continuity of Care) documents. A CCDA file is an electronic version of a patient s chart in the form of an XML file and contains information including but not limited to patient s allergies, medications and problems. Once received, a patient's chart can be generated directly from the CCDA file if it is a brand new patient. If the patient already exists in your practice the information from the CCDA file can be reconciled and merged into the chart. CCDA files may be received from an outside provider who is using a certified EHR technology (CEHRT) through DIRECT email. If you do not currently have a DIRECT email account and would like one, please contact a Medgen representative. CCDA documents may also be uploaded from this module. Setup: A Specialty Form library has been incorporated into the Medgen system. Users now have the ability to import a pre-designed specialty form from the Medgen library. This can be done from Setup Notes & Forms Encounter Forms Specialty Form An Image library has been incorporated into the Medgen system. Users now have the ability to import preloaded images from the Medgen library. This can be done from Setup Notes & Forms Note Images Image Library Referring Provider Dictionary mailing labels will now print with a provider prefix. Ability to filter items by time has been added to the audit trail filter under "Utilities" Under Procedure Setup, users have the ability modify procedure charge, modifiers, and NDC for a specific procedure Security Access: "Copayments" report has been added to the Reporting & Utilities section of security access You now have the ability to restrict Clinical Decision Rules based on user levels. If set as "No Access" this clinical decision rule will not be visible to that user in a patient's chart

Allow / View Only / No Access security levels can now be set by specific encounter forms as well as an option for "All Encounter Forms" A separate section has been created for "Prescriptions" under security access to control access for prescribing, printing and e-scribing medications. Security Access options in regards to prescribing, printing and e-prescribing medications can now be defined on a provider specific level.

Please know that the Medgen staff considers all suggestions / updates given to us by our clients and we try our best to implement as many of these features as possible. Please do not hesitate to make recommendations through our suggestions module, but we do ask for your patience when it comes making changes to the program. We will be adding more updates to this release up until the final release date so be sure to review all final release notes at that time. Thank You. Special Note: Meaningful Use is now required! If Medicare eligible professionals do not adopt and successfully demonstrate meaningful use of an EHR technology by 2015, the EP s Medicare physician fee schedule amount for covered professional services will be adjusted down by 1% each year. If you have never attested for Meaningful Use it is very important that you complete this in year 2014. Providers should contact their labs to set up an interface with our Medgen EHR software. Having a lab interface is a requirement for Stage 2 meaningful use and could take time to be setup. Please contact your laboratory representative to begin this process as soon as possible. You may give them our contact information when making your request. Continuous submission to your state s immunization registry is a requirement for Stage 2 meaningful use. Please contact the immunization registry for your state and follow their procedures to create your account to perform submissions. All information given to you from your state s registry should be forwarded to Medgen support so that it may be setup on our end. Please know Stage 1 Meaningful Use only required a test submission of a single immunization message however Stage 2 requires continuous production messages. If you have a testing account with your states immunization registry you should begin working on moving your account to production status. Medgen Mobile will be here soon! For our Premium subscription members we are pleased to announce the release of Medgen Mobile in the coming months. Using this new mobile app, designed by Comtron, providers and staff will be able to access patient chart information and appointments directly through their smart phones. Please review future news releases from the main Medgen EHR log in page for more information. EPCS Electronic prescribing of controlled substances will soon be mandatory in some states. Please know that Comtron is currently in the process of getting the Medgen EHR e-prescribing system certified for the prescribing of controlled substances and we will have this feature ready ahead of the scheduled mandate. PQRS reporting is now required! Providers who do not report PQRS measure to Medicare will be penalized for their 2015 Medicare reimbursements. Do not lose money. Please visit the CMS website to learn more information about PQRS. ICD-10 testing will begin soon! Comtron has already been preparing for the transition to the ICD-10 code list in October 2015. During the remainder of this year and 2015 we will be testing with the various 3 rd party organizations that we communicate with.
