e-mds Charting the Future of Healthcare Interoperability Presenters Michael Stearns, MD, CPC, CFCP President and CEO Board certified neurologist Over 10 years in health care informatics Integral in development of SNOMED-CT Certified Professional Coder Kip LeCrone Director, Information Exchange & Integration Services 7 years experience in health care interfacing Certified in HL7 v2 1

e-mds Solution Series Product Information Fully integrated EMR and PM software ONC-ATCB Certified Complete Ambulatory (MU) 2010 Best in KLAS Award for Ambulatory EMR & Practice Management (2-5) 3 time #1 in AAFP Satisfaction Survey 3.5 stars in ACP survey Topics Brief Product Description / Awards Participation i in IHE/HIMSS HIE Trends XDS + PCC Profiles Interoperability Challenges Vocabulary Future Steps 2

e-mds Participation in IHE/HIMSS e-mds 4 th year participating in IHE Connectathon and HIMSS Interoperability Showcase 1 st year at Leadership Level Presentation will focus on current movements of e-mds, challenges experienced in the industry, and plans to overcome such issues to achieve a success story that makes use of multiple IHE profiles Why IHE? Why HIMSS? IHE does not create new standards from scratch IHE utilizes existing standards that are best suited for particular use cases No need to shop around and compare competing standards HIMSS brings IHE profiles together to tell a story, illustrating how the profiles can be combined to achieve optimal coordination of patient care 3

Profiles to Watch For Users of Ambulatory EHR Software ATNA for secure transmission of information and auditing of events PIX/PDQ for patient identity management XDS for sharing documents PCC framework profiles for building appropriate p clinical documents XDS-MS for Medical Summaries Etc. HIE Trends HIEs are immediately solving the tangled web of transactional messaging 4

HIE Trends Now what? HIE Trends 1 st Stage: create a centralized hub Lab, Radiology, and Transcription results/reports Orders Demographics 2 nd Stage CCD from EMRs to HIE via XDS Future Stages CCD from HIE to EMRs via XDS Expand CCD support to other clinical documents 5

Existing or Planned HIE Implementations XDS + PCC Profiles XDS is catching on & adoption is growing Focus must also be paid attention to the content that is shared CCD is a great start, but what about the intent? Why is the CCD being shared? PCC profiles take the Clinical Document Architecture (CDA), which the CCD derives from, and adds purpose and intent: Referral Summary ED Referral Discharge Etc. 6

PCC Profiles This is a placeholder that may be removed, or may be expanded to detail various PCC profiles, and to emphasize the importance of supporting a variety of clinical documents. XDS Access to documents within a community. Highly scalable with Gateways y 7

XDS + XCA XDS along with XCA offer cross community access Cro Cross Gateway XDS + XCA Even more scalable with Gateways supporting XCA Cross Gateway 8

Challenges Data integrity Multiple vendors feed into an HIE Format and the quality of data must be enforced Common vocabularies Many systems are still using Local vocabulary that must be mapped Vocabularies suited for billing purposes, which typically are not clinically granular enough Privacy & Security Granular and Agnostic Vocabulary Next Generation of e-mds Software Rather than map from one vocabulary to the next Core Clinical Concept A generic core vocabulary set of concepts can be used to map to any vocabulary necessary. ICD-9 ICD-10 SNOMED CT 9

Vocabulary Placeholder This slide may be deleted or expanded to discuss vocabulary in more detail. Charting the Future of Health Care Interoperability e-mds will be targeting HIEs ready to embrace bi-directional clinical document sharing e-mds will work with these exchanges on universally recognized vocabularies Next year, e-mds intends to bring specific success stories to HIMSS in order to show how these concepts can be implemented in the real world 10
