MU Core 11 Generate Patient Lists This guide will provide instructions on how to configure the necessary setup and preferences t o enable Enterprise EHR to work for Core Measure 11: Generate a list of patients based on specific conditions to use for quality improvement, reduction of disparities, research or outreach. Objective is shared across Eligible Professionals: Yes: No: Objective must be recorded during reporting period: Yes: No: Prerequisites Requires: Patient Query (formerly known as Population Health Management): Availability v11.2: Enhanced v11.4.1: Configuration Steps Perform the following steps to generate patient lists within the EEHR: Identify the criteria in which to create the patient lists Create query and action sets in Patient Query 1. Navigate to TWAdmin > Patient Query.
2. Click the button to create a new query. 3. Enter the title of the new Query Name within the field on the bottom left of the page and then click the yellow cross icon to open the Clinical Rules Editor. To meet the measure you must select a population in your practice with a specific condition to improve upon, reduce disparities, conduct research or outreach.
4. In the Clinical Rules Editor choose the patient query criteria by clicking on the Criteria links from the list on the left side of the screen. Example: Demographics Criteria Fill out required/appropriate fields and click OK. 5. When finished selecting the criteria click Save and Return -. 6. Close the window and navigate back to the Clinical Rules Editor window. 7. Click the Refresh All icon. This will ensure that you will see your newly created query.
8. Find and highlight the query and click the Run icon and ensure the query successfully ran. 9. Click the Return to UMP link (upper right corner of screen). 10. Navigate back to TWAdmin > Patient Query and select the Add/Edit Patient Action Set tab.
11. Create a new Action Set, by clicking the button. 12. Name the new Action Set and provide a description. 13. Click on the Patient List tab and choose the Patient List Type.
14. Select an Existing List or create a new Patient List Name. 15. Provide Patient List Description and click the button. 16. Select the Action Set Properties tab and associate the new Patient Query to the Action Set by clicking the binoculars icon. This brings up the Associate Patient Population Queries Search.
17. Select the new Patient Query and click the button. 18. Click the button. 19. Click the button.
Perform the following steps to assign patient lists to users: 1. Navigate to TWAdmin > Pt List Admin > Manage Lists tab 2. Select the appropriate provider from the drop down menu, select the patient list from the Available Lists window and click the button. Workflow Considerations If the providers and clinical staff will not access the Patient Lists within the EHR, c reate internal processes to provide the Eligible Provider and clinical staff access to the patient lists. Where will it be accessed? Will it be a shared file? Will it be securely emailed? How often will the file be updated?
Create internal processes for your providers and users to be able to request specific Patient Lists. How EEHR Calculates the MU2 Report This core measure does not have a calculated report. It is a Yes / No attestation.
