Step 1 Chabot College Nursing Application Process Advanced Standing/Transfer/LVN s/program Re-Admission A Chabot College Application for Admission¹ must be submitted to the college. This application can be submitted electronically by going to http://www.cccapply.com and is available year round. Advanced Standing Students should make an appointment with a counselor for a preliminary evaluation of your transcripts for graduation requirements and acceptance as pre-requisite courses for the program. Step 2 Complete Nursing Advanced Standing Application¹ on-line or download application. Nursing Advanced Standing / LVN RN Applications are accepted April 1 st through October 1 st of each year. Once the Advanced Standing application has been submitted you have 2 years to complete the ATI exams, theory classes and/or skills testing and be ready for admission to the full time program. Step 3 Print, sign and MAIL the Nursing Advanced Standing Application (Pages 3-6) to: Chabot College, Admission and Records 25555 Hesperian Blvd. Hayward, CA 94545 It is recommended you obtain a Certificate of Mailing and Return Receipt from the U.S. Postal Service at the time of the mailing. Include one copy of your official transcripts from all previously attended schools and colleges. Hand carried transcripts or application will not be accepted. LVN applying for 30 unit (non-degree) option must also submit high school transcripts. Suggestion: Order and keep a copy of your transcripts in your personal education file. Step 4 Go to on-line to http://www.chabotcollege.edu, click Class-Web and logon, click Student Services, Student Records to verify receipt of transcripts. Due to the volume of applicants we are not able to notify students individually if any information is missing. Step 5 Admission and records will evaluate your transcripts. When complete the Advanced Standing Application and transcripts will be forwarded to the Nursing Program Director for a final review. You will then be notified by mail regarding your eligibility and courses required for you to enter and complete the program. Please allow 6-8 weeks for this process to complete. Step 6 AFTER receipt of letter from the Nursing Program Director, students wishing to enter the program must complete all courses or testing as outlined in their individual letter. Step 7: LVN RN Transition Course students must attend mandatory orientation listed on webpage. In August, LVN s will be required to enroll in Nursing 70/N70L, LVN RN Transition Course. This course is a review of nursing topics including nursing care plan, nursing process, communication principles, and legal/ethical concepts common to the role of registered nurse. It will explain the functions of the California Board of Registered Nursing and selected psychomotor skills utilized by the registered nurse in the delivery of patient care. ¹Any falsification of data on the application(s) will result in immediate disqualification.
Chabot College Nursing Program Advanced Standing Order of Program Acceptance Ranking 1-5 Advanced Standing status is granted to students who have previously completed any portion of the defined Chabot College nursing curriculum or its equivalent as determined by the Nursing Program Director. (Please see Re-Entry Policy and Procedures for details. Testing: Students who are seeking Spring Program Admission must have basic skills and ATI testing completed by November 15th; Final Skills exam by December 15 th. If space is limited, students will be ranked by November 15th. In December the list will be evaluated to determine if standards have been met and re-ranked if necessary. Students who are seeking Fall Program Admission must have basic skills and all ATI testing completed by April 1 st ; Final skills exam by May 15. Incoming students will be evaluated in January of each year to set ranking and again on April 15 th of each year to determine if standards have been met and re-ranked if necessary. Ranking: Admission is based on space availability. Applicants will be ranked according to the following: 1. Waitlist from previous semester Student must have completed all ATI testing, all skills testing and all nursing program classes. Students who have been waitlisted from the previous semester will have to repeat all skills testing, as these tests are only valid for 6 months. 2. Re-admission Chabot program students that left the program for issues other than failing academic standing. a. Will be ranked by GPA from nursing program course grades only. 3. Entry Level LVN Students completed N70 and N70L and basic skills by April 1 st, final skills exam by May 1 st. a. TEAS Score b. GPA from N70 and N70L c. Completion of ATI testing on first attempt or second attempt. d. GPA for N88 and N88L and N82, N81 if taken. 4. Re-admission Chabot program students who withdrew due to failing academic performance will be readmitted using GPA based on nursing program course grades only. a. Student will be ranked by GPA from program course grades only. b. Ties may be broken with skills exam scores and /or TEAS scores. 5. LVN Non-Degree Students and LVN Students (30 Unit Option) must complete N70 and N70L, basic skills by May 1 st. Advanced Standing and LVN * Registered Nursing Application www.chabotcollege.edu/nurs To be completed by LVN s and Re-Admission Chabot Students to the Chabot Nursing Program All applicants are advised that space availability is limited to openings that occur after the first day of the Fall Semester of the first year of the Chabot Nursing Program. These openings are created only when someone leaves the program or declines admission to the program. Every effort is made to tailor the applicant s educational needs to the available spaces as they occur. Some placements may take longer than others, depending on the individual applicant s needs. A background check is required on all nursing students. We will review the following on everyone accepted to the program: Social Security Trace, Misdemeanor or Felony Criminal Conviction Search, Combined OIS.GSA exclude parties/debarment List Search and a Pre-placement Drug Screen. If you have been convicted of a misdemeanor of felony it is possible you will be denied placement in one or more of our hospitals that are used for clinical experience or the Board of Registered Nursing (BRN) may not grant you a license. Please contact the BRN or make an appointment for a confidential meeting with the Nursing Program Director for guidance in this area. Admission Requirements: All students must take and pass the Test of Essential Academic Skills (TEAS ) Version V with Adjusted Individual score of 62.0%. Student must have been previously enrolled in an accredited Registered Nursing Program within the last two years. Student must have an overall GPA of 2.0 or higher. Science grades must be 2.5 overall. Licensed Vocational Nurse (LVN) must be currently working and hold a current State of California LVN License. LVN must attend a mandatory orientation in December of May to apply to the LVN-RN program. o LVN must have a minimum of one year experience or 500 hours. Submit a letter from employer on employer letterhead stating job title and start date of position. Pre-Requisite Requirements: the student must have a minimum in 4-5 Semester Units with a laboratory component of each of the following courses: Anatomy 1 Physiology 1 Microbiology 1 Overall GPA is 2.5 with only one W OR failing grade from any of the three course. All science courses must be completed prior to submitting this application. Completion of one term of Anatomy/Physiology combined course WILL NOT satisfy this requirement. Space Availability; students are placed into the full time program when space only if space is available. Students will be ranked by type of student (LVN, Advanced Standing, Re-admission), ATI scores and theory grades etc. See Ranking Information for complete listing. If student completes all testing and space is not available a waitlist will be established. All candidates accepted into the program as Advanced Standing Students will need to demonstrate theoretical and clinical competency through written and clinical performance examinations for Fundamentals of Nursing, Nursing Care of Children, Maternal Newborn Nursing, and Pharmacology assessments. A fee is charged for each exam and is the responsibility of the student. Any prospective applicant needs to understand that Chabot Colleges Advanced Standing Program has a single admission policy. If any student does not received a C or better in all the transition N70/N70L or a nursing course N88/N88L, N84 or she/he will be dismissed from the transition program and is not eligible to reapply for this program. This applicants must complete requirements and be admitted to the full time program within two years of completing the application..
